Oestrus, ovulation and peri-ovulatory hormone profiles in tethered and loose-housed sows.

Multiparous sows that had been tethered during lactation were put in two different housing conditions after weaning (Day 0); the sows were either tethered by neck chain, or individually housed in a pen of approximately 6 m2. After two months, ten tethered and eleven loose housed sows were used to assess stress and reproductive parameters. Stereotypic behaviour after the afternoon feeding was assessed from Day 18 onwards; at Day 53 stereotypic behaviour tended to occur during a higher percentage of time in the tethered sows (P = 0.11) and at Day 66, the differences were significant (tethered, 78 +/- 5 vs. loose-housed, 40 +/- 10% of time (mean +/- sem); P = 0.03). At Day 35 and 55, cortisol profiles after afternoon feeding were similar for the two groups of sows (P > 0.10). Around oestrus (approximately Day 64), the profiles of oestradiol-17 beta, luteinizing hormone and progesterone were measured and proved to be similar for both treatment groups (P > 0.10). The duration of oestrus was shorter in the tethered sows (42 +/- 4 vs. 63 +/- 2 h; P < 0.001) and, consequently, the timing of ovulation during oestrus (h after onset of oestrus) was advanced in the tethered sows (28 +/- 2 vs. 41 +/- 2; P < 0.001). The duration of ovulation did not differ (tethered, 2.9 +/- 0.5 vs. loose-housed, 2.1 +/- 0.2 h; P = 0.16). The sows were sacrificed at Day 5 after ovulation; ovulation rate, fertilization rate, embryo development and embryo diversity were similar for the two groups, as were adrenal weight and size of adrenal cortex. Duration of oestrus and the levels of stereotypies at Day 60 tended to be negatively related in the tethered sows (P = 0.07), but not in the loose-housed sows (P = 0.65). In conclusion, sows that had been tethered during pregnancy and lactation, and were housed loose or were tethered again at weaning within two months differed both in stereotypic behaviour and in duration of oestrus, without apparent effects on reproductive hormones.

Vaginal temperature is not related to the time of ovulation in sows.

The relationship between vaginal temperature and ovulation time was studied in sows. The vaginal temperature was measured continuously between Day 4 and Day 10 after Altrenogest-treatment in 10 sows. Oestrus was checked with a vasectomized boar at 8-h intervals, and during oestrus, ovulation time was checked with transrectal ultrasonography at 2-h intervals between 07:00 h and 23:00 h. Two sows ovulated between 23:00 h and 07:00 h, and these sows were taken out of the experiment. In the eight remaining sows, a clear day/night rhythm in vaginal temperature was found: between 03:00 h and 09:00 h, vaginal temperature (LSM +/- sem, corrected for sow) was on average 38.2 +/- 0.01 degrees C; between 15:00 h and 21:00 h, vaginal temperature was on average 38.5 +/- 0.01 degrees C (P < 0.001). Between 4 days before ovulation and 2 days after ovulation, no changes in temperature could be found that were related to ovulation time in any of the sows. Therefore, in sows, changes in vaginal temperature cannot be used as a predictor for ovulation time, and consequently cannot be used to predict the best time for insemination.

Associations between cytochrome P4502E1 genotype, mutagen sensitivity, cigarette smoking and susceptibility to lung cancer.

Cytochrome P4502E1 (CYP2E1) is involved in the metabolic activation of carcinogenic N-nitrosoamines. We therefore assessed the genotype frequencies of PstI or RsaI CYP2E1 restriction fragment length polymorphisms and another susceptibility marker, mutagen sensitivity, in 137 lung cancer cases (92 African American and 45 Mexican American) and 206 controls (114 African American and 92 Mexican American) identified in a molecular epidemiological study of lung cancer. The CYP2E1 c1/c1 genotype was found in 86.7% of Mexican American cases, 70.6% of Mexican American controls, 89.1% of African American cases and 86.8% of African American controls. By multivariate analysis, this genotype was found to be associated with a 14.0-fold increased risk of lung cancer in Mexican Americans but not in African Americans; a 9.9-fold increased risk of lung cancer in Mexican American former smokers, but not in non-smokers or current smokers; a 15-fold increased risk of lung cancer in Mexican American males, but not in females. Patients with the susceptible genotype appeared to have developed cancer at an earlier age and with lower cigarette pack-year of exposure than did patients with the c1/c2 or c2/c2 genotypes. Stratified analysis suggested a greater than multiplicative interaction between cigarette smoking and CYP2E1 c1/c1 genotype, although not statistically significant. The odds ratios (ORs) for the CYP2E1 c1/c1 genotype, cigarette smoking and both risk factors combined were 1.3, 6.7 and 16.3, respectively. The association between CYP2E1 c1/c1 genotype and pack-years of smoking followed the same pattern. The interaction between mutagen sensitivity and CYP2E1 c1/c1 genotype was especially strong in former smokers (the ORs for the CYP2E1 c1/c1 genotype, mutagen sensitivity and both risk factors combined were 3.9, 5.4 and 23.0, respectively). Therefore, the data suggest that individuals who lack a c2 allele might be at higher risk for developing lung cancer.

Influence of insemination-ovulation interval and sperm cell dose on fertilization in sows.

This experiment was conducted to determine the effects of sperm dose at insemination on fertilization rates and accessory sperm cells attached to day 5 embryos. Multiparous sows (n = 115) were artificially inseminated once with 1 x 10(9), 3 x 10(9) or 6 x 10(9) sperm cells between 3 h and 48 h before ovulation. Transrectal ultrasonography was performed at intervals of 4 h to determine the time of ovulation and sows were killed at 120 +/- 5 h after ovulation to assess the results of fertilization. The insemination-ovulation interval had a major influence on the fertilization rate and accessory sperm count. A nonsignificant but consistent increase in fertilization rate and in number of accessory sperm cells due to the sperm dose was observed. During the insemination-ovulation interval of 12-24 h, the median fertilization rates were 95%, 100% and 100%, and the median accessory sperm counts were 11, 17 and 31 for the 1 x 10(9), 3 x 10(9) and 6 x 10(9) doses, respectively. During the insemination-ovulation interval of 24-36 h, the median fertilization rates were 88%, 95% and 97%, and the median accessory sperm counts were 6, 8 and 11 for the 1 x 10(9), 3 x 10(9) and 6 x 10(9) doses, respectively. No direct relationship was detected between embryo quality and the accessory sperm count but there was a relationship between insemination-ovulation interval and accessory sperm count. The fertilization rate was positively correlated with the breeding value for litter size of the sows. In conclusion, the effects of sperm dose on fertilization rate and on accessory sperm count in sows were small and nonsignificant, indicating only small effects of sperm dose on the functioning of the sperm reservoir in the sow.

[Uterine metastasis of invasive lobular breast carcinoma. Case report and review of the literature with reference to differential diagnostic problems and clinical consequences].

Uterine metastases of malignant tumours are rare. Among extragenital malignancies breast cancers of the lobular type most likely spread out to this site. The respective diagnosis is rendered more difficult by the late onset of clinical symptoms that may imitate the picture of primary uterine cancer. We report on a patient with advanced invasive lobular breast cancer (ILC) where this form of metastazation was detected after conization for PAP IVa. Especially in patients with advanced lobular breast cancer and suspicious symptoms (such as pathologic PAP-Smear, conspicuous colposcopic finding, vaginal bleeding, growth-progressive uterus and hyperplastic endometrium) the possibility of uterine metastases should be included in preoperative planning.

Consequences of variation in interval from insemination to ovulation on fertilization in pigs.

This review describes effects of variation in the interval between insemination and ovulation on the fertilization process in the sow. Inseminations performed too early or too late relative to ovulation decrease litter size and especially farrowing rate. This effect can be explained to a large extent by the increase in the percentage of non-fertilized eggs, resulting in partial fertilization or no fertilization at all. No effects of variation in the interval from insemination to ovulation are found on the percentage of degenerate embryos. Only moderate effects are found on mean embryonic development and variation in embryonic development at day 5 after insemination. In general, insemination between 0 and 24 h before ovulation gives good fertilization results. Factors influencing the optimal interval from insemination to ovulation, such as number of sperm cells used for insemination, storage time of liquid semen and use of frozen semen, and sow factors such as parity and breed are discussed.

Expression of oestrus and timing of ovulation in pigs.

Oestrus is the period around ovulation in which sows show a standing response for boars, thus allowing the boars to mate with the sows. The duration of oestrus may vary between sows from 24 h up to 96 h, which is not related to systemic oestrogen concentrations. The moment of ovulation after onset of oestrus may vary from 10 h to 85 h. Consequently, onset of oestrus is not a good predictor for the time of ovulation. A reliable prediction of ovulation time would be worthwhile, since fertilization results are highly dependent on the moment of insemination relative to the moment of ovulation. However, at present, the duration of oestrus is the best 'predictor' of ovulation time and is independent of the duration of oestrus; ovulation takes place on average 70% of the way through oestrus. The following factors have been found to influence the duration of oestrus (and consequently the optimal time for insemination): boar-stimuli, stress, interval from weaning to oestrus, season and parity. Further study is needed to determine the causes of the variability in oestrus expression between sows, including the regulation of oestrus behaviour at the level of the brain.

Colour Doppler sonography improves the pre-operative diagnosis of ovarian tumours made using conventional transvaginal sonography.

OBJECTIVE: Conventional transvaginal ultrasound-and transvaginal colour Doppler flow were used to assess morphology and circulation of pelvic masses. STUDY DESIGN: One hundred and nine adnexal masses in 101 women were examined between January 1993 and September 1994. Morphology was classified after a score published by Sassone et al. in 1991. Doppler waveforms using the lowest resistance index (RI), the pulsatility index (PI) and peak flow velocity were used for analysis. Ninety five patients underwent laparotomy. Following histopathological evaluation best cut-off values, sensitivity and specificity were calculated. Score results were compared with Doppler results and a combination of both methods. RESULTS: A combination of Doppler sonography and conventional transvaginal sonography led to a sensitivity of 74.0% and a specificity of 73.7%. Eight out of 15 malignant masses were classified as stage I. An analysis of the false positive diagnoses showed that important information can be gained when Doppler sonography is performed. In particular, on solid appearing adnexal masses, Doppler sonography leads to a high accuracy (84.6%). CONCLUSION: Colour Doppler sonography is not applicable in routine clinical practice, but can give important additional information in specific cases. For solid appearing masses and in early ovarian malignancy, Doppler sonography facilitates the preoperative discrimination between benign and malignant processes.

[Balloon dissection of the cavum retzii--an innovative preparatory technique for endoscopic, extraperitoneal Burch colposuspension-plasty].

Colposuspension is known as "Gold Standard" in incontinence surgery; however, a variety of different techniques exist for performing this measure. Minimal invasive surgery offers new treatment possibilities. Besides the intraperitoneal approach, there is also an extraperitoneal technique for the Burch procedure. We describe an innovative technique, namely, balloon dissection, which is well known in endoscopic hernioplasty for an atraumatic approach to the cavum retzii. This technique that was used in six patients with urinary stress incontinence has the following advantages: minimal blood loss, better visualisation of the operative field, and a shortened hospital stay and recovery period than with abdominal colposuspension.

Performance and body composition of finishing gilts (45 to 85 kilograms) as affected by energy intake and nutrition in earlier life: I. Growth of the body and body components.

Forty-eight commercial hybrid gilts were used to determine the effects of energy intake from 20 to 45 kg on the relationship between energy intake and weight gain of the body and body components from 45 to 85 kg. Two groups of 24 gilts received a single diet either at 2.2 (restricted) or 3.7 (control) times maintenance (M) from 20 to 45 kg. From 45 to 85 kg the pigs were fed the same diet at one of six intake levels (1.7, 2.2, 2.7, 3.2, or 3.7 x M, or ad libitum). At 85 kg, the gilts were dissected into organs, lean tissue (trimmed major joints), and other carcass parts (fat fraction). In the restricted gilts, body and lean tissue gain between 45 and 85 kg increased curvilinearly with increasing energy intake from 432 to 1,412 g/d and from 228 to 507 g/d, respectively. In the control gilts, body and lean tissue gain increased from 394 to 1,201 g/d and from 238 to 508 g/d, respectively. The percentage of lean tissue in the carcass decreased curvilinearly with increasing energy intake from 62.9 to 56.5%, and from 62.5 to 53.9% in the restricted and control gilts, respectively. The restricted gilts gained on average 140 g/d faster (P < .001) and their carcass lean tissue content at slaughter was approximately 3% higher (P < .001). These differences increased with increasing energy level between 45 and 85 kg. The increased gain was largely the result of an increase in organ growth and gut contents and was only evident up to 65 kg. The higher lean content at 85 kg in previously restricted gilts was not the result of compensation in lean gain but was already present at the end of the restriction phase at 45 kg.

Glucose tolerance of pregnant sows is related to postnatal pig mortality.

An experiment was conducted on three farms to study the relationship between the results of an oral glucose tolerance test on d 104 +/- 4 of pregnancy and subsequent reproductive performance of 104 multiparous sows. After an overnight period without feed, sows were fed (3 g/kg BW.75) glucose, and blood samples were taken for analyses of glucose at -10, 10, 20, 30, 40, 50, 60 70, 80, 90, 105, and 120 min after administration of glucose. Glucose concentrations before glucose administration ranged from 2.0 to 3.6 mmol/L. Sows with lesser pretest glucose concentrations had longer (P < .01) durations of pregnancy (corrected for number of live pigs born) and heavier (P < .05) pigs at birth (corrected for farm and number of live pigs born). Peak glucose concentrations and areas under the curves (i.e., first 70 min and entire 120 min) ranged from .1 to 4.4 mmol/L, -40 to 211 (mmol/L).min, and -95 to 247 (mmol/L).min, respectively. Sows with greater (P < .01) peak concentration of glucose and greater area under the curve (first 70 min [P < .01] and entire 120 min [P < .01]) had increased pig mortality during the first 7 d after farrowing (corrected for live weight of the sows). In 83 sows, a sample of morning urine (before feeding) was collected and tested for glucose and ketone bodies. Neither glucose nor the ketone body acetoacetic acid was detected in the urine. The results indicate that sows that are less glucose-tolerant have greater pig mortality.

Relationship of weaning-to-estrus interval to timing of ovulation and fertilization in sows.

Data from two experiments were used to study the relationship between weaning-to-estrus interval (WEI, varying from 3 to 6 d) and the duration of estrus, the interval from onset of estrus to ovulation and fertilization results at 120 h after ovulation, in 201 multiparous sows. An increase in WEI resulted in a decrease in the duration of estrus (Duration of estrus (h) = [81 (exp1) or 88 (exp2)]-.33 x WEI, R2 = .25, P < .001) and a decrease in the interval from onset of estrus to ovulation (onset estrus to ovulation (h) = [54 (exp1) or 59 (exp2)]-.20 x WEI, R2 = .23, P < .001). Sows that were inseminated between 0 and 24 h before ovulation showed consistent high fertilization results (91% normal embryos), irrespective of WEI. Sows that were inseminated outside this range showed an increased proportion of unfertilized eggs (P < .001). This increase occurred similarly in all weaning to estrus intervals. These results suggest that negative effects of a longer WEI on subsequent litter sizes and farrowing rates as described in literature might be a result of suboptimal timing of insemination relative to ovulation and not of poor fertility in these sows.

Microsatellite alteration at chromosome 3p loci in neuroendocrine and non-neuroendocrine lung tumors. Histogenetic and clinical relevance.

Although chromosome 3p regions are the most frequent site for genetic alterations in small-cell lung carcinoma (SCLC) and non-small-cell lung carcinoma (NSCLC), the extent of such abnormality in carcinoid tumors remained to be investigated. Moreover, the histogenetic and biological implications of these findings in non-carcinoid lung tumors remain unclear. We studied eight microsatellite loci on chromosome 3p regions by multiplex polymerase chain reaction in paired normal and tumor DNA from 17 carcinoid tumors, 5 SCLCs, and 38 NSCLCs to determine the histogenetic and the clinical significance of their alterations in these neoplasms. Our results revealed a lack of microsatellite abnormalities at all loci tested in both typical and atypical carcinoid tumors. SCLCs and NSCLCs showed loss of heterozygosity in 100% (5/5) and 58.0% (22/38), respectively. Loss of heterozygosity at more than two loci correlated significantly with poor histological differentiation and were preponderantly found in high proliferative index and DNA aneuploid NSCLCs. Microsatellite instability was noted in only one (1.7%) of the lesions. Our study suggests that 1) the difference in chromosome 3p alterations between carcinoid tumors and SCLCs favors a stochastic rather than linear evolution of these tumors, 2) 3p alterations may constitute an initial event in the development of small cell carcinomas, and 3) loss of heterozygosity at 3p loci is associated with aggressive tumor characteristics in non-small-cell carcinomas.

Calcofluor white stain for the detection of Pneumocystis carinii in transbronchial lung biopsy specimens: a study of 68 cases.

Calcofluor white (CFW), a chemofluorescent agent, has proven effective in the recognition of Pneumocystis carinii cysts in respiratory fluids and secretions. However, its usefulness in the recognition of P. carinii cysts in tissue preparations has not been established. We studied 68 formalin-fixed, paraffin-embedded, transbronchial tissue biopsy specimens from individuals seropositive for the human immunodeficiency virus and stained them with the CFW stain and the conventional Gomori methenamine silver (GMS) stain to determine the concordance rate of the two stains. CFW-positive specimens showed light peripheral staining and a unique double parenthesis-like structure near the center of the cysts. Thirty-six (52.9%) of the 68 specimens were CFW and GMS positive, whereas 27 (39.7%) of the specimens were negative by both techniques, yielding a concordance rate of 92.6%. Five (7.4%) of the 68 specimens showed disparate results, and, of these, four (5.9%) were CFW positive and GMS negative, whereas one (1.5%) was CFW negative and GMS positive. We conclude that the CFW stain is suitable and useful for the demonstration of P. carinii cysts in tissue preparations.

Dysgerminoma of the ovary in a patient with triple-X syndrome.

A 24-year-old patient with dysgerminoma of the right ovary and hypoplastic left ovary was screened for chromosomal aberration. Triple-X syndrome was found in the oral epithelium and lymphocyte culture, while chromosomal in situ hybridization of the tumor itself showed a normal XX karyotype. Although trisomy X and dysgerminoma are coincidental findings in our case report, women with malignant germ cell tumors and abnormal genitalia should be checked for chromosomal aberrations to exclude anomalies carrying a Y chromosome.

Ductal carcinoma in situ treated with lumpectomy and irradiation: histopathological analysis of 49 specimens with emphasis on risk factors and long term results.

Forty-nine women with ductal carcinoma in situ (DCIS) treated with lumpectomy and irradiation were studied retrospectively. The median age was 50 years (range, 29 to 73 years) and the median follow-up time from initiation of therapy was 86 months (range, 17 to 230 months). Twelve patients presented with palpable masses (0.4 to 4 cm), three with breast thickening, and three with nipple discharge. In 31 patients the tumors were detected by mammography. Intraoperatively, excision of lesions was confirmed by specimen x-ray (38 specimens) or gross inspection (five specimens) and was recorded to be complete. No record was available in the other six patients. Margins of excision free of DCIS were microscopically confirmed in 25 specimens. The size of impalpable DCIS lesions recorded in 25 patients ranged from 0.4 to 5.0 cm (mean, 1.5 cm). Using Lagios' classification system, there were 18 classic comedocarcinomas, high nuclear grade (NG) with necrosis; seven cribriform/papillary, high NG with necrosis; 17 cribriform/micropapillary, intermediate NG with or without necrosis; and seven cribriform/micropapillary, low NG without necrosis. In two patients residual malignant calcifications were present on the postoperative mammogram. Disease recurred in the treated breast at the site of incision in five patients at 18 months and 8, 11, and 12 (two patients) years from initial therapy. The rate of local disease recurrence was 2% at 5 years and 6% at 10 years; three recurrences showed invasive ductal carcinoma and two were DCIS. To evaluate risk factors the following characteristics were considered: necrosis, NG, histological type, periductal fibrosis, periductal lymphoid infiltrate, margin status, age, and method of tumor detection. The end points chosen were recurrence and death from any cause (because only one patient died of disease). Although the recurrences were attributed to residual disease in two patients, of the clinical and pathological parameters evaluated, only periductal fibrosis showed a significant relationship with outcome, with a P value < or = .05 by the Wilcoxon test. On the other hand, using the proportional hazards model, necrosis was a significant predictor for recurrence (P = .02), as was the pair fibrosis and tumor detection when taken together (P = .05). Fibrosis significantly associated with high NG, Lagios' histological subtypes I and II, periductal lymphoid infiltrate, and necrosis (P < or = .0006).(ABSTRACT TRUNCATED AT 400 WORDS)

Treatment of progressive outer retinal necrosis with sorivudine.

PURPOSE/METHODS: We examined a patient with progressive outer retinal necrosis, which is presumably caused by the varicella-zoster virus in patients with the acquired immunodeficiency syndrome. RESULTS/CONCLUSIONS: The patient was successfully treated with a combination of intravitreal ganciclovir and oral sorivudine. Treatment for progressive outer retinal necrosis has been disappointing; both acyclovir and ganciclovir have had only limited success. Sorivudine, a new antiviral medication with activity against varicella-zoster virus, may represent an effective alternative treatment for retinal necrosis.
