Cystic lymphangiomatosis of the spleen: a radiologic-pathologic correlation.

Cystic lymphangiomatosis of the spleen is an extremely rare pathologic entity. We report the radiologic, sonographic, and scintigraphic findings in two cases of splenic cystic lymphangiomatosis. In both patients echograms demonstrated multiloculated cystic lesions in the intra-abdominal masses, which were identified on radiocolloid scans as abnormal spleens. The gross and microscopic findings of the excised spleens were characteristic of cystic lymphangiomatosis. The findings on echograms and scintigrams correlated well with the abnormal splenic morphology. The clinical presentation, the findings on diagnostic imaging, and the gross and microscopic appearances of the pathologic spleens in our patients were remarkably similar. When a massively enlarged spleen with a multicystic appearance is encountered during a diagnostic workup, awareness of the scintiscan and sonographic findings might aid in the noninvasive preoperative recognition of cystic lymphangiomatosis.

Leiomyosarcoma of the colon: a case report and review of literature.

Leiomyosarcoma of the colon, excluding that of the rectum, is extremely rare. We report a case of leiomyosarcoma of the ascending colon with a brief review of literature. The clinical manifestations in our case were typical: abdominal pain, diarrhea, anorexia, and a palpable tumor that was firm, smooth, and mobile inspite of its bulk. Toxic features, cachexia and weight loss were less prominent. Morphologically the tumor was of dumbbell type with intracolic and extracolic components. The microscopic picture, consisting of spindle-shaped smooth-muscle cells with pleomorphism and one to three mitotic figures per high-power field, was characteristic. In the absence of local spread, a hemicolectomy was considered curative, but our patient developed extensive metastasis in 25 months. The correlation between mitotic activity of the tumor and prognosis in our case was consistent with that reported in literature.

Evaluation of focal defects on technetium-99m sulfur colloid scans with new hepatobiliary agents.

In 3,600 patients evaluated with technetium-99m sulfur colloid for metastatic liver disease, 40 had equivocal scans due to a solitary defect in the interior margin of the right lobe. In these patients, the authors differentiated the gallbladder fossa from a metastatic focus using one of two new hepatobiliary agents: technetium-99m labeled pyridoxylideneglutamate and paraisopropylacetanilido iminodiacetic acid. In 31 patients, the focal defect filled with radioactive bile, showing that the solitary defect in the radiocolloid scan was a prominent gallbladder fossa. On laparoscopic biopsy, eight of nine patients whose focal defects were not due to the gallbladder were found to have metastatic disease. The ninth patient had carcinoma of the gallbladder. The need for laparoscopic biopsy was obviated in a majority (78%) of the patients by identifying the gallbladder fossa.
