Effect of felodipine on the myogenic response to dynamic stretch in vascular smooth muscle.

In the present experiments we examined the effect of felodipine, a vasoselective dihydropyridine calcium antagonist, on contractile responses to dynamic and static stretch of the isolated portal vein of the rat. Dynamic stretch was applied to the vascular smooth muscle at graded rates (from 0.5-1.5% muscle length s-1). Earlier observations (Johansson & Mellander 1975) of a rate-dependent excitation of the vascular smooth muscle by dynamic stretch were confirmed. Addition of felodipine, 3 nM, reduced the spontaneous activity at static lengths to about 50% but resulted in much stronger inhibition of the dynamic stretch responses. Particularly the rate-dependent increase in active force was no longer evident since the response at high rates of passive lengthening was most clearly reduced by felodipine. By contrast, lowering of the extracellular Ca2+ concentration resulted in a comparable attenuation of the spontaneous contractile activity and of the dynamic stretch responses which still showed the typical rate dependence. Therefore, the pronounced inhibition by felodipine of the dynamic myogenic reactivity of the rat portal vein appeared to be a specific effect and not simply related to the overall reduction in contractile activity. We suggest that felodipine, in addition to its inhibition of action potentials and excitation-contraction coupling may exert a special negative influence on the mechano-electrical coupling, i.e. the process that couples dynamic stretch of the vascular smooth muscle to membrane excitation.

Neuropeptides in cerebrospinal fluid in normal-pressure hydrocephalus and dementia.

Delta-sleep-inducing peptide (DSIP), vasoactive intestinal peptide (VIP), peptide YY (PYY) and somatostatin (SOM) were assayed with specific radioimmunological methods in cerebrospinal fluid (CSF) of healthy volunteers, 12 patients with Alzheimer's disease (AD), 11 patients with multi-infarct dementia (MID) and 10 patients with normal-pressure hydrocephalus (NPH). Patients with NPH were reinvestigated 3 months after a ventriculoperitoneal shunt operation. DSIP, PYY and SOM levels in CSF were decreased in patients with NPH compared to controls. The CSF concentration of SOM was also significantly reduced in patients with AD. No correlations were found between the degree of dementia in any of the illnesses and the CSF concentrations of the peptides. The concentration of DSIP, VIP and SOM increased significantly in parallel to the clinical improvement after the shunt operation in NPH patients.

Behavioral effects of an intrauterine or neonatal diabetic environment in the rat.

Maternal diabetes during pregnancy may cause lasting effects on the psychoneurological development in the offspring. The aim of the present study was to investigate possible effects of an intrauterine or neonatal exposure to a diabetic environment on behavior during infancy and adulthood. On days 4 and 6 of age, offspring of streptozotocin-diabetic rats emitted higher numbers of ultrasound calls compared to control offspring. Neonatally streptozotocin-treated rats explored their environment by diminished sniffing and rearing intensity compared to control rats. However, in adult life neither of these rat groups displayed behavioral differences compared to their respective control group. The results suggest that development of basic behavioral patterns in the rat proceed almost normally despite exposure to a diabetic environment in the early embryonic period or in early infancy.

Percutaneous nephrolithotomy--the birth of a new technique.

Extraction of renal calculi via a percutaneous track was for the first time practised at the Karolinska Hospital in 1973. After very strict indications in the beginning because of fear of uncontrollable bleeding, the method was found to be safe and the technique was spread all over the world. Now being to a large extent replaced by extracorporeal shock wave lithotripsy (ESWL), percutaneous nephrolithotomy (PNL) has still many applications e.g. alone in obstructed kidneys or together with ESWL in staghorn calculi.

Radioimmunoscintigraphy of gynecologic tumors with 131I-labeled anti-PLAP monoclonal antibodies.

Radioimmunoscintigraphy (RIS) was performed in 20 patients with gynecologic tumors, 14 ovarian, 5 cervical, and one endometrial carcinoma. One murine monoclonal antibody (mab) against placental alkaline phosphatase (H7) was used after radiolabeling with 131I. The labeling procedure yielded antibodies with specific activity varying between 60 and 73 MBq/mg mab. Each patient received 57 to 100 MBq of the preparation. RIS was performed 7 to 35 days later. Patients with ovarian adenocarcinoma had an accumulation of activity on RIS at tumor sites (79%, 11/14) verified by ultrasonography, CT, and clinical examination. A low or absent accumulation of activity was seen in patients with cervical tumors. The patient with an endometrial adenocarcinoma was seen to have an activity accumulation at RIS corresponding to tumor sites determined by ultrasound and/or CT. It is concluded that RIS using monoclonal antibodies against placental alkaline phosphatase can provide information which will supplement that gained from other investigations of patients with ovarian adenocarcinomas.

Relative biological effectiveness of high-energy photons (up to 50 MV) and electrons (50 MeV).

In vitro studies of the relative biological effectiveness (RBE) of 50-MV X rays have shown an RBE of 1.1 relative to 4-MV X rays. This will be important in clinical radiotherapy. The aim of this study was to verify these results and to investigate whether photonuclear processes might cause the difference in RBE. To do so, 50- and 20-MV X rays and 50-MeV electrons were investigated with respect to RBE. Chinese hamster V79 cells were irradiated in a specially designed system which allows for a high reproducibility of geometry and dosimetry. Fractionation experiments were also carried out to establish the RBE at the clinically relevant dose level, 2 Gy. Fricke dosimetry was used, and the results were confirmed with ionization chamber measurements. The RBE for 50-MV X rays was estimated to be 1.14 at a surviving fraction of 0.1 and 1.12 at a surviving fraction of 0.01. The RBEs for the other qualities were equal to one. The RBE calculated for the 2 Gy/fraction experiments was 1.17.

Karyotypic patterns in chronic myeloproliferative disorders: report on 74 cases and review of the literature.

During the 15 year period 1975-1989, 74 cases of chronic myeloproliferative disorder (CMPD) were cytogenetically analyzed in our department. Thirty patients had polycythemia vera (PV), 23 had idiopathic myelofibrosis (MFS), 15 had idiopathic thrombocythemia (IT), and six had unclassifiable CMPD (UCMPD). The overall frequency of clonal chromosome aberrations was 36% (50% in PV, 30% in MFS, 27% in IT, and 17% in UCMPD). The frequency was markedly higher (53%) in the subset of patients who had received myelosuppressive therapy and/or had developed acute leukemia prior to the initial cytogenetic analysis. The pattern of the chromosome rearrangements in our series is in agreement with the karyotypic findings in the 411 previously reported cases of CMPD. Trisomy 8 and 9 and del(20q) dominate in PV. The picture in MFS is more heterogenous with several aberrations, dup(1q), -5, del(5q), -7, del(7q), +8, +9, del(13q), del(20q), and +21, found equally frequently. No pathognomonic chromosome aberration has been detected in IT, but t(9;22) occurs more often than other changes. Thus, although a non-random cytogenetic pattern is discernible in CMPD, there is considerable overlap both with other myeloid malignancies and among the different CMPD subtypes.

Cytogenetic findings in acute megakaryoblastic leukemia (ANLL-M7).

The clinical, morphologic, and cytogenetic findings in a patient with acute megakaryoblastic leukemia (ANLL-M7) are described. At the time of diagnosis, the karyotype contained three related clones: 47,XY,t(3;6)(p13;q27), +8,t(12;22)(p13;q12), +15, -21, -22, +der(22)t(21;22)(q11;p13)/47,XY, +8,del(9)(q22),t(12;22), +15, -21, -22, +der(22)/46,XY, +8, t(12;22), -21, -22, +der(22). Complete remission was achieved with intensive combination chemotherapy. At relapse 20 months later, there was clear evidence of clonal evolution, and the karyotype was now 45,XY,t(2;18)(q33;p11),del(9)(q22),t(12;22)(p13;q12), -16, +der(16)t(8;16)(q13;p13), -21, -22, +der(22)t(21;22)(q11;p13)/46,XY,t(2;18),del(9),t(12;22), -16, +der(16), -21, -22, +der(22), +mar. A comparison with the few previously cytogenetically analyzed cases of ANLL-M7 reveals that structural rearrangements of chromosomes 21 and 22 might be of primary importance in the pathogenesis of acute megakaryoblastic leukemia.
