The effects of preexercise stretching on muscular soreness, tenderness and force loss following heavy eccentric exercise.

The present study sought to investigate the effects of preexercise stretching on delayed onset muscle soreness (DOMS), i.e. soreness, tenderness and loss of muscle force, that usually occurs after strenuous or unaccustomed eccentric exercise. Ten female volunteers performed 10 sets of 10 maximal isokinetic eccentric contractions for knee flexion with both legs after a 5-min ergometer cycling warm-up. Prior to the exercise for one leg, randomly chosen, 4 x 20 s of static stretching for the hamstring muscle group was implemented. Rated soreness, tenderness on algometer pressure and loss of maximal eccentric contractile force was evaluated preexercise and 24, 48 and 96 h postexercise. The exercise bout produced severe DOMS, with parameters peaking and troughing at 48 h postexercise. However, no significant differences were found, regarding any of the parameters, when comparing stretched and nonstretched legs. The present study thus suggests that preexercise static stretching has no preventive effect on the muscular soreness, tenderness and force loss that follows heavy eccentric exercise.

Phase Equilibria and Structure of the 1-Dodecylpyridinium Bromide-Dodecane-Water System.

The isothermal ternary phase diagram for the 1-dodecylpyridinium bromide/dodecane/water system was determined at 40 degreesC by 2H NMR and polarizing microscopy methods. Two liquid crystalline phases, a large cubic area and a normal hexagonal phase, and one isotropic normal micellar solution phase were characterized, and their ranges of existence were determined. The micelles were found to be probably small and spherical at lower concentrations of surfactant, and were found to grow at higher concentrations and on addition of oil. The two-phase areas, L1 + HI and HI + I, are both very narrow. The comparatively large cubic area, containing 43-63 wt% surfactant and 3-10 wt% dodecane, is probably consistent of more than one structure. Repeated maximum reciprocal knee movements in patients with minimal overt symptoms after ischaemic stroke: an evaluation of mechanical performance and EMG.

The ability to perform reciprocal knee flexions and knee extensions was investigated in patients with minimal or no overt motor symptoms after stroke. Ten patients and 22 controls performed 10 maximal reciprocal knee extensions and knee flexions without intervening rest period using an isokinetic dynamometer (Cybex II). Peak torque (PT; Nm), signal amplitude (RMS) and mean frequency (fmean) of the electromyography were registered for each extension and flexion separately and as the ratio extension/flexion. The patients exhibited pronounced motor deficit despite no or minimal overt clinical symptoms. The reduced motor capacity in the knee muscles was shown as a decrease in the PT and in a different electromyographic pattern compared with the controls. A bilateral affection was found with the formerly hemiparetic side mostly affected. Repeated reciprocal contractions influenced the motor performance, shown as an increase of the PT ratio in the patients, which was especially pronounced with an increasing number of contractions.

Ternary Phase Equilibria of the 1-Dodecylpyridinium Bromide-Dodecanol-Water System.

The isothermal ternary phase diagram for the 1-dodecylpyridinium bromide (1-DPB)-water-dodecanol system was determined at 40 degreesC, using 2H NMR, polarizing microscopy, and SAXS methods. All of the phases were characterized, and their ranges of existence were determined. The surfactant is easy to dissolve in water, yielding a normal micellar solution phase. After the normal micellar phase, on the binary surfactant-water axis, a normal hexagonal liquid crystalline phase is found at higher surfactant concentrations. On addition of dodecanol, four more phases are formed, i.e. a cubic, a lamellar, and a reverse hexagonal phase, followed by a reverse micellar solution phase. The lamellar liquid crystalline phase dominates the ternary phase diagram. The structures of the liquid crystalline phases were further examined using SAXS measurements, and the results are discussed in terms of the critical packing parameter, cpp, and electrostatic forces. The SAXS experiments show a pronounced swelling of the rods in the hexagonal phase, from 28.5 to 33 Å on addition of dodecanol, whereas the cylindrical aqueous core of the reverse hexagonal phase has a diameter of 18-21 Å, depending on sample composition. The average bilayer thickness of the lamellar phase is about 24 Å. Copyright 1997 Academic Press.

Skeletal muscle fatigue and endurance in young and old men and women.

The effects of increasing age on skeletal muscle fatigue and endurance were assessed in 22 healthy young (14 men and 8 women; mean age, 28 +/- 6 years) and 16 healthy old (8 men and 8 women; mean age, 73 +/- 3 years) individuals. All subjects performed 100 repeated maximum dynamic knee extensions at 90 degrees.s-1 (1.57 rad.s-1) using an isokinetic dynamometer (Cybex II). Peak torque was recorded during every contraction, and for each individual the maximal voluntary contraction (MVC), the fatigue rate, the endurance level, and the relative reduction in muscle force were determined. MVC and endurance level were significantly lower in old men and women, but there was no discernible difference in relative muscle force reduction and fatigue rate between young and old individuals. We conclude that thigh muscles of older individuals are weaker than those of younger individuals, but relative to their strength, older individuals have similar properties as younger individuals with respect to muscle fatigue and endurance.

No difference in blood thiamine diphosphate levels between Swedish Caucasian patients with congestive heart failure treated with furosemide and patients without heart failure.

OBJECTIVES: To determine whether furosemide treatment in congestive heart failure (CHF) patients is associated with thiamine deficiency. DESIGN: Patients without heart failure and without diuretic treatment were included to compare with patients with CHF belonging to New York Heart Association (NYHA) functional class II and III-IV, respectively, and receiving furosemide therapy. SETTING: All patients were recruited from the emergency ward of the cardiology section. Huddinge University Hospital, where they were admitted due to CHF or acute myocardial infarction. SUBJECTS: Ninety-nine patients were included from whom a blood sample was taken, as well as routine admission blood samples for the analysis of thiamine diphosphate (TPP) concentrations. Patients taking vitamin preparations were excluded. MAIN OUTCOME MEASURES: Blood TPP concentrations were measured by high performance liquid chromatography (HPLC) and compared between the patient groups by the use of ANOVA. RESULTS: No significant difference was found between the groups in blood TPP concentrations. CONCLUSIONS: Thiamine deficiency may not be a complication of furosemide treatment in the studied Swedish patient population.

Assessment of postoperative pain: impact of clinical experience and professional role.

BACKGROUND: Unrelieved postoperative pain is still reported to be a rather common clinical problem which may be related to inadequate routines for pain assessment. Therefore, the aim of the study was to describe strategies used by experienced and less experienced nurses and physicians in their assessment of postoperative pain and to relate different approaches, clinical experience, and professional role to the accuracy of the pain ratings. METHODS: Data collection was based on repeated interviews with nurses (n = 30) and physicians (n = 30) in connection with clinical pain assessments (n = 180) including VAS-scoring. RESULTS: Commonly used strategies in the pain assessment were: -how the patient looks, -what the patient says, -the manner of talking, and -past experience of similar circumstances. The mean VAS-score given by the patients (6.1 +/- 1.1) was significantly (P < 0.001) higher than that rated by the staff members (4.9 +/- 1.2). Nurses as well as physicians overestimated low and underestimated high levels of pain indicated by the patients. The accuracy of the ratings by nurses, especially by more experienced ones (> or = 10 years in nursing), was found to be less precise than that of physicians. The pain assessment of these very experienced nurses was characterized by a systematic underestimation. CONCLUSIONS: The present study emphasizes a need for definition of more precise strategies for clinical postoperative pain assessment which better take into consideration the pain experiences and needs of individual patients.

Quality of life in children with congenital heart defects.

Quality of life was measured in children with congenital heart defects (CHDs) registered in a total population of infants born live in the period 1982-91 (n = 22,810), using essential life spheres: external living conditions, interpersonal and personal conditions. In 200 children with CHD alive at the time the investigation was performed, 164 (82%) of the families answered a questionnaire addressing different dimensions of these quality of life spheres. Three subgroups of CHDs were investigated: CHDs spontaneously cured (n = 80), CHDs treated by surgery (n = 56), and CHDs with associated syndromes/malformations (n = 29). 301 (75%) out of 400 controls, matched for age and habitat (county), answered the same questionnaire. The children's ages at investigation were 2 y 2 months-12 y 2 months (median 6 y 1 month). There were no statistically significant differences between the CHD groups and the controls for overall quality of life for any of the three life spheres (p > 0.05). In children with operated CHDs and CHDs associated with syndromes/malformations, quality of life was influenced at some aspects of the external as well as at the interpersonal and personal levels. A trend existed for a higher subjective experience of quality of life in the total CHD group as well as in all the subgroups. It is speculated that this may represent development of coping mechanisms and recalibration of values of life.

Does an early neonatal diagnosis of a later spontaneously closed ventricular septal defect impair quality of life?

The hypothesis that early diagnosis of a ventricular septal defect (VSD) with spontaneous closure later on may impair the parental-infant bonding process, with consequences for the child's quality of life in the longer term, was tested in 51 children born in 1986-1991 (gestational age > or = 37-42 weeks) with VSDs diagnosed in the early neonatal period and closing spontaneously during the first 24 months of life. The results were compared to 83 healthy controls matched for gestational age, time and place of birth. A method (questionnaire) taking into consideration the quality of life spheres of external living conditions and the interpersonal and personal conditions of the child was used. Except for a lower satisfaction with family network in the VSD group (p < 0.05), no differences were found between the VSD group and the controls for any of the parameters tested or for overall quality of life (p>0.05).

Intraoperative electrical stimulation of the stapedius reflex in children.

OBJECTIVE: To describe the use of intraoperative electrical stimulation on the stapedius reflex in children. SETTING: Hospital cochlear implantation center. PATIENTS AND METHODS: Eighteen children under the age of 10 years, who have undergone cochlear implantation, have been examined during surgery with the electrically elicited stapedius reflex in the monopolar stimulation mode. RESULTS: A stapedius reflex could visually be detected for all subjects. CONCLUSION: The responses serve to test the implant, provide guidance in avoiding overstimulation of the child when the device is switched on, and estimate the optimal C-levels.

Radiological videoconferencing in Sweden.

For the last four years there has been a videoconferencing link between the Karlskrona hospital and the university hospital of Lund. Because of a need to transmit angiographic films, its use has been gradually expanded to become an everyday routine, used for both elective and emergency examinations. During 1994 and 1995, 1121 cases were examined over the video link. Apart from 156 normal cases all were discussed with surgeons, most at 96 weekly conferences but approximately 50 at conferences taking place as soon as possible after the examination. Assuming that an average of three doctors attended the conferences before the video link was available, this amounts to a saving of 3 x 8 h in travelling time per videoconference. The average salary was 300 SKr/h and train tickets cost a further 400 SKr per person. The cost of the ISDN connection was approximately 1800 SKr/conference. The net gain was thus about 6500 SKr per videoconference, without taking into account the work that could be done with the doctors still in Karlskrona after the conference.

Knee extensor performance of dominant and non-dominant limb throughout repeated isokinetic contractions, with special reference to peak torque and mean frequency of the EMG.

The aim of this study was to investigate whether significant differences in peak torque (PT), mean power frequency of EMG (fmean) and perceived fatigue exist between the dominant and non-dominant knee extensors throughout repeated contractions. The present study forms part of a research project aimed at developing tests for the determination of the degree of motor control in patients with sequelae after CNS injury. A total of 22 clinically healthy subjects (14 males and eight females) took part in the investigation. The subjects performed endurance tests of the lower limbs, consisting of 100 repeated knee extensions using a Cybex dynamometer at 1.57 rad s-1. The patterns of PT, fmean and perception of fatigue throughout the endurance test were investigated. The endurance curves of PT and fmean showed, in common with other studies, an initial steep decrease followed by a stable phase. No significant differences existed between dominant and non-dominant knee extensors with regard to PT, fmean or perception of fatigue. The results indicate that it may be possible to use the contralateral knee extensors as an indicator of motor recovery in hemiplegia in patients with a high degree of recovery.
