Insufficiency fractures of the sacrum after radiotherapy for gynaecological malignancy.

An entity is described which is characterized by low back pain and increased radionuclide uptake in the sacrum at bone scintigraphy in postmenopausal women having received radiotherapy towards the pelvis because of gynaecological malignancy. The findings stimulate bone metastases, but are in all likelihood caused by insufficiency fractures of the sacrum promoted by bone weakness induced by postmenopausal osteoporosis and radiotherapy combined. The increased radionuclide activity has a characteristic appearance which in the fully developed fracture acquires the shape of an 'H'. In plain radiography, changes are absent or subtle. The proper complementary examination is CT, in which the fractures can be visualized and malignant changes excluded. Awareness of this benign entity is important to avoid overdiagnosis of bone metastases.

Family interaction and metabolic balance in juvenile diabetes mellitus. A prospective study.

Family interaction was assessed in 30 children with insulin-dependent diabetes mellitus (IDDM) of at least 2 years' duration using video-tapes of standardised family tasks. The relationship between measures of family interaction and metabolic balance (HbA1) was investigated 1 and 5 years after the task performance. No significant relationship between family interaction and HbA1 was found after 1 year, but HbA1 values 5 years after the family assessments were significantly higher in the children who belonged to families in which family interaction was considered disturbed (P less than 0.05). If family interaction was used as a predictor of metabolic control, 67% of the cases were correctly classified. In the group with disturbed family interaction HbA1 increased during the observation period in 44% of cases while in the group with functional interaction this occurred in only 11% of the cases. When the children were divided according to age, significantly higher HbA1 values were found only in the younger group (less than 18 years at the 5-year evaluation) of the children of families with disturbed family interaction (P = 0.025). Here a correlation between disturbed family interaction and HbA1 was found (rs = 0.46, p = 0.03). In conclusion, certain family interaction patterns seem to influence metabolic balance in IDDM in children during adolescence, but not during pre-adolescence and early adulthood.

Blood flow in thigh muscle during bicycling exercise at varying work rates.

16 male subjects exercised at 25, 50, 75, 90, 100 and 120% of VO2max on a von Döbeln bicycle ergometer. The muscle mass was measured in a whole body counter. Muscle blood flow (MBF) estimated from the rate of 133Xe clearance from m. rectus femoris showed a levelling-off at about 0.5 1 of blood per min and liter of muscle tissue (equal to an irrigation coefficient of 0.5 min-1) at work rates above 50 to 60% of VO2 max. This concurs with clearance data from the literature. However, when MBF is calculated from VO2, muscle mass, and reliable values for a- vo2 differences, MBF in the present subjects would: 1. Not level off before 90 to 100% VO2max, 2. reach a value of 1.0 min-1. The underestimation of MBF calculated from 133Xe clearance and the levelling-off shown by this method may be due to a systematic error inherent in the method, the 133Xe clearance being diffusion limited at high flow rates.
