Urinary excretion of 5-S-cysteinyldopa (5-S-CD) was followed in 20 patients receiving PUVA therapy. Large increases were observed after PUVA treatment, more predominant in males and unrelated to skin types. Relative increases rather than absolute values paralleled cutaneous reactions.
Skin fibroblasts from eight scleroderma patients were seeded in collagen lattices, and their capacity of retraction was compared to that of fibroblasts from normal volunteers. In all cases, pathological fibroblasts retracted collagen lattices earlier and more intensively than controls. This in vitro feature may be related to the cutaneous retraction which characterizes scleroderma lesions in vivo.

It was shown in a previous paper that a connective tissue glycoprotein (CTGP) extracted from normal rabbit dermis was able to inhibit total protein and collagen syntheses by normal dermis fibroblast cultures. In the present study, the effects of CTGP on scleroderma fibroblasts were investigated. [14C]Proline incorporation into total proteins of the supernatant was not significantly different from that found in controls. By contrast, the amount of collagen, expressed as percentage of total secreted protein, was far higher in scleroderma cultures than in normal ones (14.4% +/- 6.0% vs 4.6% +/- 0.9%). Addition of CTGP to the medium induced a concentration-dependent inhibition of [14C]proline incorporation into proteins from both control and scleroderma cells. In control cultures, no significant decrease of the percentage of collagen was observed, but over 60 micrograms/ml, both cytotoxic effects and inhibition of protein synthesis occurred. In scleroderma cultures, the inhibition was twice as effective on collagen as on noncollagen protein synthesis. The inhibition of collagen secretion was not related either to changes in collagen hydroxylation or to the intracellular catabolism of newly synthesized procollagen.
We report the cases of two adult patients in whom fulminant hepatitis developed after 17 and 103 days of ketoconazole administration. Histologic administration showed massive, predominantly centrilobular necrosis. Clinical manifestations of hypersensitivity and eosinophilia were absent in both patients, which suggests that ketoconazole hepatotoxicity is not mediated through an immunoallergic mechanism.

A double-blind therapeutic trial was undertaken in 40 subjects with large venous ulcers and distributed on a homogeneous basis. They were treated with naftidrofuryl by intravenous infusions for one week followed by oral treatment for three months, in comparison with a placebo (infusions of saline and lactose tablets). All the patients received the same local treatment. Results were assessed not only clinically (changes in ulcer) but also objectively by the measurement of TcPO2. TcPO2 by the measurement of transcutaneous tissue oxygenation is an indicator of the state of the microcirculation and hence the trophic condition of the tissues. Study of the results (Student t) showed that in the treated group and at the ulcer site there was a significant (p less than 0.05) difference from D7 onwards, which increased further during treatment (p less than 0.02 at 3 months). For the placebo group, significance occurred only at D90 (p less than 0.05). The intergroup course difference was itself significant at D45 and D90. The tolerance of naftidrofuryl in both of its forms was unremarkable. Clinical study showed a good correlation between improvement in PO2 and ulcer healing. This trial confirmed the efficacy of naftidrofuryl in the treatment of venous ulcers.
The authors report the case of a labial embryonal rhabdomyosarcoma of a baby. This appears to be an exceptional localization. The study of the first biopsy which was too superficial leads to the thought of a capillary angioma. At the age of 6 months the lesion is 35 mm in diameter, a lobulated formation, typically botryoid. The excision after a frozen section control was in healthy tissue. At the age of 10 months, there is a local relapse with a nodule of 18 mm with left submandibular lymph nodes. A second surgical operation allows the culture of the tumoral tissue. The conventional histopathological examinations shows the aspect of an embryonal rhabdomyosarcoma. On the ultrastructural examination one can visualize in some cells an intracytoplasmic filamentous material. The cellular proliferation in culture, after May Grunwald Giemsa coloration is monomorphous, spindle shaped and of fibroblastic aspect. The ultrastructural study of this cellular proliferation after trypsinization on the 7th day shows some cells including an intracytoplasmic filamentous material. On the data of the cellular culture, the ultrastructural studies and the review of the literature, the authors discuss the possibility of the relationship between the cells of embryonal rhabdomyosarcomas and the fibroblasts and myofibroblasts. Embryonal sarcoma seems to be a better denomination than embryonal rhabdomyosarcoma, as regards the histogenesis.
This study examines 5-S-cysteinyldopa, which is a melanoma-associated marker, and sialic acid whose increase appears to be a common feature of numerous cancers. In spite of some interferences due to sun exposure, 5-S-cysteinyldopa seems a significant indicator of metastases; the difference between 46 metastasis-negative and 34 metastasis-positive melanomas is significant at P less than 0.001. Cerebral metastases give little or no increase. In contrast with the 75% of patients who keep normal 5-S-cysteinyldopa excretion, all melanoma patients have elevated sialic acid. No difference occurs between glycoprotein carbohydrates of controls and patients after pronase digestion and con A chromatography. The use of those two parameters in association is proposed to have a proper index of tumor burden or success of therapy.
Plasma exchange, together with oral immunosuppressive drugs and various doses of adrenal steroids, were used in the treatment of ten patients with pemphigus. Initially plasma exchange decreased the intercellular antibody (IC Ab) level in all patients and induced a clinical improvement in the conditions of eight of ten patients. This improvement was sustained for the six patients who received simultaneously 0.5 mg/kg/day or more of prednisone. Among the eight patients, four previously corticoresistant patients had perfect control of their disease. Conversely the other four patients, treated with low doses of prednisone, experienced a posttreatment rebound in IC Ab level and showed no or only transient clinical improvement. Plasma exchange may be useful in bringing corticoresistant pemphigus under control but it is not to be used without accompanying high doses of adrenal steroids.

The distribution of fibronectin (FN), a major glycoproteic component of extra-cellular matrix, has been studies by an indirect immunofluorescence technique in the skin of 50 normal controls and 19 sclerodermic patients. In the normal skin, FN was present mainly in the papillary dermis, as thin strips and less abundant in reticular dermis, bound to collagen bundles. In scleroderma skins, FN was increased in the deep dermis of extensive and evolutive lesions (11 cases). In an other hand, the distribution of FN was not modified in stabilized lesions (8 cases). We conclude that the detection of FN in the scleroderma skin is an useful marker of the activity of the systemic sclerosis process and we discuss the possible role of FN as a primary matrix for organization of the collagenous connective tissue during the sclerosing process.
