Prevalence of infections and their risk factors in geriatric institutions: a one-day multicentre survey.

This one-day prevalence survey of 1919 patients (74% females and 44% aged greater than or equal to 85 years) in nine geriatric hospitals, six of them located in France and three in Switzerland, indicated a high prevalence of infections in elderly institutionalized patients (infection rate: 18.7% in males and 15.4% in females). The risk factors for infection were identified and the relative risks assessed. Prevention of infections in geriatric wards should be one of the goals of every care-giver. A low rate of infection in elderly inpatients is thus likely to be associated with care of good quality.

Hymenoptera venom immunotherapy. I. Induction of T cell-mediated immunity by honeybee venom immunotherapy: relationships with specific antibody responses.

The specific cell-mediated and humoral immune responses of 14 children allergic to honeybee venom were studied. An 8-day rush venom immunotherapy induced an increase in T proliferative (p less than 0.04) and T suppressive (p less than 0.003) cell-specific activities. Antibody variations, an increase in specific IgG4 (p not equal to 0.05), and a decrease in specific IgE (p less than 0.01) were observed 1 year later. Initial high T suppressive cell activity prevents T proliferative cell increase during rush venom immunotherapy. High initial levels of specific IgG1 and specific IgG4 have opposing effects on the increase in T suppressive cell activity, the former being positively correlated with intensive increase (r = 0.840; p less than 0.005), the latter negatively with T suppressive cell increase (r = -0.709; p less than 0.001). These data indicate that there are interrelationships between the cell-mediated immunity and the antibody responses in honeybee allergy.

Hymenoptera venom immunotherapy. II. T proliferative and T suppressive activities induced by Vespula immunotherapy: effects on long-term antibody responses.

Thirty-five children who had exhibited major clinical reactions to yellow jacket stings were desensitized during an 8-day rush immunotherapy (RIT). Maintenance treatment was continued for at least 1 year in all cases and for 2 years in 21 cases. Blood samples, obtained before, during, and after RIT, were analyzed for specific T proliferative (TP) and T suppressive (TS) cell activities and for specific IgE, IgG1, and IgG4 levels. TS and TP cell activities increased during RIT (p less than 0.001) and remained elevated throughout the 2-year follow-up. There were transient rises in specific IgG1 and IgE during RIT, but the levels of both decreased thereafter. Specific IgG4 increased throughout the follow-up period. The data indicate that the levels of specific IgE and specific IgG1 were correlated with TP cell responses (inducers) and TS cells responses (suppressors) and therefore might be controlled by them; data also suggest that TS cell responses should play a role in the IgG4 response. These changes in TS and TP cell responses and antibody responses have been incorporated into a model of antibody-response regulation by cell-mediated immunity in this type of allergy.

Multitest CMI for standardized measurement of delayed cutaneous hypersensitivity and cell-mediated immunity. Normal values and proposed scoring system for healthy adults in the U.S.A.

The Multitest CMI system, a disposable device that simultaneously applies seven standardized preloaded antigens and diluent control, is a major advance for measurement of delayed type hypersensitivity (DTH) in assessment of cell-mediated immunity (CMI). The system was tested in 402 healthy adults, aged 17 to 92 years, to determine normal values for incidence and size of DTH responses. Incidence of positive responses to individual antigens varied from 85% to 46%, with great variability related to age and sex. To better assess CMI, a two-part score based on 48-hour readings was employed. The mean number of positive antigens ranged between four and five, and the mean sum of their mm induration ranged between 18 and 25, with both scores increasing with advancing age. A statistical zone of reduced DTH scores (hypoergy) was identified. The Multitest CMI system appears to be a practical means of reproducibly assessing CMI in subjects with immunologic, metabolic, infectious, or neoplastic disorders. The scores in our population may serve as reference values to which results from any tested adult can be compared.

[Is a rise in haematocrit associated with a risk of cerebrovascular accident? A retrospective study of 150 subjects with previous CVA and 150 paired controls (author's transl)].

A retrospective study of the various factors associated with the risk of cerebrovascular accident (CVA) was conducted in 150 patients (87 women and 63 men) who had had a CVA and 150 paired control subjects of the same sex and age. No difference was found between these groups in systolic and diastolic BP, nor in blood glucose and cholesterol levels. However, there was a highly significant increase in haematocrit in CVA patients as compared with controls. The mean figures were +5,69% in men (p less than 0.00005) and + 3.85% in women (p less than 0.00001). It must be noted that the haematocrit values in CVA patients, although increased, remained within the normal range. A rise in blood viscosity could be the mechanism through which the haematocrit contributes to the risk of CVA.

[Isoprinosine treatment of herpes zoster. Résults of a control study in 36 subjects (author's transl)].

A double-blind trial comparing isoprinosine with a placebo was conducted in 36 subjects. Improvement of skin lesions was significantly better in the treatment group (p less than or equal to 0.01). Before entering the trial, patients in the treatment group had significantly more severe pain than those in the placebo group (p less than or equal to 0.01), the difference disappeared after treatment. Healing of skin lesions was more rapid in patients treated before the 5th day of the disease (p less than or equal to 0.03). None of the subjects developed post-zoster neuralgia. The clinical effects of the drugs were accompanied by a decrease in serum rosette activity. We consider isoprinosine as an innocuous and effective treatment of herpes zoster.

Myelin basic protein in CSF and blood. Relationship between its presence and the occurrence of a destructive process in the brains of encephalitic patients.

Serum and CSF levels of myelin basic protein (MBP) were measured in 50 patients with encephalitis of various origins and severity. In nearly 50%, the CSF samples were found to display immunoreactivity of MBP. Positivity was found to be correlated with the severity of the clinical signs. More precisely, it corresponded to cases with suspected extensive brain destruction. No relationship could be observed with the cause of disease. Positive tests of sera were infrequent, even from patients whose CSF was rich in MBP. Longitudinal studies performed on 20 patients who were serially investigated during periods ranging from three weeks to 18 months demonstrated that after an attack, MBP liberation into the CSF persists for one to three weeks. The MBP assay should serve as an index for destruction of nervous tissue.

Human dialysable leucocyte extracts (transfer factor) in interstitial pneumonia. A retrospective trial in immunodeficient patients.

Thirty-six cases of interstitial pneumonia in acquired immunodeficient states were treated with transfer factor (Dialysable Leukocyte Extract) and studied retrospectively. The criteria of efficacity of this treatment were: rapidity of immediate improvement, improvement after failure of other immunostimulant therapy and demonstration of a dose-related effect. The mechanism of the therapeutic action is unclear. There is no evidence in favour of "transfer" of cell mediated immunity. A non-specific mode of action (adjuvant effect, interferon synergy, proinflammatory action) seems much more likely.
