Neuronal correlates of encoding and retrieval in episodic memory during a paired-word association learning task: a functional magnetic resonance imaging study.

The investigation of memory function using functional magnetic resonance imaging (fMRI) is an expanding field of research. The aim of this study was to demonstrate brain-activity patterns related to a word-pair association task employing a whole-brain EPI sequence. Six right-handed, healthy male volunteers (mean age: 27.5 years) took part in the study. fMRI was performed at a field strength of 1. 5 Tesla with 26-32 slices parallel to the AC-PC line, depending on individual brain size. Distributed brain regions were activated in episodic encoding and retrieval with similarities, but also (distinct) differences in activation patterns. Bilateral prefrontal cortical areas were involved when comparing encoding as well as retrieval to the reference condition (nonsense words). Furthermore, activation was observed in cerebellar areas during encoding, and activation in bilateral parietal areas (precuneus and inferior parietal cortex) was differentially more pronounced during retrieval. The activation of left dorsomedial thalamus during retrieval of high imagery-content word-pair associates may point to the role of this structure in episodic retrieval. The direct cognitive subtraction of encoding minus retrieval yielded a differentially larger left prefrontal activation. There was a differentially higher right prefrontal activation during retrieval than during encoding, underlining the proposed right/left asymmetry for episodic memory processes.

Functional anatomy of intrinsic alertness: evidence for a fronto-parietal-thalamic-brainstem network in the right hemisphere.

Alertness, the most basic intensity aspect of attention, probably is a prerequisite for the more complex and capacity demanding domains of attention selectivity. Behaviorally, intrinsic alertness represents the internal (cognitive) control of wakefulness and arousal; typical tasks to assess optimal levels of intrinsic alertness are simple reaction time measurements without preceding warning stimuli. Up until now only parts of the cerebral network subserving alertness have been revealed in animal, lesion, and functional imaging studies. Here, in a 15O-butanol PET activation study in 15 right-handed young healthy male volunteers for this basic attention function we found an extended right hemisphere network including frontal (anterior cingulate-dorsolateral cortical)-inferior parietal-thalamic (pulvinar and possibly the reticular nucleus) and brainstem (ponto-mesencephalic tegmentum, possibly involving the locus coeruleus) structures, when subjects waited for and rapidly responded to a centrally presented white dot by pressing a response key with the right-hand thumb.

Network analysis in episodic encoding and retrieval of word-pair associates: a PET study.

The involvement of distributed brain regions in declarative memory has been hypothesized based on studies with verbal memory tasks. To characterize episodic declarative memory function further, 14 right-handed volunteers performed a visual verbal learning task using paired word associates. The volunteers underwent positron emission tomography. 15O-butanol was used as a tracer of regional cerebral blood flow (rCBF). Inter-regional functional interactions were assessed based on within-task, across-subject inter-regional rCBF correlations. Anatomical connections between brain areas were based on known anatomy. Structural equation modelling was used to calculate the path coefficients representing the magnitudes of the functional influences of each area on the ones to which it is connected by anatomical pathways. The encoding and the retrieval network elicit similarities in a general manner but also differences. Strong functional linkages involving visual integration areas, parahippocampal regions, left precuneus and cingulate gyrus were found in both encoding and retrieval; the functional linkages between posterior regions and prefrontal regions were more closely linked during encoding, whereas functional linkages between the left parahippocampal region and posterior cingulate as well as extrastriate areas and posterior cingulate gyrus were stronger during retrieval. In conclusion, these findings support the idea of a global bihemispheric, asymmetric encoding/retrieval network subserving episodic declarative memory. Our results further underline the role of the precuneus in episodic memory, not only during retrieval but also during encoding.

Episodic retrieval activates the precuneus irrespective of the imagery content of word pair associates. A PET study.

The aim of this study was to evaluate further the role of the precuneus in episodic memory retrieval. The specific hypothesis addressed was that the precuneus is involved in episodic memory retrieval irrespective of the imagery content. Two groups of six right-handed normal male volunteers took part in the study. Each subject underwent six [15O]butanol-PET scans. In each of the six trials, the memory task began with the injection of a bolus of 1500 MBq of [15O]butanol. For Group 1, 12 word pair associates were presented visually, for Group 2 auditorily. The subjects of each group had to learn and retrieve two sets of 12 word pairs each. One set consisted of highly imaginable words and another one of abstract words. Words of both sets were not related semantically, representing 'hard' associations. The presentations of nonsense words served as reference conditions. We demonstrate that the precuneus shows consistent activation during episodic memory retrieval. Precuneus activation occurred in visual and auditory presentation modalities and for both highly imaginable and abstract words. The present study therefore provides further evidence that the precuneus has a specific function in episodic memory retrieval as a multimodal association area.

Encoding and retrieval in declarative learning: a positron emission tomography study.

We present neuroanatomical correlates of encoding and retrieval in an episodic memory task using visually presented highly imaginable word-pair associates. A total of 13 right-handed normal male volunteers took part in the study. Each subject underwent six (15)O-butanol PET scans. On each of the six trials the memory task began 30 s before the injection of a bolus of (15)O-butanol. The subjects had to learn and retrieve 12 word pairs (highly imaginable words, not semantically related, hard associations). The presentation of nonsense words served as a reference condition. Recall accuracy after 2-4 presentations was 66.1%+/-21.1 correct during the PET measurement so that scanning during the retrieval of word pair associates was appropriate to capture the brain activity associated with retrieval. The results obtained support the hypothesis of the presence of an asymmetric network consisting of distributed brain structures subserving associative memory. We show left dorsolateral prefrontal activation during the encoding of visually presented word pair associates, whereas retrieval led to bilateral frontal activation. Furthermore, the importance of the precuneus in the retrieval of highly imaginable word-pair associates using visual imagery as a mnemonic strategy is demonstrated.

Early diagnosis of cardiac involvement in systemic sclerosis by 123I-MIBG neurotransmitter scintigraphy.

Cardiac involvement of systemic sclerosis (SSc) is associated with a poor prognosis. Arrhythmias and conduction disturbances are a known feature of SSc. From histopathological examinations, it is known that the conducting system is secondarily involved as a result of focal fibrosis of the myocardium. Fibrotic changes are believed to be related to vasospasms, such as Raynaud's phenomenon. The sympathetic nervous system is very sensitive to ischaemia, impairing the energy-dependent uptake of intraneuronal norepinephrine. 123I-metaiodobenzylguanidine (123I-MIBG) is a metabolic analogue of norepinephrine and can therefore be used as a marker of norepinephrine depletion. The aim of the study was to evaluate the incidence and extent of SSc-associated ischaemic damage due to primary cardiac involvement by assessing intraneuronal 123I-MIBG uptake and distribution. Supplementary myocardial stress and rest perfusion scintigraphy, together with cardiological examinations (including an exercise stress test, Holter ECG and echocardiography), were performed in 18 patients. None of the patients showed evidence of ischaemia upon myocardial perfusion stress SPET or exercise stress. ECG at rest detected pathological conductance disturbances in one patient (6%). Holter ECG evoked pathological arrhythmias in three patients (17%). The echocardiograms of four patients (22%) showed a slight impairment of left ventricular diastolic function. 123I-MIBG scintigraphy revealed an inhomogeneous reduction of norepinephrine content in 15 patients (83%). It would appear that 123I-MIBG scintigraphy is able to detect cardiac SSc involvement prior to cardiological investigations.

[O-15-butanol Pet activation study on the cerebral representation of declarative memory].

AIM: In this study, neuroanatomical correlates of encoding and retrieval in paired associate learning were evaluated with positron emission tomography using auditorily presented highly imaginable words. METHODS: Six right-handed normal male volunteers took part in the study. Each subject underwent six O-15-butanol PET scans. On each of the six trials the memory task began with the injection of a bolus of O-15-butanol. The subjects had to learn and retrieve twelve word pairs (highly imaginable words, not semantically related). The presentation of nonsense words served as reference condition. RESULTS: Recall accuracy after 2-4 presentations was high during the PET measurement. In both encoding and retrieval we found anterior cingulate activation. We show bilateral dorsolateral prefrontal activation during the encoding of auditorily presented word pair associates, whereas retrieval led to left frontal activation. Furthermore, we demonstrate the importance of the precuneus in the retrieval of highly imaginable word-pair associates. CONCLUSION: Our results support the hypothesis of the presence of distributed widespread brain structures subserving episodic declarative memory.

Human monoclonal antibody 99mTc-88BV59: detection of colorectal cancer, recurrent or metastatic disease and immunogenicity assessment.

This study presents immunoscintigraphic results in 24 patients suffering from primary colorectal cancer, recurrent or metastatic disease after the injection of 1197-1351 MBq technetium-99m labelled totally human monoclonal antibody 88BV59. Labelling efficacy of 99mTc-88BV59 ranged from 97% to 99%. Immunoscintigraphy was performed 18-20 h after injection. Scintigraphic findings were compared with those of computed tomography (CT). Patients underwent surgery in order to evaluate immunoscintigraphic findings histologically. Sera of the patients (before injection and 1 and 3 months post infusion) were analysed for the presence of human anti-human antibodies (HAHA). None of the patients showed a HAHA response as assessed by a solid-phase ELISA assay. The antibody scan detected about 25% more lesions than CT. In the detection of extrahepatic disease, the sensitivity of the antibody scan proved to be 68%, whereas the sensitivity of CT was 41%.

Markov random field segmentation of brain MR images.

We describe a fully-automatic three-dimensional (3-D)-segmentation technique for brain magnetic resonance (MR) images. By means of Markov random fields (MRF's) the segmentation algorithm captures three features that are of special importance for MR images, i.e., nonparametric distributions of tissue intensities, neighborhood correlations, and signal inhomogeneities. Detailed simulations and real MR images demonstrate the performance of the segmentation algorithm. In particular, the impact of noise, inhomogeneity, smoothing, and structure thickness are analyzed quantitatively. Even single-echo MR images are well classified into gray matter, white matter, cerebrospinal fluid, scalp-bone, and background. A simulated annealing and an iterated conditional modes implementation are presented.

[Psoriatic osteoarthropathy and bone scintigraphy].

Psoriasis is a generalized disease that affects bone as well as skin. Three patterns of bone involvement can be differentiated in psoriatic arthritis distal, central, and distal combined with central. Distal involvement is more specific for psoriatic arthritis, with radial and/or transverse involvement of the distal interphalangeal joints. The progression dynamic and the pattern of distribution are different from those pattern of rheumatoid arthritis: psoriatic arthritis originates in the periarticular bone and extends towards the synovia, whereas in rheumatoid arthritis it extends in the opposite direction. In psoriatic arthritis anterior chest wall syndrome, insertion tendinopathies, and localized or diffuse bone disease are often observed. Bone scintigraphy is more sensitive in the diagnosis of psoriatic bone involvement than clinical examination or conventional radiological imaging, allowing earlier diagnosis through the visualization and documentation of specific patterns and presence of disease in multiple sites. It also allows more discriminating selection of subsequent X-ray examinations to limit radiation exposure. Thus, bone scintigraphy is now the most important diagnostic tool in the assessment of psoriatic arthritis.

[Evaluation of the sympathetic nervous system in silent ischemia with 123I-metaiodobenzylguanidine (MIBG)].

AIM: Examination of the integrity of the sympathetic nervous system in patients with angiographically proven coronary artery disease and ECG documented asymptomatic ST-segment depression. METHOD: Stress and rest myocardial perfusion scintigraphy using either Thallium-201 or 99mTc-isonitrile was performed in SPECT technique in order to localize ischemia or scar associated perfusion defects. 15 min and 4 h p.i. static anterior 123I-MIBG uptake was acquired. In order to localize norepinephrine depletion 4 h p.i. additional 123I-MIBG SPECT acquisition was performed. Incidence of arrhythmias was investigated by Holter ECG. Patients with diabetes mellitus were excluded. RESULTS: SPECT images showed in all patients regional 123I-MIBG depletion which corresponded with scintigraphically infarcted or ischemic regions. Well perfused myocardial regions matched with regular 123I-MIBG uptake. There was no evidence of increased arrhythmias in long-term ECG. CONCLUSION: The finding of regular 123I-MIBG uptake in well-perfused myocardium and infarction- or ischemia-associated regional 123I-MIBG depletion confirms that silent ischemia is not caused by a global sympathetic nervous dysfunction in a sense of cardiac polyneuropathy.

Sympathetic re-innervation after heart transplantation: dual-isotope neurotransmitter scintigraphy, norepinephrine content and histological examination.

Cardiac transplantation entails surgical disruption of the sympathetic nerve fibres from their somata, resulting in sympathetic denervation. In order to investigate the occurrence of sympathetic re-innervation, neurotransmitter scintigraphy using the norepinephrine analogue iodine-123 metaiodobenzylguanidine (MIBG) was performed in 15 patients 2-69 months after transplantation. In addition, norepinephrine content and immunohistochemical reactions of antibodies to Schwann cell-associated S100 protein, to neuron-specific enolase (NSE) and to norepinephrine were examined in 34 endomyocardial biopsies of 29 patients 1-88 months after transplantation. Anterobasal 123I-MIBG uptake indicating partial sympathetic re-innervation could be shown in 40% of the scintigraphically investigated patients 37-69 months after transplantation. In immunohistochemical studies 83% of the patients investigated 1-72 months after transplantation showed nerve fibres in their biopsies but not positive reaction to norepinephrine. Significant norepinephrine content indicating re-innervation could not be detected in any biopsy. It was concluded that in spite of the lack of norepinephrine content there seemed to be immunohistological and scintigraphic evidence of sympathetic re-innervation. An explanation for this contradictory finding may be the reduced or missing norepinephrine storage ability compared to the restored uptake ability of regenerated sympathetic nerve fibres.

Validation of a dual-isotope technique using 123I-MIBG and 201Tl in the assessment of sympathetic reinnervation following heart transplantation: phantom and patient studies.

Dual-isotope studies using 123I and 201Tl allow the assessment of sympathetic reinnervation in patients following heart transplantation. 123I-meta-iodobenzyl guanidine (123I-MIBG) serves as a tracer of the integrity of the sympathetic nervous system. 201Tl is used for landmarking to allow better delineation of the myocardium due to faint 123I-MIBG accumulation in heart transplants. The aim of this study was to evaluate the influence of parameters such as crossover, attenuation and the 123I/201TI activity ratio on the assessment of the myocardial 123I-MIBG uptake ratio using phantom and patient studies. Crossover was calculated using the ratio: 201TI counts in the 123I energy window/201Tl counts in the 201Tl window. Phantom studies were performed using a rectangular phantom (RP) and a cardiac phantom (CP). The mean crossover from the 201Tl to the 123I energy window was 11.48% (RP) and 11.13% (CP). Depending on attenuation in water (depth of water 0-5 cm), crossover increased from 10.92 to 15.85% (RP) and from 11.05 to 15.79% (CP). In order to confirm that the experimental crossover results were equivalent to those obtained in patient studies, 15 patients underwent myocardial scintigraphy. After injection of 201Tl, a simultaneous dual-window acquisition was performed to assess crossover from the 201Tl to the 123I window. The mean crossover was 15.35%, as high as the crossover assessed in phantom studies, taking into account attenuation and scatter caused by the chest wall. In order to reduce 201Tl crossover, the 123I activities were six times as high as the 201Tl activities.(ABSTRACT TRUNCATED AT 250 WORDS)

Myocardial perfusion with [11C]methyl triphenyl phosphonium: measurements of the extraction fraction and myocardial uptake.

The present study describes extraction fraction and uptake measurements of the [11C]methyl triphenyl phosphonium (11C-MTP), a promising positron emission tomography (PET) agent for cardiac imaging. PET imaging was performed in mongrel dogs. Under physiological flow conditions 11C-MTP uptake reached a maximum within the first 10 minutes after injection and remained constant during the entire observation period of 80 minutes. Over the same time period, the heart/blood ratio was 46-106:1, and the heart/lung ratio 14:1. Following permanent occlusion of the left anterior descending coronary artery, 11C-MTP uptake in the normally perfused myocardium also reached a maximum at 10 minutes after injection, whereas in the infarcted area there was no significant accumulation of 11C-MTP. For a time period of 80 minutes the noninfarcted/infarcted myocardium ratio was 12:1. Extraction was measured in anesthetized dogs with a double isotope method using 99mTc-HSA as the reference tracer. The extraction fraction was 91% at a flow of 69 mL/min/100g. As flow increased to five-fold (342 mL/min/100g) following administration of adenosine, extraction fell to 61%. Following coronary artery occlusion, the 11C-MTP content in the myocardium was highly correlated (r = 0.93, p < 0.01; y = 10.46 + 0.92x) with the microsphere determined regional myocardial blood flow.

[Sympathetic reinnervation following heart transplantation--a double-tracer study with 123I-MIBG and 201Tl].

Sympathetic reinnervation was evaluated in 15 patients 2-69 months after heart transplantation using a double-tracer technique with 123I-MIBG and 201Tl. Since MIBG is accumulated in the same manner as norepinephrine it may serve as a tracer of the integrity and function of the sympathetic nervous system. 201Tl was used for landmarking. Planar anterior imaging was performed 15 min and 4 h after i.v. injection of 220 MBq 123I-MIBG and 37 MBq 201Tl. Image quantitation was based on the ratio of myocardial to mediastinal MIBG-uptake. Cardiac regions of interest were defined according to the 201Tl uptake. There was no evidence of sympathetic reinnervation in 8 patients 2-34 months after transplantation. Increased MIBG-uptake could be observed in the anterior basal region in 6 long-term cardiac transplants (37-69 months). One patient with a 59-month-old transplanted heart did not reinnervate. Increased MIBG-uptake in the anterior basal region indicating partial sympathetic reinnervation could be shown in 40% of the investigated patients with an average organ age of 51 months.
