Testicular sarcoidosis: case report and review of the literature.

We report a case of clinically evident and histologically proved sarcoidosis of the testicle. Six cases have been reported in the literature. This condition is present most commonly in black patients. Sarcoidosis is known to be associated with malignancy and several cases of coexistent testicular cancer have been reported in the white population. Management should include complete testicular neoplasia diagnostic studies, biopsy of peripheral or mediastinal lymph nodes, and radical orchiectomy in cases of unilateral lesions. Testicular biopsy from the inguinal approach and frozen section evaluation may be considered in bilateral lesions or in the patient with previous orchiectomy for testicular sarcoidosis.

Renal carbuncle: comparison between surgical open drainage and closed percutaneous drainage.

This article represents a retrospective study of 12 patients with renal carbuncle treated at the combined urological services of The Mason Clinic in Seattle, Washington, and Madigan Army Medical Center in Tacoma, Washington. All patients were initially treated with antibiotics. Two recovered without further treatment. Of the 10 patients who failed to respond, 2 underwent nephrectomy for nonfunctioning kidneys, 4 underwent closed percutaneous drainage, and 4 underwent surgical open drainage. All patients treated with open surgical drainage recovered and retained functioning kidneys. Of the 4 patients who underwent closed percutaneous drainage, 2 failed to respond and required subsequent emergency nephrectomy because of sepsis. The authors believe that renal exploration and open drainage should be the initial definitive mode of surgical treatment of renal carbuncle in those patients who fail antibiotic therapy alone.

Urolithiasis and race: another viewpoint.

We describe 193 consecutive individuals with documented urolithiasis in a military population. In contrast to other reports, the prevalence of stone disease was nearly identical in white-and black patients. This unique black population refutes the concept that urinary calculi are rare in blacks.

Bilateral synchronous germ cell tumors.

We present cases 38 and 39 of primary bilateral synchronous germ cell tumors of the testes and review the literature. Seminoma (42 of 68 cases) is the most common cell type reported, and bilateral seminoma is the most common cell type combination (19 of 34 cases). The incidence of cryptorchidism is not increased in patients with bilateral synchronous germ cell tumors. A tendency towards occurrence in older patients is noted. The stage of disease at presentation appears to be no worse than that for unilateral disease.

Solitary crossed renal ectopia.

An 18-month-old girl had crossed ectopia involving a solitary left kidney. There have been 26 cases reported previously. Several theories for this occurrence are discussed.

Intracranial metastases in prostate cancer.

In a clinicopathologic review of 126 autopsied cases with prostate cancer, 14 demonstrated intracranial metastases. Only two of nine symptomatic patients were evaluated for suspected central nervous system metastases prior to death, and five asymptomatic patients were incidentally found to have metastases at autopsy. Intracranial metastases in prostate cancer occur in the setting of widespread disease, and tissue pathology may reveal moderately to poorly differentiated tumor (11 of 14 cases).

Brain adenosine 5'-triphosphate-creatine phosphotransferase.

1. The purification of creatine kinase (ATP-creatine phosphotransferase, EC 2.7.3.2) from ox brain by a method that is quicker, simpler and gives much higher yields than other published procedures is described. 2. Stoicheiometric inhibition studies with iodoacetate showed that the enzyme, like that from muscle, has two reactive thiol groups that are essential for enzyme activity. 3. The amino acid sequence around the essential thiol groups was determined and found to be virtually identical with that in creatine kinases from rabbit and ox muscle, and very similar to that found in arginine kinase; the evolutionary significance of this is discussed. 4. The identification of DNS-amino acids on thin layers of silica gel was found to have, in many cases, distinct advantages over that on polyamide layers.

The danger of applying group-level utilities in decision analyses of the treatment of localized prostate cancer in individual patients.

The optimal management strategy for men who have localized prostate cancer remains controversial. This study examines the extent to which suggested treatment based on the perspective of a group or society agrees with that derived from individual patients' preferences. A previously published decision analysis for localized prostate cancer was used to suggest the treatment that maximized quality-adjusted life expectancy. Two treatment recommendations were obtained for each patient: the first (group-level) was derived using the mean utilities of the cohort; the second (individual-level) used his own set of utilities. Group-level utilities misrepresented 25-48% of individuals' preferences depending on the grade of tumor modeled. The best kappa measure achieved between group and individual preferences was 0.11. The average quality-adjusted life years lost due to misrepresentation of preference was as high as 1.7 quality-adjusted life years. Use of aggregated utilities in a group-level decision analysis can ignore the substantial variability at the individual level. Caution is needed when applying a group-level recommendation to the treatment of localized prostate cancer in an individual patient.

The effect of digital rectal examination on prostate-specific antigen levels.

OBJECTIVE--To identify the effect of digital rectal examination (DRE) on serum prostate-specific antigen (PSA) levels. DESIGN--A prospective trial before and after DRE. SETTING--Multicenter outpatient screening program. PATIENTS--A total of 2754 healthy men aged 40 years and older who presented to a prostate cancer screening program and consented to two phlebotomies. MAIN OUTCOME MEASURE--Changes in serum PSA levels after DRE. RESULTS--Patients were divided into four groups based on their initial serum PSA levels. The levels were chosen based on previous studies that showed different incidences of prostate cancer within these groups. The two groups with the lowest initial PSA values (0.1 through 4 micrograms/L and 4.1 through 10 micrograms/L) were found to have statistically insignificant changes in the serum PSA levels after DRE. The group with initial PSA levels of 10.1 through 20 micrograms/L had increases in serum PSA values that showed a trend toward statistical significance. The group with initial PSA levels of greater than 20 micrograms/L had statistically significant increases in serum PSA values after DRE. The alterations in serum PSA levels in the two groups with the highest PSA values were not clinically important as the patients' clinical treatment was not altered. CONCLUSIONS--No clinically important effects on serum PSA levels were noted after DRE.
