[Combined treatment of pseudohermaphroditism].

OBJECTIVE: To diagnose and treat female pseudohermaphroditism caused by 21-hydroxylase deficiencies. METHODS: Four female patients with 21-hydroxylase deficiencies have received combined treatment of reconstructive surgery and supplementary glucocorticoid in our hospital since 1990. RESULTS: The patients obtained a female external genitalia through the operation. Various symptoms resulted from 21-hydroxylase deficiency were improved with glucocorticoid therapy. CONCLUSION: A satisfying result can be achieved for pseudohermaphroditism by combination of surgery and medicine.

[Uptake of particulate lead via gills of goldfish].

This paper studied the uptake of Pb absorbed on suspended gibbsite via gills of goldfish (Carassius auratus). Under conditions of the concentration of the particulate Pb varied while that of free Pb maintained at a constant level, the levels of both Pb and Al in the gills increased with increasing exposed Pb concentration. The increasing rate (slope 0.171) of Pb level in the blood with the Pb level in the gills was lower than that (slope 0.343) of soluble Pb exposure experiment. Partial Pb adhered on the gills did not transfer into blood. The ratio of Pb/Al in the gills was 1-2 orders of magnitude higher than that in the water, and on the microtome section of filament, the gibbsite particles adhered on gill mucus were observed, indicating the bioavailability of the particulate Pb uptake by the gills of goldfish. The major mechanism was: when the absorbed Pb on suspended particles was flowed into gill filaments, it was desorbed on the surface of gills, and then, passively diffused or transferred by carriers via gill cells into flood, and after the particles stayed on the surface of gill filaments for a short time, in company with sloughed mucus, they were released from the gills along with waterflow.

Molecular and immunohistochemical study of the inactivation of the p16 gene in primary hepatocellular carcinoma.

OBJECTIVE: To determine whether p16 gene is involved in the genesis of primary hepatocellular carcinoma (HCC). METHODS: Twenty-five HCC tumor samples with corresponding non-tumor liver tissue specimens were examined for p16 gene alterations. The identification of deletion of exon 1 and exon 2 in p16 gene was performed using comparative multiplex polymerase chain reaction (PCR) analysis. The point mutation of exon 2 in p16 gene was investigated by single strand conformational polymorphism (SSCP) analysis, and the status of p16 gene methylation was screened using a PCR based methylation analysis. 35 parafin-embedded specimens of HCC with corresponding non-tumor liver tissues, including the 25 cases described above for screening p16 gene alterations, were investigated for p16 protein expression using immunohistochemical analysis. RESULTS: Among 25 cases, 2 homozygous deletions and 1 hemizygous deletion were found in HCC samples. No point mutation was identified in the remaining 22 tumor samples without p16 gene deletions. Hypermethylation was detected in 24% (6/25) of tumor samples. However, the corresponding non-tumor liver tissue specimens were always unmethylated at the p16 locus. Loss of p16 protein expression occurred in 16 of 35 (45.7%) tumor samples, and all the non-tumor liver tissue specimens showed positive p16 staining. For the 25 cases examined for p16 gene alterations, the loss of p16 protein expression was observed in all tumors with p16 gene alterations and also in 3 tumors without p16 gene alterations. CONCLUSION: Inactivation of the p16 gene may play an important role in the genesis of primary hepatocellular carcinoma.

[Evaluation of tumor necrosis factor-alpha and interleukin-6 levels in serum and peritoneal fluid of patients with endometriosis].

OBJECTIVES: To evaluate the levels of tumor necrosis factor-alpha (TNF-alpha) and interleukin-6 (IL-6) before and after conservative laparoscopic surgery in patients with endometriosis. METHODS: The TNF-alpha and IL-6 levels in serum and peritoneal fluid of 20 endometriotic patients and 10 control patients were determined by radioimmunoassay. RESULTS: Significant parallel relationship existed between the stage of endometriosis and the TNF-alpha and IL-6 levels in serum and peritoneal fluid in endometriosis patients (r values: serum: 0.80, 0.52; peritoneal fluid: 0.79, 0.49, P < 0.05). After clearance of endometriosis foci with laparoscopic conservative surgery, the TNF-alpha and IL-6 levels decreased significantly. CONCLUSION: Measuring TNF-alpha and IL-6 level may have important value in postoperative follow-up, surveillance and evaluation of the effectiveness of surgery.

[Relationship between serum epidermal growth factor and uterine leiomyoma].

OBJECTIVE: To study the effects of epidermal growth factor (EGF) on uterine leiomyoma and the relationship of EGF and estrogen, progesterone. METHODS: Fourty patients with uterine leiomyoma and twenty normal women were studied. Serum EGF level was determined by radioimumunoassay, estradiol (E2) and progesterone (P) levels were measured by enzyme marked monoclonal antibody assay. RESULTS: Serum EGF, E2 and P levels in observed group were significantly higher than those in control group during secretive phase (P < 0.005). The serum EGF level in observed group during prolifrative phase was significantly higher than that of secretive phase (P < 0.05). The serum EGF level increased while uterus enlarged; EGF wasn't related to E2(r = 0.25, P > 0.05) and was related to P (r = 0.71, P < 0.005). The EGF level was the highest when E2 and P both increased. CONCLUSION: EGF has the effect on growth of uterine leiomyoma. It is possible that EGF secretion can be stimulated by E2 and the effect of P is stronger than E2.

[Effect of 962 capsule on liver peroxidation, thymus and spleen index].

OBJECTIVE: To investigate the effect of 962 capsule, a TCM compound recipe, on liver peroxidation and weight of thymus and spleen (thymus index, spleen index) in aged rats, and observe the anti-oxidation effect and the influence on immunological function in rats. METHODS: Rats were divided into young control, aged model, positive control (piracetam), 962 middle (0.9 g/kg) dose and high dose (1.8 g/kg) groups, and the medicated rats used in each group were aged rats. All tested drugs were administered for 1 month by gastrogavage. The liver peroxidation was determined by thiobarbituric acid method. The thymus index and spleen index were determined by weighing method. RESULTS: Middle and high dose of 962 capsule attenuated liver peroxidation, their liver MDA lowered, increased the thymus index of aged rats. There was no effect on spleen index in all medicated groups. CONCLUSIONS: 962 capsule could reduce liver lipid peroxidation and increase thymus index. It is suggested that 962 capsule might be beneficial in retarding aging process.

[Study on regulatory effect of composite taixian tablet on immune function of red blood cell in patients with oral lichen planus].

OBJECTIVE: To explore the regulatory effect of Composite Taixian tablet (CTXT) on red blood cell (RBC) immune function in patients with oral lichen planus (OLP) for the sake of providing the basis of clinical medication. METHODS: Sixty patients with OLP were assigned randomly into three groups and were treated by CTXT, Tripterygium hypoglaucum tablet and polyactin-A tablet respectively. And the changes of RBC-C3b receptor and immune circulating rosette complex on the surface of erythrocytes (RBC-ICR) were measured by saccharomycetic assay. RESULTS: Effect of CTXT was superior to that of Tripterygium hypoglaucum and polyactin-A tablets. CONCLUSION: CTXT is a relatively effective remedy with less side effect, it is worthy to be studied further.

[Penetrating wounds of the heart: analysis of 56 patients].

OBJECTIVE: To assess the management of penetrating cardiac injuries. METHODS: 56 patients with penetrating wounds of the heart were studied retrospectively. The study lasted for 11 years. RESULTS: Stab wounds accounted for 89.3% of the 56 patients. 56 patients underwent thoracotomy. Preoperative infusion was less than 1,000 ml in 68% of the patients. Preoperative pericardiocentesis was done only in 2 patients, of whom one was false negative. Four patients with cardiac arrest soon after arrival were subjected to emergency thoracotomy; three survived. After the operation, 2 patients died of associated abdominal injuries and failure of cerebral resuscitation. The overall survival rate was 96.4%. CONCLUSIONS: Early diagnosis and prompt thoracotomy are essential to the treatment of penetrating cardiac injuries. Preoperative massive transfusion and pericardiocentesis are not advocated.

[Surgery for patients with primary liver cancer: report of 1,038 patients].

OBJECTIVE: To investigate methods for enhancing safety of and improving the survival in patients with primary liver cancer (PLC). METHODS: One thousand and thirty eight patients with PLC were treated in our hospital from January 1990 to December 1998. The early period group (from January 1990 to December 1996) consisted of 731 patients [312 patients (42.7%) receiving liver resection]; the late period group (from January 1997 to December 1998) included 307 patients [217 patients (70.7%) receiving liver resection]. RESULTS: The mortality within one month after hepatectomy was 2.24% in the early group and 0.74% in the late group. The 1-, 3- and 5-year survival rates were 73.1%, 54.2%, and 34.0% respectively in patients undergoing hepatectomy in the early group and 91.1%, 63.8% and 40.2% in the late group. CONCLUSIONS: Regular follow-up of high risk patients is helpful to discover small PLC. High proportion of small PLC, progress of perioperative management, and improvement of operative techniques including hepatopetal blood occlusion of half liver or in vitro-bypass through bio-pump during hepatectomy might be important approaches to enhance the safety of surgical treatment and improve the prognosis of patients with PLC.

[Measurement of gastric emptying with Xianshizi in patients with resected esophageal cancer].

OBJECTIVE: To determine gastric emptying radiological in patients with esophageal cancer and to obtain a similar effect produced by radionuclide study. METHODS: A new product-Xianshizi (XSZ) made of the mixture of barium and silica gel was used quantitatively to evaluate gastric emptying. 17 patients with resected esophageal cancer before and after operation, and 7 healthy volunteers were studied. RESULTS: Preoperative patients had obviously delayed gastric emptying compared with the control group (P < 0.01). Postoperative patients had more delayed gastric emptying compared with themselves (P < 0.05). CONCLUSIONS: Postoperative gastroparesis is related to delayed gastric emptying. The results of this study were identical with those obtained by scintigraphy. Compared with isotope studies, X-ray exam after taking meal composed of XSZ may eventually contribute to detecting delayed gastric emptying quantitatively.

[The histogenesis, clinical features and histopathological analysis on 52 cases of ciliary body neoplasms].

OBJECTIVE: To analyze the histogenesis, classification, incidence, clinical feature, the key point of differential diagnosis and histopathologic characteristics of ciliary body neoplasms to provide the basis for clinical diagnosis and treatment. METHOD: 52 specimens of ciliary body neoplasms on file were analyzed and studied for the characteristics of clinical presentation and histopathology. RESULTS: Of the 52 specimens, 38 were located in the ciliary body (73.1%), and 14 lesions involved the iris (26.9%). According to the results of histogenetic classification, the tumors of ciliary sensory epithelium were ranked the first place (20 cases, 38.5%) while malignant melanoma, the second one (14 cases, 26.9%) followed by melanocytoma (9 cases, 17.3%) and granulomatous lesions (5 cases, 9.6%). CONCLUSION: The clinical features and histopathologic characteristics of ciliary body neoplasms are systemically and deeply recognized, that may help clinical ophthalmologists to make correct diagnosis and proper treatment.

[An observation of antiproliferative effect of germanium-132 on cultured pterygium fibroblasts].

OBJECTIVE: To detect the antiproliferation of carboxyethyl germanium sesquioxide (Ge-132) on fibroblasts of pterygium in vitro and try to find a potentially effective agent for treatment of primary pterygium and prevention of its postoperative recurrence. METHODS: Primary culture and subculture of pterygium fibroblasts were established in vitro. Different concentrations of Ge-132 (39 - 5,000 mg/L) or mitomycin-C (3.13 - 4.00 mg/L, the control) were added to the fibroblast culture of the third or forth passage respectively. The inhibitory effect was determined by MTT (tetrazolium bromide) method. The influence of addition of Ge-132 on the growth curve of fibroblasts was observed, and the changing expression of proliferating cell nuclear antigen (PCNA) in fibroblasts was studied by immunohistochemical method. RESULTS: The addition of Ge-132 in the culture caused significant inhibition of the fibroblast proliferation in dose dependent manner (625 - 50,000 mg/L) without cytotoxicity (IC(50) = 3,000 mg/L), the marked descent of growth curve and suppression of the expression of PCNA in cultured cells (P < 0.01). CONCLUSION: Ge-132 can inhibit the proliferation of pterygium fibroblast in vitro significantly.

[MRI features of confusable tumors of orbital apex].

OBJECTIVE: To analyze magnetic resonance imaging (MRI) features of confusable tumors including cavernous hemangioma, cavernous lymphangioma and neurilemmoma located at orbital apex, which could provide evidences for diagnosis and surgical treatment. METHODS: MRI was performed on 16 cases with tumors of orbital apex confirmed by pathology, and their MRI findings were analyzed. RESULTS: The time taken to attain complete enhancement after contrast on MRI was significantly different in the three types of tumor (P < 0.01) Their enhancement patterns were related to their pathologic findings. CONCLUSIONS: The different enhancement patterns of various tumors of orbital apex are characteristic for diagnosis and differential diagnosis.

[A case-control study on the risk factors of esophageal cancer in Linzhou].

OBJECTIVE: To explore the characteristics of prevalence and influencing factors on the genesis of esophageal cancer. METHODS: A population-based 1:1 matched case-control study was conducted in Linzhou. A total number of 352 pairs of cases and controls matched on sex, age and neighborhoods. Data was analysed by SAS software to calculate the odds ratio of and to evaluate the relative risks. RESULTS: It was found that lower socio-economic status, environmental pollution around the residential areas, lampblack in room, lower body mass index (BMI), more pickled food intake, cigarette smoking, alcoholic drinking, vigor mental-trauma and depression were risk factors of esophageal cancer. It also showed that the subjects having had history of upper digestive tract operation, dysplasia of esophagus and family history of carcinoma markedly increased the risks of developing esophageal cancer. CONCLUSION: Esophageal cancer seemed to be resulted from the combination of genetic and environmental factor, hence called for of medical surveillance and comprehensive prevention.

[Effects of chronic exposure to lead on long-term potentiation in hippocampal CA1 area of young rats in vivo].

OBJECTIVE: To study the effects of chronic exposure to lead on learning and memory function in young rats, based on the plasticity of their synapse function. METHODS: With method of extracellular glass microelectrode recording, effects of exposure to lead at different concentrations on long term potentiation (LTP) induction in hippocampal CA(1) area of young rats in vivo were observed. RESULTS: Incidence of LTP and mean amplitude increase after application of high frequency stimulus decreased, as blood lead in rats reached (3.28 +/- 0.88) micromol/L, even causing long term depression. To the exposed rats, their mean increase ratio in their population spike amplitudes after high-frequency stimulation correlated inversely with their brain lead load. CONCLUSION: Chronic lead exposure could damage in vivo LTP induction and maintenance in the CA(1) area in hippocampus of rats, and the severity of damage increased with the extent of exposure to lead.

[Analysis of TCR-gamma gene rearrangement in single H/R-S cells of Hodgkin disease].

OBJECTIVE: To detect the rearrangement of T cell receptor gamma chain gene in single H/R-S cell as an indication of early T lymphocyte differentiation. METHODS: CD30 positive H/R-S cells were isolated with micromanipulator from the frozen sections and analyzed by single-cell PCR. TCR-gamma chain gene rearrangement was detected through semi-nested PCR. In the first round of PCR, a pair of TCR-gamma primers which amplify the segments between Vgamma and Jgamma region (including the N region) of TCR-gamma gene mixing with a pair of beta-globin primers were used. In the second round, only the products positive beta-globin were amplified with the primers for TCR-gamma gene. RESULTS: A 268 bp beta-globin gene specific product was obtained in 24 out of 56 H/R-S cells from 6 cases of Hodgkin's disease. After the first round of PCR, no rearrangement of the TCR-gamma gene was noticed at a single cell level. CONCLUSION: H/R-S cells from the selected cases were not derived from the T-cell lineage but possibly from the B-cell lineage.

[Study on the combined toxicity of aflatoxin B1 and deoxynivalenol].

The toxicity of aflatoxin B1(AFB1) and deoxynivalenol(DON) on the injury and repair of DNA was studied. Replicative DNA synthesis was used to detect the change of DNA synthesis in the primary rat hepatocytes at the S phase of cell cycle. The results showed that both AFB1 and DON could induce DNA synthesis in primary rat hepatocytes at the S phase of cell cycle, and an interaction between them was found. It suggested that AFB1 and DON produced marked effects on engendering tumor as genetoxic and nongenotoxic carcinogens complementarily.
