The investigation of atypical psychosomatic illness. A team approach to diagnosis.

The experience of 12 children with atypical psychosomatic illness who presented considerable diagnostic difficulties is described. All patients were admitted to the hospital, and using a team approach, it was possible to make a confident diagnosis. No organic disease was identified, and a psychosomatic diagnosis was made. In general, atypical symptoms and signs, false-positive investigations, and differing medical opinion had compounded the diagnostic difficulties. The value of the observations made by ward staff of the childrens' symptoms and behavior is stressed.

Comprehensive management of children on a paediatric ward: a family approach.

A team approach in the comprehensive management of children on a general paediatric ward is described. The team comprises paediatricians, nurses, a child psychiatrist, and a social worker, with a psychologist, play-leader, and teachers making important contributions. In this way members of the team learn from each other, and the paediatrician in training gains valuable experience about the management of children with emotional problems. The team approach is based on the principles of family therapy, but it also uses other forms of therapy--such as behavioural, marital, and individual. Children with severe psychosomatic problems in particular can be helped by such treatment, but other emotional problems are also suitable for management using such an approach.

Childhood asthma. A controlled trial of family psychotherapy.

In an attempt to evaluate the effectiveness of family psychotherapy as an adjunct conventional treatment in childhood asthma, children with moderate to severe asthma were randomly allocated to a control group or to an experimental group; the latter group received 6 hours of family treatment during a 4-month period, and both groups had standard medical treatment. While there was no significant difference between the two groups on three parameters, the experimental group were significantly better in day-wheeze score and thoracic gas volume. These results suggest that family treatment in selected cases may have a place in the overall management of childhood asthma, and that more research with larger numbers of children is necessary.

Talking with children.

Talking with children is an enjoyable but often challenging aspect of a doctor's work. It is a two-way process, with verbal and non-verbal communication being equally important. Patience, humour and imagination are major assets.
