Postural behaviour of intraocular pressure following trabeculoplasty.

Postural behaviour of intraocular pressure (IOP) was studied in 29 glaucomatous eyes before and after argon laser trabeculoplasty (ALT) and compared with that of 60 normal eyes. Although argon laser trabeculoplasty was successful in lowering the IOP below 21 mm Hg, it produced little effect on the postural behaviour of the IOP. No significant change was observed in the amplitude of postural rise of IOP in ALT treated eyes.

Mutagenicity of 4-nitrodiphenyl thioether-derived products and their potential metabolites.

4-Nitrodiphenyl thioether (4-NO2TE) and its various potential metabolites 4-nitroso- (4-NOTE), 4-hydroxylamino- (4-NHOHTE), 4-hydroxyacetylamino- [4-N(OH)ACTE], 4-acetoxy-acetylamino- [4-N(OAc)-ACTE], 4-amino- (4-ATE) and 4-acetamido- (4-AATE) were tested for their mutagenicity toward Salmonella typhimurium TA98 in the presence and absence of S9 activation system. 4-NO2TE, 4-NOTE, 4-NHOHTE and 4-N(OAc)ACTE were direct-acting mutagens with 4-NHOHTE having the highest activity. With S9 mix, the mutagenicity of 4NO2TE almost doubled and that of 4-NOTE and 4-N(OAc)ACTE decreased. In TA98NR, 4-NOTE induced higher mutations, while with 4-NHOHTE the number of revertants decreased. Both 4-NHOHTE and 4-N(OAc)ACTE showed decreased mutagenicity in TA98/1,8-DNP6. 4-ATE, 4-AATE and 4-N(OH)ACTE expressed mutagenicity only with S9 mix. 4-Nitrodiphenyl sulphoxide (4-NO2SO) and 4-nitrodiphenyl sulphone (4-NO2SO2), the sulphur-oxidised products of 4-NO2TE, were non-mutagenic as such and after addition of S9 activation system.

Urethro-venous intravasation during retrograde urethrography (report of 5 cases).

Five instances of urethro-venous intravasation during retrograde urethrography are reported. Four cases were of urethral strictures and one case was of urethral hemangioma. All patients had post procedural bleeding while one patient got allergic reactions, another showed chills and rigors. Anatomy of the drainage veins is described. Factors responsible for this complication and its clinical implications are discussed. It is suggested that urethro-venous intravasation should be considered a diagnostic sign of urethral inflammation.

Intraocular pressure profile during general anesthesia.

The effect of some anesthetic drugs on intraocular pressure (IOP) was studied in 120 normal healthy patients undergoing non-ophthalmic surgical procedures. IOP rose significantly following the injection of succinylcholine (SCh) alone, or when such injection had been preceded by a pretreatment with a "self-taming" dose of SCh or d-tubocurarine (d-Tc). Though the rise in IOP after diazepam pretreatment was significant, the magnitude was lower than that observed in the groups pretreated with the other two agents. Halothane brought the IOP down faster and lower than ether. SCh is unsafe for intubation for the administration of general anesthesia in cases involving penetrating ocular injuries. It can, however, be used safely for routine ophthalmic surgery, providing that 8 minutes are allowed to elapse between injection and corneal or scleral incision. Halothane is preferred to ether, since the former lowers IOP faster and in a greater amount than the latter.

Actinomycetic corneal ulcer.

A rare case of keratoactinomycosis developing in the absence of any known ocular trauma is described. It showed a dramatic response to penicillin therapy. Steroids should be cautiously used in the presence of active corneal disease. This case highlights the importance of repeated examination of corneal scrapings.

Malignant melanoma of the choroid in a naevus of Ota.

A rare case of choroidal malignant melanoma in a naevus of Ota is described. This is the first reported case from Asia outside the Japanese population. This case illustrates the need for close observation of all pigmented lesions of the eye.

Aged

Argon laser trabeculoplasty in Asian eyes.

Twenty two Asian eyes suffering from open angle glaucoma were treated by argon laser trabeculoplasty and were followed for an average of eleven months. 77.27% of eyes were controlled with ALT alone. The pressure lowering effect of argon laser trabeculoplasty in this series was found to be consistent. Argon laser trabeculoplasty may become the preferred method of treating open angle glaucoma in the Asian population.

Fundus hemorrhages in infancy.

The anatomical location and appearance of retinal hemorrhages in the infant provide important clues in the diagnosis of underlying disorders. While neonatal retinal hemorrhages related to birth trauma are common, benign, and self-limited, other retinal hemorrhages in infancy may signify intracranial aneurysms, accidental or non-accidental injury, and a variety of ocular (e.g., Coats' disease, PHPV, ROP, retinal dysplasia, hypertension, myopia) or systemic disease (e.g., hematologic or cardiovascular disorders, infection, protein C deficiency). In this review, retinal hemorrhages are illustrated and classified according to location, appearance, and etiology. Prompt diagnosis of retinal hemorrhages in infants is crucial, because treatment may be required to prevent early deprivation amblyopia and blindness. Ophthalmological findings may also be a valuable contribution to the overall medical evaluation of the infant.
