Transthyretin in endocrine pancreatic tumors.

The occurrence of transthyretin (TTR) in 25 endocrine pancreatic tumors was investigated by immunohistochemical methods using both polyclonal and monoclonal antibodies. All malignant insulinomas were strongly TTR immunoreactive, more so than their benign counterparts, which in some cases were TTR negative. All glucagonomas and nonfunctioning tumors were TTR immunoreactive, whereas gastrinomas and VIPomas were TTR negative. TTR, chromogranin A, and the argyrophil reaction (Grimelius' silver technique) had similar distributions among the cells in many, but not all, tumors. Coexistence of TTR with glucagon, insulin, or pancreatic polypeptide in tumor cells was demonstrated. TTR was also quantitated in preoperative serum samples by electroimmuno assay in some cases. Although one patient with a glucagonoma had a markedly increased serum TTR level, five other patients with endocrine tumors, including two patients with glucagonoma, had TTR levels in serum that were within or below the reference range.

Effects of dosage and infusion time on the incorporation of 5-fluorouracil into DNA and RNA of normal tissues and an adenocarcinoma transplanted to rat liver.

In a model of secondary liver cancer in Wistar rats the incorporation of 5-FUra into the acid soluble fraction, RNA, and DNA of several normal tissues and of an adenocarcinoma of the colon transplanted to the liver was determined; 300, 1,200 or 24,000 nmoles of 5-FUra were infused via the gastroduodenal artery for 0.5, 2, or 24 hr. The rats were killed 1.5, 3, or 24 hr after the beginning of the infusions. In general, higher doses resulted in a higher labelling. However, the ratio of incorporation into tumor RNA compared to normal tissue RNA was higher at the 1,200 than at the 24,000 nmole dosage. There was a decreased RNA/DNA ratio in the tumor at 24 hr after 24,000 nmoles of 5-FUra had been infused over 0.5 or 2 hr, indicating decreased synthesis and/or increased breakdown of RNA.

Immunohistochemical monitoring of metastatic colorectal carcinoma in patients treated with monoclonal antibodies (MAb 17-1A).

A therapeutic trial using repeated doses of a mouse monoclonal antibody against the tumor-associated antigen (TAA) CO17-1A in metastatic colorectal carcinomas was carried out. Metastatic lesions sampled by repeated thick needle (1.2 mm) biopsies during therapy were examined immunohistochemically for the presence of various TAAs, mouse IgG, complement, and infiltrating leukocytes. The CO17-1A was consistently expressed in all cases along the basement membrane of tumor glands and could only be demonstrated on cryostat sections whereas the TAAs GICA19-9, GA73-3, and Br55-2 were also visualized in B5-fixed paraffin-embedded biopsies. The CO17-1A and GA73-3 were predominantly present at the basal region in contrast to the GICA 19-9 and Br55-2 which were predominant at the luminal and the apical region of the tumor glands. Antigenic modulation was not seen either after 24-72 h or during prolonged treatment. In all cases the infused mouse IgG was detected, from 24 h after infusion up to 6-8 weeks, mainly along the basal region of tumor glands. In 13/14 posttreatment biopsies, complement factor C3 was found at the same sites as mouse IgG. In 6 out of 9 posttreatment biopsies an increase in mononuclear cells (monocytes, natural killer (NK) cells and/or T cells) was observed. Monocytes were close to the tumor cells whereas NK cells and T cells were predominantly scattered in the stroma.

The effect of diabetes on phosphatidylinositol turnover and calcium influx in myocardium.

Diabetes was induced in rats by administration of streptozotocin. Diabetes occurred within 24 h after treatment. Two forms of diabetes were studied, an acute form (4 days) and a chronic form (2 months). In a separate experiment the effect of insulin and an aldose reductase inhibitor on acute diabetes was studied. Phosphoinositide labelling was done in biopsies of heart with [3H] myo-inositol. It was shown that the incorporation of myo-inositol amounted to about 65% in acute diabetes and 80% in chronic diabetes compared to age-matched controls. The incorporation both in atria and ventricles was affected in a similar way. Muscarinic receptor-mediated phosphatidylinositol breakdown and release of myo-Ins-1 P (myo-inositol 1-phosphate) was unaffected in diabetic hearts in the chronic model. In hearts of diabetic ketotic animals uncoupling of the muscarinic receptor from the phosphoinositide metabolism was apparent. Calcium net influx was significantly reduced in both acute and chronic diabetes compared to age-matched controls. Insulin supplementation to acute diabetic animals significantly improved phosphoinositide labelling with [3H] myo-inositol. No improvement was seen in calcium transport. An aldose reductase inhibitor also facilitated phosphoinositide labelling without improving calcium transport. It is suggested that phosphoinositide metabolism and calcium entry through the slow inward current are independent of one another and the former is sensitive to insulin. It is suggested that insulin by regulating the pool of phosphoinositides and release of endogenous calcium may modulate cardiac function.

A comparison of nail-plate fixation and Ender's nailing in pertrochanteric fractures.

A randomized comparison was made of condylocephalic (Ender) nails and a nail-plate technique (McLaughlin) for fixation of trochanteric fractures, to evaluate the consumption of acute hospital bed-days, number of reoperations, peroperative blood loss, and operation time. Reoperations were performed in 30% of the Ender group and in 10% of the McLaughlin group and blood loss was doubled for the McLaughlin method. Our study has shown, that in our hands, patients operated on with Ender nails had more reoperations, but in spite of this the total cost, in the form of days in hospital or operation time was not increased.

Studies on calcium exchange in platelets in human diabetes.

Calcium has a key role in platelet aggregation. In order to assess the role of calcium metabolism in the platelet in diabetes mellitus, calcium exchange into and out of the cytosol was investigated in platelets from eight patients with insulin-treated diabetes with a mean duration of 10 years without any clinical complications. Their mean HbAIc (glycosylated haemoglobin) was 9% (normal range 3.4-5.4%). Influx rate of 45Ca2+ was significantly increased by 35% in diabetic platelets compared with controls. The efflux rate was significantly reduced during the first 15 min but total efflux measured over 2 h was equal in diabetic and control platelets. Access to sequestered non-mitochondrial calcium in the dense tubular system was gained by permeabilization of the cells with saponin. Into cells loaded with 45Ca2+, Inositol 1, 4, 5 triphosphate (IP3) was introduced and release of the sequestered ion would be determined. In control platelets calcium release was prompt and amounted to 43%. In diabetic platelets the response to IP3 was blunted and was only 17%. The nutritional and hormonal status influenced the response to IP3 in diabetes and, to a lesser extent, in controls. In the fasting state and without insulin the release was reduced but in the fed state and after insulin the release of calcium from diabetic platelets was equal to those of controls. It is concluded that abnormalities exist in platelet calcium handling in diabetes that can be influenced by insulin and the nutritional status.(ABSTRACT TRUNCATED AT 250 WORDS)

Occurrence of an insulin-like peptide in extracts of human nervous tissue.

Peripheral nerves of the cat such as the vagal and the sciatic nerves have been shown to contain a peptide with insulin-like properties. The aim of the present study was to investigate whether insulin-like immunoreactivity (ILI) can be demonstrated in human nervous tissue collected from autopsy material. Biopsies were taken in connection with autopsy from various peripheral nerves and their content of ILI was investigated. ILI in amounts up to about 12 ng g-1 was found in about 30% of all biopsies taken from peripheral nerves. The ILI coeluted with a bovine insulin standard in an HPLC system indicating that it corresponds to a peptide identical with or similar to pancreatic insulin. Autopsy specimens taken from the sciatic nerve of individuals with diabetes type II or from individuals without established diabetes contained similar amounts of ILI.

One-year incidence of occupational injuries among teenagers in a Swedish rural municipality.

The aim of this study was to investigate occupational injuries among teenagers in a defined population and geographical area with regard to incidence and severity and to compare our registration system with that of the Swedish Occupational Injury Information System. All occupational accidents in the age group 15-19 years were registered at public health centres. We found a higher incidence compared to school accidents in the same age group as well as in relation to occupational accidents in the older population. Part-time work, lack of experience, temporary employment, unskilled work, and age-specific behavioural patterns are factors that might explain the higher incidence rate. The highest rates were found in service, military and unspecified occupations. Only 25% of the work-related accidents in our study were registered by the Swedish Occupational Injury System (ISA). It seems possible to reduce the number of accidents through the instigation of fairly simple measures. The coverage of the registration by the ISA seems to be incomplete.

Intraarticular pressure during operation of cervical hip fractures.

The intraarticular pressure was measured during nailing of femoral neck fractures in 21 patients. A three-flanged nail or three screws were used randomly. During traction and inward rotation of the leg, the intracapsular pressure increased over diastolic values, but as soon as the lateral cortex was penetrated, the pressure dropped. The hammering of the nail did not increase the intraarticular pressure.

Coxarthrosis and labour.

To investigate whether environmentally associated stress may contribute to the development of coxarthrosis, 85 males with primary coxarthrosis and 262 male urological patients were questioned about their occupation, and their hips were graded radiologically. Patients with coxarthrosis had been more frequently exposed to heavy labour, heavy lifting, and much tractor driving. Our results indicate that environmentally associated stress, as reflected by a varied life style or occupation, may contribute to the development of coxarthrosis.

Renal growth after neonatal urinary tract infection.

This study presents the result of 12-21 years' follow-up in a group of children with neonatal urinary tract infection (onset within 1 month after birth) in whom early renal growth retardation was noted without concomitant classical renal scarring. In all cases the neonatal infection was diagnosed and treated within a few days of onset and the patients were closely supervised thereafter. Renal length, parenchymal thickness and area were measured at urography. At first follow-up (22 children, mean age 4.1 years) a significant reduction of renal parenchymal thickness was noted. Long-term follow-up (18 patients, mean age 17 years) demonstrated a normalization of renal size in the entire group, although less complete in the subgroup with reflux. There were two major findings in the present study. Firstly, renal growth retardation was seen after neonatal infection, both with and without reflux. Secondly, normalization of renal size in previously small kidneys was demonstrated, suggesting that growth retardation can be a reversible phenomenon. The tendency for such normalization was slightly more marked in children without reflux. Reduction of parenchymal thickness without calyceal deformity, therefore, does not necessarily mean irreversible damage, and differentiation between permanent scarring and temporary growth retardation can thus only be made at later follow-up, possibly not until after puberty. The demonstration of renal growth retardation in spite of early diagnosis and treatment emphasizes the great vulnerability of the kidney in the newborn.

A computerized system for handling renal size measurements from urograms.

The size of a kidney, as measured on a urogram, is a sensitive indicator of renal damage in a child with urinary tract infection and renal surface area correlates well with glomerular filtration rate. Sequential measurements can be invaluable in evaluating the efficacy of a regimen of treatment. A system utilizing a personal microcomputer has been developed to facilitate the measuring procedure and the handling and analysis of data.

Balloon dilatation of bilharzial ureteric strictures.

Balloon dilatation was performed in nine patients with bilharzial ureteric strictures. In six patients a percutaneous, antegrade approach was used and in three a balloon catheter was introduced into the ureter via a cystoscope. In another four patients, negotiation of the strictured area with a guide wire failed. In all dilated patients, symptomatic and urographic relief of the obstruction was obtained during follow-up periods of 3 to 38 months (mean 19). One patient was re-dilated after 24 months and has remained symptom-free for a further 18 months. Balloon dilatation seems to be a promising method of treating this condition.

Effect of corticosteroids on muscarinic receptors on cultured myocardial cells.

Rat myocardial cells in tissue culture were utilized to investigate modulation by corticosteroids on muscarinic receptor affinity. Competition binding experiments between tritiated quinuclidinyl benzilate and carbachol were performed on the muscarinic receptor both on whole cells and on a crude membrane preparation from cultured myocardial cells. Hydrocortisone was shown to promote a transition of the high affinity site of the muscarinic receptor towards super-high affinity. This was analogous with the effect of divalent ions. The affinity shift was detectable after 6 hr exposure to 0.1 microgram/ml of hydrocortisone in the tissue culture dishes but was abolished by concomitant incubation with a protein synthesis inhibitor (10 microM puromycin). The response on agonist binding of Gpp(NH)p in the presence of manganese ions was altered indicating that the adenylate cyclase-coupled muscarinic receptor nucleotide binding protein was modulated. No effects of corticosteroid exposure were noted on phosphatidyl inositol turnover.

Home accidents among children and teenagers in a Swedish rural municipality.

The aim of this study was to investigate home accidents among children and teenagers with regard to incidence and severity and to compare the accident rate at home with that of nursing homes/day-care centres. All accidents in the age group 0-19 years, in a defined population, were, during a period of one year, registered at public care centres. Our findings show that home accidents contributed to 26% of all accidents in the age group 0-19 and about 10% of all health care consumption. The accident rate decreased with age. The severity was about the same as in other accidents. 10% were hospitalized. When time of exposure had been taken into consideration there was no difference as to incidence between the accident rate at home as compared with the rate in nursing homes/daycare centres. When comparing with studies in the past there seems to be a lower rate of home accidents which can be explained by secular changes and local preventive measures.

Synaptophysin. A new marker for pancreatic neuroendocrine tumors.

Synaptophysin (SYP) is a glycoprotein recently isolated from presynaptic vesicles of bovine neurons. Initial studies have demonstrated its presence in neurons in the brain, spinal cord and retina, and in adrenal medullary cells. A subsequent study demonstrated it in pancreatic islet cells and certain neuroendocrine (NE) neoplasms, including several pancreatic islet cell tumors. Based on these preliminary observations, we examined, by immunohistochemistry, conventionally fixed, paraffin sections of 57 pancreatic endocrine tumors with a monoclonal antibody to SYP. Furthermore, we compared the SYP immunoreactivity of 30 of these same tumors with that of neuron-specific enolase (NSE) and of chromogranin (CG). SYP was demonstrated in all but one of the 57 tumors. In the comparative study, for which material was available in only 30 cases, SYP and NSE were present in 29 of the tumors, whereas CG was seen in only 15 cases. We conclude that SYP is a highly sensitive and useful marker for pancreatic NE neoplasms. Moreover, in view of the increasingly evident limited specificity of NSE, SYP should be considered the marker of choice for pancreatic NE neoplasms.

Muscarinic receptors in mammalian myocardium: effects of atrial and ventricular receptors on phosphatidylinositol metabolism and adenylate cyclase.

Cat myocardium was used to investigate muscarinic receptor function in atria and ventricles. Carbachol and oxotremorine were used to determine agonist binding to the muscarinic receptors. It was found that carbachol was bound with almost the same characteristics in atria (KdH 1 microM; KdL 150 microM) as in ventricles (KdH 3 microM; KdL 150 microM). However, in the presence of guanylylimidodiphosphate Gpp(NH)p a difference was apparent so that the ventricular curve was shifted to a majority of low affinity sites whereas in atria two affinity sites remained even if the guanine nucleotide concentration was increased to 10 mM. Oxotremorine was bound with almost equal affinity in both atria and ventricles. The addition of Gpp(NH)p left all the receptors in the low affinity state irrespective of receptor localisation. The two agonists were also used to determine inhibition of adenylate cyclase activity. It was shown that the magnitude of adenylate cyclase inhibition was more pronounced in ventricles than in atria whether it was induced by oxotremorine or carbachol. When the effects of muscarinic agonists were determined on phosphatidylinositol metabolism it was shown that carbachol mediated a greater effect in atria than in ventricles. Almost no effect was seen with oxotremorine on phosphatidylinositol breakdown. Pirenzipine binding showed the presence of M1 receptors both in atria and ventricles. On the basis of diversity of muscarinic agonists on function and receptor occupancy it is suggested that heterogeneity exists for muscarinic receptors in both atria and ventricles.
