[DPOAE in high-risk neonatal screening for hearing].

64 neonates under intensive care have been tested by distortion product otoacoustic emission (DPOAE) and the auditory brain stem response (ABR) to investigate the value of DPOAE as a test for hearing impairment. The DPOAE test has been found to be practical and quick to perform. The proportion of NICU infants producing a recordable DPOAE is 83.6%, and the sensitivity and specificity to the ABR result is 92.4% and 92.2%, false positive is 7.8%, false negative is 7.6% respectively. These figures are high enough and the reduction in time compared to ABR is sufficient for the DPOAE to be considered as the primary screen, firm conclusions on the sensitivity of DPOAE to long term hearing impairment await the results from larger numbers of infants and further follow up data.

[Morphological studies of myocardial fibrosis in different types of Keshan disease].

OBJECTIVE: To clarify the localization, distribution, content and type of collagen in the myocardium of patients with Keshan disease (KD). METHODS: Picrosirium red staining, polarization microscopy and image analysis were used to study autopsy specimens from 335 KD patients. RESULTS: A loose network of collagen and a higher ratio of type III collagen were found in the myocardial scar tissues of acute KD patients. In the myocardial scar tissues from chronic KD patients, the collagen fibers were dense, the ratio of type I collagen was higher, the ratio of type I to type III is increased in chronic KD and decreased in acute KD. The amount of collagen in non-necrotic regions was significantly higher than that of the controls. CONCLUSION: The degeneration and necrosis of myocytes in KD is associated with disorders of collagen metabolism and with structural modelling of collagen.

[Placental site trophoblastic tumor: a study of clinicopathologic features and differential diagnosis].

OBJECTIVE: To study the clinicopathologic features and differential diagnosis of placental site trophoblastic tumor (PSTT). METHODS: 5 PSTT cases were studied and compared with 10 choriocarcinoma cases and 2 exaggerated placental site (EPS) cases by light and electron microscope examination and immunohistochemical staining. RESULTS: PSTT occurs in reproductive women and often following term deliveries. Amenorrhea and or vaginal bleeding are common presenting symptoms. The serum levels of hCG are often slightly or moderately elevated. Microscopically, PSTT is composed of only one kind of trophoblast cell, displaying extensive invasion of myometrium and vessels without obvious hemorrhage and necrosis. Mitotic figures are not common. Ultrastructure shows that the tumor cells contain prominent paranuclear filaments. Immunohistochemical reaction reveals positive staining for hPL and mostly negative staining for hCG. CONCLUSION: PSTT is a rare trophoblastic tumor with unique microscopic, ultrastructural and immunohistochemical features which can help to differentiate them from other trophoblastic diseases.

[Application of MVR-PCR at D1S8 locus with HRP-labeled MS32 oligonucleotides probes].

The purpose of this study is to detect small amount of biological materials using the method of MVR-PCR at D1S8 locus with HRP-labeled MS32 oligonucleotides probe. The results showed that the MVR-PCR patterns, obtained from trace blood stain (equal to 5 ul blood), trace saliva stain (equal to 4 ul saliva) or a hair root, can be read clearly and easily. The sensitivity of the method is 1 ng DNA. The applications of the method in personal identification were discussed.

[Secondary infection of the ovarian endometriotic cysts].

OBJECTIVE: To explore the clinic manifestations, diagnostic methods and principles of management in the secondary infection of the endometriotic cysts of ovary. METHODS: Thirteen cases of the infected ovarian endometriotic cysts were analysed retrospectively in our hospital from Jan. 1990 to Dec. 1996. The diagnosis was confirmed by the operation and pathologic findings. RESULTS: The clinic manifestations were divided into five types, namely acute, subacute, chronic, occult and stabile types. The definitive diagnosis can be made by abdominal pain, fever and adnexal mass combined with B-scan findings and chocolate-brown purulent fluid which was punctured from the adnexal mass. Operation was the main method for treatment. CONCLUSIONS: Secondary infection of the endometriotic cyst is one of the gynecologic emergency. Correct diagnosis and prompt management are of vital importance.

[Study on 102 cases of anorectal post-operational pain treated by long-acting analgesic].

OBJECTIVE: To observe the clinical effect and experimental result of long-acting analgesic in the treatment of anorectal post-operational pain. METHODS: Analysing the pathological features of anorectal post-operational pain, and select the Radix Stephana Tetrandra, which could disperse pathogenic Wind and dampness, relieve spasm and pain, as the major component. The authors extracted Sinomenine from the Stephana and by mixing with methylene blue and bupivacaine hydrochloride, an injection was prepared. Three groups were in the clinical observation: Long-acting analgesic group (102 cases), routine treatment group (80 cases) and the control group treated by Tramal (30 cases). The curative effect was observed and animal experiment was conducted. RESULTS: The total analgesic rate of the long-acting analgesia was 56.86%. The clinical effect was better than using Bupivacaini hydrochloride or Tramal alone (P < 0.05). It was shown in experiment that long-acting analgesic had obvious effects of topical analgesic and muscle relaxant. CONCLUSIONS: Long-acting analgesic has significant effect in relieving the anorectal post-operational pain.

[Study on gas exchange of the patients with lung cancer in perioperation].

OBJECTIVE: To study changes of gas exchange in perioperative patients with lung cancer. METHOD: Indices including P/F, a/A, P50O2 and PaCO2 on gas exchange in 68 male patients(50-78 y) with lung cancer were monitored on preoperative day 1 2 3 and postoperative day 7. RESULT: Comparing with preoperation data P/F a/A decreased significantly in 3 days(P < 0.01) and PaCO2 P50O2 increased significantly on the day after operation (P < 0.01), especially in the group of pneumonectomy and abnormal ventilation, hypoxemia measured in preoperation. CONCLUSION: The gas exchange of the patients with lung cancer reduced significantly in postoperative 3 days.

[A histochemical study on baicalin].

Baicalin in the root of Scutellaria baicalensis was analysed histochemically by means of TLC and HPLC. It was determined that the content of baicalin in phloem was 11.08%, in xylem 7.86%, in phellem 2.19% and in centric decayed part 0.815%. Thus the quality of root of Scutellaria baicalensis is believed to consist in thin phellem, high ratio of phloem to xylem and absence of centric decayed part.

EMG study in the diagnosis and differential diagnosis of lipid storage myopathy.

Clinical, electromyographic and pathological features were studied in 18 patients with lipid storage myopathy (group I) and 18 patients with polymyositis and dermatomyositis (group II). The results showed a remarkable lower spontaneous activity(SA) incidence (14%) in group I than that (55%) in group II; 46% and 34% short-duration motor unit potentials(MUAPs) with polyphasic potentials and 74% and 71% short-duration MUAPs without polyphasic potentials respectively; the percentages of increased polyphasic MUAPs were same in the two groups. The reduced or pathologic interference patterns accounted for 61% in the group I and 50% in group II. Increased CPK, LDH and HBD were also found in both of them. It is suggested that the lipid storage myopathy may be diagnosed when patients have muscle weakness and myalgia with short-duration and low-amplitude and polyphasic MUAPs without or with occasional spontaneous activities, and increased CPK, LDH and HBD.

[Treatment of arteriovenous malformation by linear accelerator radiosurgery].

OBJECTIVE: To summarize the effectiveness of radiosurgery for cerebral arteriovenous malformations (AVMs). METHOD: We treated with linear accelerator radiosurgery 17 patients harboring AVMs in the brain. 15 AVMs were located supratentorially and others infratentorially. All the patients were treated with single fraction. The doses at AVM margin were 15 to 30 Gy. RESULT: AVMs disappeared in 13 of the 17 patients, reduced in 3 and unchanged in only 1 at 2 year follow up. Vasogenic edema observed in 2 patients, their neurologic deficits were all improved with steroid. CONCLUSION: Linear accelerator radiosurgery is effective in dealing with cerebral AVMs. It provides with a new means of treatment for AVMs which can't be effectively treated with microsurgery and embolization.

[CD44v and nm23-H1 gene product expression and its clinical significance in human recurrent bladder cancer].

OBJECTIVE: To investigate the expression of the CD44v and nm23-H1 gene products and its clinical significance in human recurrent bladder cancer. METHOD: The CD44v and nm23-H1 gene products in 45 cases of bladder cancer were analysed quantitatively by flow cytometry and cytoimmunofluorescence staining. RESULT: The CD44v gene product overexpression and nm23-H1 gene product low expression were correlated with invasive extent and recurrence of bladder cancer (P < 0.05). The correlation coefficients between CD44v gene product and nm23-H1 gene product were gamma = -0.2876 in bladder cancer. The correlation coefficients between CD44v gene product and nm23-H1 gene product were gamma = -0.4238 in recurrent bladder cancer. CONCLUSION: The expression of CD44v and nm23-H1 gene product cooperated in growth and progress of bladder cancer. Both CD44v and nm23-H1 gene products were an objective indication of invasive extent and recurrence of bladder cancer.

[An experimental research of recombinant human epidermal growth factor on corneal wound healing].

OBJECTIVE: To investigate the effects of recombinant human epidermal growth factor (rhEGF) eye drops on corneal wound healing. METHODS: Twenty-four white rabbits were randomly divided into 4 groups, 6 rabbits 12 eyes each. Anterior keratectomy of 8 mm in diameter and 1/3 cornea in thickness was performed on each eye. Each of the following concentrations of rhEGF: 1, 10, 100 microg/ml eye drops or normal saline (control) was applied four times daily for a week respectively for one group. The wound area was determined by computer imaging analysis. RESULTS: The mean epithelial healing rate of rhEGF 1, 10, 100 microg/ml groups was 9.31, 9.96, 9.31 mm(2)/day respectively, significantly greater than 8.11 mm(2)/day of the control group. The action of rhEGF of 10 microg/ml was somewhat better than that of 1 or 100 microg/ml, and no significant difference was noticed among the three rhEGF groups. Moderate inflammation and corneal neovascularization were induced in the rhEGF 100 microg/ml treated group. CONCLUSION: rhEGF 1 - 10 microg/ml can accelerate corneal wound healing in the rabbit with no adverse side-effects. It may be used to treat serious corneal trauma and ulcer clinically.

[Study on the surface EMG pattern classification with BP neural networks].

This paper presents a surface electromyography (EMG) motion pattern classifier which combines Neural Network (NN) with parametric model such as autoregressive (AR) model. This motion pattern classifier can successfully identify four types of movement of human hand, wrist flexion, wrist extension, forearm pronation and forearm supination, by using of the surface EMG detected from the flexor carpi radialis and the extensor carpi ulnaris. The result shows that it has a great potential application to the control of bionic man-machine systems such as prostheses because of its fast calculating speed, high recognition ability, and good robust.

[Successful media exchange and network communication of CT digital image].

The digital image standard in medical is discussed. Based on the DICOM standard by using the media exchange and establishing the network communication between PC and CT system, we successfully acquired the CT original digital image and convert it to DICOM standard image. The CT image can easitly be read and adjusted on the PC platform.

[The mechanisms of antioxidant against inflammatory lesion in animal oral mucosa. II. A study of glutathione peroxidase].

In order to understand the mechanisms of superoxide dismutase (SOD) against inflammatory lesion in hamster cheek pouch, glutathione peroxidase (GPx) activity in the cheek pouch mucosa was assayed in different time. The efficacy of GPx scavenging lipid peroxide was also measured. In the SOD group GPx activity decreased significantly in the early stage (8 h). But the level of lipid peroxide in the SOD group not increase at the same time. The results of the model group were contrary. It was implied that exigenous SOD scavenged lipid peroxide in the oral mucosa sufficiently in stead of GPx.

[Effect of taixian tablet on hamster buccal pouch carcinogenesis].

Eighty syrian hamsters were divided into 4 equal groups. The right buccal pouches of hamsters in group I and group II were painted three times weekly with 0.5% DMBA dissolved in acetone, but the hamsters in group I received 0.45 g Taixian tablet daily by mouth. The animals in group III only received 0.45 g Taixian tablet daily and group IV was control group. After 9, 12 weeks, animals were killed with their pouches excised, and tumors were counted and measured. The results showed; 1. Comparing with group II, the malignant rate of group I was lower after 9 weeks and the tumor volume was smaller after 12 weeks (P < 0.01); 2. It was found histologically many inflammatory cells locating in the epithelial layer and lamina propria of group I after 9 weeks, while only a few inflammatory cells in group II. The high differentiated squamous cell carcinoma could be seen in group I and group II after 12 weeks, but no abnormal changes in cervical lymphnodes and organs (lung, liver, spleen, et al) of all animals in 4 groups. It suggests that Taixian tablet can restrain the development of oral carcinogenesis.
