Osteochondritis dissecans of the hip.

PURPOSE: To investigate the clinical and radiological characteristics of osteochondritis dissecans (OD) of the hip and the outcome of this condition after treatment. MATERIAL AND METHODS: Twelve male and 3 female patients with OD were retrospectively studied. Six patients had a history of Legg-Calvé-Perthes disease (LCPD) and among them 2 also had had a trauma to the hip. A further 5 had had trauma and 1 a developmental dislocation of the hip (DDH). The remaining 3 patients had no history of previous hip disease or trauma. All patients were examined with plain radiography, 7 with MR, 3 with CT and 2 with hip arthrography. RESULTS: All OD lesions were detected at plain radiography, and most of them were located near the fovea. At MR the lesions had low signal intensity at T1-weighted sequences, and 6/7 had edema or fluid collection in or adjacent to the lesion on T2-weighted sequences. The early treatment in 7 patients was surgery, 2 had had conservative treatment and 6 no treatment. At follow-up 12 years after radiological diagnosis, 5 patients had hip arthrosis, 4 of whom were treated with arthroplasty. All but 3 had reduced hip rotation and all but 2 (with arthroplasty) had load pain. Three of the patients with earlier surgery had not developed arthrosis. CONCLUSIONS: OD lesions are usually well seen with plain radiography. There is a great risk of developing early arthrosis and it seems that early surgery is connected with arthrosis development. Thus only symptomatic treatment is recommended.

Symptom criteria do not distinguish between functional and organic dyspepsia.

DESIGN: Retrospective study. SETTING: Teaching hospital, Sweden. OBJECTIVE: To find out if various diagnostic criteria could distinguish organic from non-organic causes of dyspepsia. SUBJECTS: 635 patients previously interviewed by computer questionnaire. INTERVENTIONS: Upper gastrointestinal endoscopy, laboratory tests, clinical examination. MAIN OUTCOME MEASURE: Differentiation between organic and functional dyspepsia. RESULTS: 106 patients had functional dyspepsia. Of these 83 had ulcer-like dyspepsia, 76 motility-like dyspepsia, and 50 reflux-like dyspepsia. Eight patients had unspecified dyspepsia. CONCLUSIONS: There was a considerable overlap between different subgroups, and the criteria did not differentiate between organic and non-organic causes of dyspepsia though the symptom criteria in most cases showed an independent value in discriminating between different subgroups. The clinical usefulness of the criteria remains to be shown.

A double-blind study to compare the efficacy and safety of meloxicam 15 mg with piroxicam 20 mg in patients with osteoarthritis of the hip.

Meloxicam 15 mg once daily (n = 128) was compared with piroxicam 20 mg (n = 127) in this 6 week, double-blind, parallel-group, randomized, multicentre study in out-patients with symptomatic osteoarthritis (OA) of the hip. Assessments of pain, global efficacy and global tolerance were made on a 10 cm horizontal visual analogue scale; severity of OA was evaluated by Lequesne's index. Efficacy results showed significant improvement compared with baseline, with no significant difference between meloxicam 15 mg and piroxicam 20 mg. The type and frequency of adverse events were comparable for the two drugs. The most frequent events reported were gastrointestinal (GI) disorders, occurring in 21 and 23% of meloxicam and piroxicam patients respectively. The global tolerance assessment by patients at the end of treatment favoured meloxicam. In conclusion, meloxicam at a dose of 15 mg/day is comparable in efficacy and safety to piroxicam 20 mg.

Tolerance and efficacy of Omniscan (gadodiamide injection) in MR imaging of the central nervous system.

PURPOSE: This Swedish multicenter trial was performed on patients with known or suspected lesions of the CNS for which an MR examination using a contrast medium was indicated. A total of 8 MR centers participated in the study to establish the safety and efficacy of Omniscan (gadodiamide injection) in clinical routine using a standard dose of 0.1 mmol Gd/kg b.w. MATERIAL AND METHODS: Seven hundred adult patients who had been referred for MR investigation of suspected CNS lesions were included in the study. Since most patients were examined on an outpatient basis, it was decided to use an explicit questionnaire regarding adverse events that developed within 24 h after examination. The efficacy evaluation involved comparisons of detectability, delineation, and number of lesions before and after injection of Omniscan. RESULTS: No serious or unexpected adverse event was found. There were a total of 70 (10.2%) patients with adverse events, excluding those judged not to be contrast media-related. However, only 15 patients (2.2%) had adverse events that possibly or probably were related to the contrast medium. Usually, the symptoms were headache, dizziness, abnormal taste, and nausea. Two patients complained of itching, but only one developed urticaria. The efficacy was similar to that of other currently used Gd-based MR agents. Lesions were more often seen on T2-weighted images, but the contrast medium improved lesion delineation, contributing to higher certainty in diagnosis, and provided more confidence in excluding suspected abnormality. CONCLUSION: Omniscan was found to be a safe and clinically valuable contrast medium for MR imaging of the CNS.

Konzo: a distinct disease entity with selective upper motor neuron damage.

Two Tanzanian patients with konzo were severely disabled by a non-progressive spastic paraparesis, since the sudden onset during an epidemic six years earlier. At the time of onset they had a high dietary intake of cyanide from exclusive consumption of insufficiently processed bitter cassava roots. MRI of brain and spinal cord were normal but motor evoked potentials on magnetic brain stimulation were absent, even in the only slightly affected upper limbs. Other neurophysiological investigations were largely normal but the more affected patient had central visual field defects. Konzo is a distinct disease entity with selective type upper motor neuron damage.

Fixed implant-supported prostheses with welded titanium frameworks.

Eighty-six edentulous patients provided with Brånemark implants were randomly selected for fixed prostheses with modified framework designs. This study describes alternative laboratory techniques in which premachined titanium components are welded together to form the framework. Clinical experience, following the patients for 1 year after placement, indicates that it is a predictable technique with a similar pattern of complications as experienced by patients with cast frameworks supported by implants. The prostheses are considered to be slightly more bulky than cast frameworks, but seem to have, on a clinical level, a better fit to the implants.

Osteochondritis dissecans of the elbow. A long-term follow-up study.

Thirty-one patients with osteochondritis dissecans of the capitellum humeri were followed for an average of 23 years. There were symptoms in about half of the elbows at the follow-up examination. Impaired motion and pain on effort were the most common complaints. Roentgenographic signs of degenerative joint disease were present in more than half of the elbows and correlated with a reduced range of motion. The diameter of the radial head increased in comparison with the contralateral elbow in two thirds of the patients.

Failures and complications in 127 consecutively placed fixed partial prostheses supported by Brånemark implants: from prosthetic treatment to first annual checkup.

Ninety-six partially edentulous maxillae and mandibles were consecutively treated with 127 freestanding fixed prostheses supported by 354 implants. The patients were followed for 1 year and the overall success rate was 98.6% for the examined implants. None of the inserted prostheses was lost during the observation period. The most commonly reported problems during the first year of function were related to loose gold screws and esthetic complaints, complications that were easily resolved. Furthermore, the total number of complications was low and was less than has been reported for routine full-arch fixed prostheses.

Failures and complications in 92 consecutively inserted overdentures supported by Brånemark implants in severely resorbed edentulous maxillae: a study from prosthetic treatment to first annual check-up.

Overdentures were consecutively inserted in 92 severely resorbed maxillae and followed for 1 year. The dentures were supported by a total of 430 implants. Of these, 69 (16%) became loose and were removed during the follow-up period. The mobile implants caused 7 complete failures of the overdenture treatment. Postinsertion maintenance was more extensive for the overdentures than for fixed prostheses supported by implants. Problems arose in relation to the mucosa around the implants, and there were also fatigue fractures of the acrylic resin and the retentive clips. Fewer speech problems were reported in the overdenture group when compared to those with fixed prostheses.

Osteochondritis dissecans of the ankle. A 20-year follow-up study.

Thirty patients with osteochondritis dissecans of the ankle have been followed up for an average of 21 years. The histories and radiographs were reviewed, and it was found that most patients had only minor radiographic changes and symptoms. Two patients had developed osteoarthritis but in only one was this severe. Osteochondritis dissecans in the ankle thus seems to differ from the same lesion in the knee where late osteoarthritis often occurs. Two lesions were located on the joint surface of the distal tibia, a site not previously reported.

Osteochondritis dissecans and brachymesophalangia: a hereditary syndrome.

A kindred with an unusual congenital hand and foot anomaly is presented. It is an autosomal dominant hereditary disorder characterized by brachymesophalangia, slight clinodactyly, deformed metacarpal heads, osteochondritis dissecans in metacarpophalangeal joints, absence or hypoplasia of the ulnar styloid processes, and brachyhypophalangia in the feet.

Symptomatic chondrocalcinosis at the wrist.

Twenty-two patients with chondrocalcinosis in the wrists were treated for symptoms of pseudogout. All but 3 had signs of degenerative arthropathy; 2 patients also had chondrocalcinosis of other joints in the hand. The degree of chondrocalcinosis and the grade of arthrosis varied but there was no certain correlation between the two. Attacks of arthritis were the clinical features in 13 cases. The usual treatment was immmobilization and administration of phenylbutazone.
