Lactating adenoma: a diagnosis of exclusion.

This is a case of a large lactating adenoma which developed in a 26 year old primagravida during the third trimester of her pregnancy. The presentation was that of a grossly enlarged and engorged breast with breast erythema, warmth and tenderness. Radiologic and pathologic examinations were difficult because of the lactational changes in the breast and areas of infarcted tissue within the large tumor. Skin biopsies and core biopsies of the mass were performed to exclude malignancy. Surgical resection of the mass was necessary for definitive diagnosis. The pathology proved to be a lactating adenoma, which is the most prevalent breast mass in young pregnant females. The large size of this tumor, and the presentation of breast erythema and edema raised the possibility of inflammatory breast cancer. Following surgical resection and definitive diagnosis of this tumor, the patient required plastic surgical reconstruction of the breast because of redundant breast tissue. Although most lactating adenomas spontaneously involute, the diagnosis is not always straight-forward and surgical resection may be required for definitive diagnosis and exclusion of other pathologic processes.

In primiparous sows, plasma insulin-like growth factor-I can be affected by lactational feed intake and dietary energy source and is associated with luteinizing hormone.

During a 21-day lactation period, 48 primiparous sows were fed a fat- or a starch-rich diet (131 g fat and 183 g starch + sugar x kg(-1) vs. 31 g fat and 351 g starch + sugar x kg(-1)) at a high or a low level of feeding (44 vs. 33 MJ net energy per day) according to a factorial design. Blood samples were collected at days 7, 14, 21, 22 (weaning), 24, 25, 26, and 27 post-partum (p.p.). IGF-I levels were higher with the starch-rich than with the fat-rich diet at days 7, 21, 22, and 24 p.p. Plasma IGF-I concentrations on day 22 p.p. were positively related with LH pulse frequency on day 22 p.p. and the height of the pre-ovulatory LH surge. Sows with low body weight at farrowing and high lactational body weight loss had lower plasma IGF-I concentrations than others, before and after weaning. These results indicate that IGF-I concentrations are affected by both feeding level and dietary energy source and are related to the secretion of LH. Furthermore, body weight at farrowing interacts with lactational body weight loss to affect IGF-I concentrations.

[Concentration of estrogen and progesterone receptors in the lower uterine segment at term labor].

BACKGROUND: To determine cytosol estrogen and progesterone receptor concentrations in the human lower uterine segment in different stages of cervical dilatation during parturition at term. PATIENTS AND METHODS: Biopsy specimens of the lower uterine segment were obtained from 51 women undergoing non-elective cesarean section at term. The stage of cervical dilatation at the time of cesarean section was < 2 cm (N = 14), 2 to < 4 cm (N = 13), 4 to 6 cm (N = 11) or > 6 cm (N = 13). The cytosolic estrogen and progesterone receptor concentrations were determined by enzyme immunoassays. RESULTS: Median estrogen receptor concentrations at < 2 cm and at 2-< 4 cm cervical dilatation were 2.12 fmol/mg protein and 2.03 fmol/mg protein, respectively. After a significant drop at 4-6 cm cervical dilatation (median: 1.08 fmol/mg protein), estrogen receptor concentrations raised again at > 6 cm cervical dilatation (median: 2.00 fmol/mg protein). Median progesterone receptor concentration was 84.7 fmol/mg protein at < 2 cm cervical dilatation, diminished significantly at 2-< 4 cm cervical dilatation (36.6 fmol/mg protein) and increased at further cervical dilatation (4-6 cm: 75.7 fmol/mg protein; > 6 cm: 83.7 fmol/mg protein). CONCLUSIONS: These data suggest that predominantly a decrease in cytosolic concentration of the progesterone receptor at the beginning of the active phase of first stage labor may play a crucial role in the hormonal control of cervical dilatation during parturition at term.

[Cytokine concentrations and expression of adhesion molecules in the lower uterine segment during parturition at term: relation to cervical dilatation and duration of labor].

OBJECTIVE: To determine the concentrations of tumor necrosis factor (TNF)alpha, interleukin (IL)beta, IL-6 and IL-8 and the expression of endothelial leukocyte adhesion molecule-1 (ELAM-1) and vascular cell adhesion molecule-1 (VCAM-1) in the lower uterine segment during parturition at term and to investigate correlations with the stage of cervical dilatation and with the duration of labor. PATIENTS AND METHODS: Biopsy specimens were taken from the lower uterine segment of 59 women undergoing non-elective cesarean sectin at term at different stages of cervical dilatation (< 2 cm, n = 17; 2-3 cm, n = 12; 4-6 cm, n = 13; > 6 cm, n = 17) after varying length of the time in labor (1-6 h, n = 18; > 6-12 h, n = 16; > 12-24 h, n = 15; > 24 h, n = 10). The concentrations of TNF alpha, IL-1 beta, IL-6 and IL-8 in protein extracts of these specimens were determined by enzyme linked immunosorbent assays and the expression of ELAM-1 and VCAM-1 was studied by immunohistochemistry. Staining was considered negative if < 5% of the endothelial cells were stained and positive if > or = 5% were stained. RESULTS: At < 2 cm dilated cervix the median concentrations of TNF alpha, IL-1 beta, IL-6 and IL-8 were 8.0 pg/mg total protein (TP), 1.3 pg/mg TP, 12.6 pg/mg TP and 17.2 pg/mg TP, respectively. At 4-6 cm dilatation the median concentrations were increased (TNF alpha: 22.2 pg/mg TP [P = 0.05], IL-1 beta: 22.2 pg/mg TP [P < 0.05], IL-6: 1226 pg/mg TP [P < 0.05], IL-8: 2081 mg/mg TP [P < 0.05]). At > 6 cm dilatation no further increase was observed. The expression of ELAM-1 and VCAM-1 was significantly greater at > 6 cm dilatation than < 2 cm dilatation (P < 0.001 and 0.05, respectively). With increasing duration of labor, the concentrations of IL-1 beta, IL-6 and IL-8 (but not of TNF alpha) increased and were significantly higher at > 6 cm dilatation than at < 2 cm (P < 0.05). Expression of ELAM-1 and VCAM-1 did not vary with duration of labor. CONCLUSIONS: These first investigations of cytokines and cell adhesion molecules in the lower uterine segment at both different stages of cervical dilatation and different duration of labor support the decisive role of cytokines for normal cervical dilatation. Local changes in the lower uterine segment independent from the effects of labor (e.g. increase of endothelial adhesivity) seem to be of special importance in this process.
