Effects of long-acting propranolol on blood pressure and heart rate in hypertensive Chinese.

In a double-blind, balanced and randomised study we used treadmill exercise to assess the effects of long-acting propranolol (LA propranolol) 160 or 320 mg or placebo, given once daily for 4 weeks, on heart rate (HR) and blood pressure (BP) in 15 Chinese subjects with mild primary hypertension (PHT). We used 24 h ECG monitoring to assess drug effects on HR. Another 18 patients were similarly assessed without exercise. Steady-state plasma propranolol concentrations after LA propranolol 160 and 320 mg were comparable to those after ordinary propranolol 80 and 160 mg daily measured in 11 and 12 separate patients. LA propranolol 160 and 320 mg reduced HR and BP before and during vigorous exercise. LA propranolol 160 and 320 mg reduced HR for 17.6 and 21.4 h of the day, and 320 mg significantly reduced the mean 24 h HR, and the mean maximum HR. The drug effects on BP and HR, and the average plasma propranolol levels after LA propranolol were similar to those reported in European subjects.

Musical murmur and cardiac shudder.

When echocardiographic studies show several structures vibrating at a similar frequency to an associated musical cardiac murmur, the structure vibrating most strongly may localize the origin of the murmur.

Two-dimensional and pulsed Doppler echocardiographic abnormalities in coronary artery-pulmonary artery fistula.

We describe two patients who presented with precordial continuous murmurs which were diagnosed clinically as due to patent ductus arteriosus; however, two-dimensional echocardiographic and pulsed Doppler echocardiographic studies showed dilatation of the left main coronary artery with turbulent flow during systole in the lumen in both patients. Subsequent selective coronary angiographic studies showed bilateral coronary artery-pulmonary artery fistula in one patient and left coronary artery-pulmonary artery fistula in the other. This study illustrates that in a patient presenting with a continuous murmur which is suggestive of patent ductus arteriosus, such two-dimensional and pulsed Doppler echocardiographic findings as were seen in our patients should immediately alert the physician that he may be dealing with a case of coronary arterial fistula.

Infective endocarditis in Singapore: a six year survey.

Much has been written about the changing clinical spectrum of infective endocarditis. However our survey shows that the classical text book descriptions still hold good. The majority of our patients were young with 79% either 30 years or below. Fever was present in 93%, splenomegaly in 50% and cerebral embolism in 43% of our patients. Twelve patients had valvular heart disease and 2 patients had a ventricular septal defect. Eleven out of 14 patients had a positive blood culture. Echocardiography detected definite or probable vegetations in 66% of the examinations. Five patients responded satisfactorily to antibiotic therapy, 2 patients discharged themselves from hospital against medical advice, and 4 patients underwent successful cardiac surgery. Three patients died during medical therapy.

Cardiac tamponade in acute rheumatic carditis.

In patients with valvular heart disease, fever, and cardiomegaly echocardiography is an invaluable noninvasive tool. In this report we describe a young female presenting with cardiac tamponade due to acute rheumatic carditis. Echocardiography showed an exudative pericardial effusion which was haemorrhagic on pericardiocentesis. She responded to steroid therapy with resolution of carditis and pericardial effusion.

Successful angioplasty of renal artery stenosis due to aorto-arteritis.

A 24-year-old Chinese female who suffered from hypertension and acute myocardial infarction was found to have aorto-arteritis on angiographic examination, with total occlusion of her right coronary artery and subtotal occlusion of her right renal artery. Transluminal angioplasty of her right renal artery was successfully performed and this normalized her blood pressure from 165/105 to 135/85 mm Hg.
