[Burkitt lymphoma of African type in Europe (author's transl)].

A 34-year-old German woman who had never been to Africa developed a Burkitt lymphoma of African type during the fifth month of pregnancy. The diagnosis was confirmed during life by cytological and virological tests. There was a markedly increased antibody titre against the Epstein-Barr virus in serum as well as virus antigen in the tumour tissue. There was extensive involvement of almost all abdominal organs and of the breast. After spontaneous abortion of a female foetus in the sixth month of pregnancy cytostatic treatment was started, but without achieving remission. The patient died five weeks after admission to hospital; the autopsy confirmed the clinical findings.

Secondary athrocytotic cardiomyopathy--heart damage due to Wilson's disease.

Post-mortem atomic absorption spectrophotometry of the myocardium of a 14-year-old boy revealed a hundred-fold increase in copper. Further electrolyte analysis of the myocardium showed changes corresponding to metabolic heart muscle damage. Ultrastructural examination showed all the feature of a cardiomyopathy at the cellular level. Laser-Microprobe-Mass-Analysis demonstrated an inhomogeneous distribution of copper. An essential factor in the mechanism of death is heart damage.

Epstein-Barr virus-positive Burkitt's lymphoma in a German woman during pregnancy.

A fatal case of a Burkitt's lymphoma which occurred in a 34-year-old German woman during pregnancy is described. Nearly all organs showed either diffuse or nodular infiltration by tumor cells. Placenta and fetus were free of detectable tumor tissue. The patient had extremely high antibody titers (1 : 2056), both against Epstein-Barr virus capsid antigen (VCA) and the early antigen complex (EA). Within the tumor cells the Epstein-Barr virus-specific nuclear antigen EBNA and viral DNA was detected. A cell line established from a tumor biopsy displayed a translocation involving chromosomes 2 and 8. The role of Epstein-Barr virus in the development of Burkitt's lymphoma is discussed.

The healing process of experimentally produced bleeding lesions after hemostatic electrocoagulation with simultaneous instillation of water.

Fifty experimentally induced, standardized gastric bleeding lesions were successfully arrested in the dog using electrocoagulation with simultaneous instillation of water (EHT-coagulation). The contact pressure of the coagulating electrode was kept almost constant by means of a spring mechanism. The coagulated lesions healed without secondary enlargement of the coagulation effect and without complication. No secondary perforations were observed. Even when coagulation was carried out at an output power about 30% higher than the mimimum power necessary for reliable hemostatic coagulation, no complications were observed. Only with additional post bleeding arrest coagulation in excess of 10 seconds at this higher output, were two primary performations produced in 2 out of 12 cases. The non-perforated remaining 10 lesions healed without complication. With additional coagulation (at power output setting 6 for 20 seconds - 8 cases; at power setting 7 for 10 seconds - 6 cases), no perforations were observed and healing was without complications. Thus, in animal experiments, the therapeutic range for the arrest of bleeding using the electro-hydro-thermo-electrode is adequately large. The technique should be tested in clinical trials.

[Morphological changes in the gastric mucosa of rats following chronic antacid administration].

The effect on histological changes of the stomach mucosa of 50 male and 50 female Wistar-rats was tested after a constant application of antacidum which was given up to 12 weeks. After 8 weeks the number of agryophile endocrine-active G-cells in the antrum has doubled; then it hardly increased any more during the last period of the experiment. In the corpus mucosa an increase of argyrophile cells could be noticed at the end of the experiment. The relation of the various cell populations of the specific corpus glands shifted a little in favour of the parietal cells. This resulted in a 10 per cent increase of the parictal cells and an equal decrease of the chief cells. The mucoid superficial cells did not show any significant changes just as much as the thickness of the mucosa in all three regions of the stomach did not alter during the total lenght of the experiment. Statistically significant differences between a low dose (1 ml/die) and a high dose (2 ml/die) could not be ascertained. The hyperplasia of argyrophile cells of the corpus is seen in correlation with the hyperplasia of the G-cells respectively the increased release of gastrin, because somatostatin produces a generally inhibitory effect, especially on the release of gastrin.

Modified electrocoagulation and its possibilities in the control of gastrointestinal bleeding.

In acute experiments in the stomach of dogs electrocoagulation with simultaneous instillation of water (EHT-electrode i.e. electro-hydro-thermo-electrode) proved the most effective electrocoagulation method for controlling standardized bleeding: Optimal visibility at the bleeding site, extension of the therapeutic range as well as the avoidance of charring at the bleeding site are its major advantages. Disturbing adhesion of coagulated material on the electrode is also avoided. The electro-hydro-thermo-electrode is a cheap and portable means of controlling bleeding.

[Trichinosis: epidemiology, clinical features and diagnosis (author's transl)].

After ingestion of wild boar meat 58 patients developed typical clinical signs and symptoms of trichinosis. All patients had marked blood eosinophilia. In 45% of the patients GOT and GPT and in 62% HBDH were moderately increased, while in 76% LDH and in 86% creatine-kinase showed abnormally high values. CK-MB was increased in three patients. 92% of muscle biopsies showed typical morphological changes in skeletal muscles. Latexagglutination, complement-fixation and agar-gel diffusion tests were not reliable. In most of the examined cases clinical symptoms and histological results correlated well with the indirect haemagglutination test, the microprecipitation test with living larvae of Trichinella spiralis and evidence of specific antibodies (IgG, IgM, IgE) by indirect immunofluorescence and the ELISA technique. 25 patients were treated with thiabendazole (Minzolum), 19 of them successfully.

[The localisation of endocrine cells in the distal esophagus (author's transl)].

At the esophago-cardiac junction squamous epithelium is intermingled with gastric epithelium. Within the lamina propria of the distal 3 cm of the esophagus we found cardiac glands with many argyrophil cells in 60% of cases. Electron microscopy revealed seven well knonwn types and two further forms which we could not identify: EC, ECL, D, D1, AL, I eg. M, S. It is supposed that these cells are modified neurotransmitters with a local activity. Their function is local regulation of the lower esophageal sphincter (LES).

Functional lymphangiectasia of the duodenal mucosa.

Tiny millet-like prominent villi with dilated lymphatics may be part of a food-induced functional lymphangiectasia. Peroral olive oil load causes diffuse transient lymphangiectasia, the gross aspect, histological findings and electron-microscopy of which is described in detail.

[alpha1-antitrypsin deficiency. Clinical and morphological aspects during childhood (author's transl)].

In many cases so called neonatal hepatitis of unknown origin nowadays is recognized as a manifestation of alpha1-antitrypsin deficiency. Out of 12 patients with Pi-type ZZ, 5 were diagnosed because of cholestatic jaundice, 2 because of hepato-splenomegaly in the first trimenon, and 3 by family examination. We believe that the affection may be due to a perinataly acquired cytomegalic inclusion disease in one case, in another to a congenital rubella infection. The latter child died at the age of one year because of an esophageal hemorrhage. Over a mean observation time of 3 years the other patients are doing well and show no signs of portal hypertension. The very different course of the hepatopathy is demonstrated. Common bacteria or toxins which do not usually lead to an illness may be realisation factors just as "classical" causes of hepatitis. Up to now it is not known how these factors influence the course of alpha-antitrypsin deficiency.

[Congestive cardiomyopathy after doxorubicin (adriamycin)author's transl)].

Doxorubicin at a dose of 470 mg/m2 body surface, was administered to a 15-year-old boy with metastasising osteosarcoma. The cumulative dose caused severe "secondary congestive cardiomyopathy", which ended fatally, although the cumulative dose was below the recommended limit of 550 mg/m2. No other treatment for the tumour had been given. Postmortem examination revealed obliterative lesions in the intramural coronary vessels and electron-microscopy showed all the features of a cardiomyopathy at cellular level.

[Behaviour of intestinal goblet cells following application of oxidated water soluble terpenes. A pilot study (author's transl)].

1 and 3 h after i.p. application of Ozothin, an oxidation product of Bordeaux terpin oil, in rats there is a significant increase in goblet cells both totally and in the tightly filled ones (respectively in the villi of the small intestine). Double i.v. injection of therapeutic doses of Ozothin (first 14 h and booster 2 h preoperatively) induces a significant decrease in the number of the tightly filled goblet cells. These contradictory results may partly be explained as an initially timed ischesis of mucigenous glands by Ozothin. Statistically significant differences among the different series of tests could be found only in the epithelium of the villi. In the crypts there were similar results lacking statistical significance.
