Effect of sanguinaria extract on development of plaque and gingivitis when supragingivally delivered as a manual rinse or under pressure in an oral irrigator.

The effects on developing plaque and gingivitis following rinsing with a placebo oral rinse or an oral rinse containing 300 micrograms/ml sanguinaria extract (sanguinaria) were compared to effects produced by supragingival irrigation with dilute solutions of the rinse corresponding to 22.5 micrograms/ml sanguinaria and 90 micrograms/ml sanguinaria. The study design was a repeated measures, single-blind crossover with no oral hygiene over 2 weeks duration. After 7 and 14 [corrected] days, significantly lower plaque and gingivitis scores were obtained with use of the sanguinaria-containing rinse and irrigating solutions compared with the placebo rinse. There were no significant differences in plaque and gingivitis scores between the groups using the sanguinaria rinse and the sanguinaria irrigating solutions. A comparison [corrected] of % distribution of 0, 1 and 2+ scores also indicated that rinsing and supragingival irrigation with sanguinaria was more effective in plaque and gingivitis control than rinsing with the placebo. The results suggest that sanguinaria oral rinse may be effective in controlling plaque and gingivitis when delivered by manual rinsing or supragingival irrigation.

Effect of sanguinaria extract on established plaque and gingivitis when supragingivally delivered as a manual rinse or under pressure in an oral irrigator.

Sanguinaria extract (sanguinaria) solutions were evaluated in 44 subjects in a 2-week, no oral hygiene study as a 300 micrograms/ml manual rinse, and the results compared to supragingival irrigation with 22.5 micrograms/ml sanguinaria concentration and supragingival irrigation with water. Both the manual use of sanguinaria and supragingival irrigation of dilute sanguinaria produced significantly less plaque growth than supragingival irrigation with deionized water. In terms of % changes from baseline, manual rinsing and supragingival irrigation with sanguinaria limited plaque growth to 17.7% and 24.2%, respectively, while irrigation with water had a 51.5% growth. For gingivitis, control supragingival irrigation with sanguinarine and with water were statistically different from manual rinsing with sanguinaria. Compared to baseline, the groups irrigating with sanguinaria and with water had gingivitis reductions of 68.7% and 73.3%, respectively, while manual rinsing with sanguinaria had a 29.6% reduction. The results suggest that dilute solutions of sanguinaria delivered via rinsing or supragingival irrigation are effective in controlling plaque as an additional benefit to the use of supragingival irrigation to control gingivitis. Supragingival irrigation with sanguinaria as part of a home care routine for patients with plaque and gingivitis is suggested.

Jejunal water and electrolyte absorption from two proprietary enteral feeds in man: importance of sodium content.

Jejunostomy losses of Na+ and water during enteral nutrition after massive intestinal resection may be severe. We have attempted to analyse this practical problem by using an in vivo perfusion technique in healthy volunteers to study Na+, water and nutrient absorption from a short (25 cm) segment of jejunum during perfusion of an isotonic solution of the elemental diet Vivonex. Further solutions made from the amino acid and carbohydrate components of Vivonex were also perfused in part I of the study in order to determine the causes of the marked Na+ and water secretion seen during Vivonex perfusion. Low initial Na+ concentration was found to be the major determinant of net Na+ secretion, initial Na+ concentration correlating significantly with Na+ absorption (r = 0.95, n = 7 p less than 0.001). Water absorption correlated with net absorption of NaCl (r = 0.82, n = 7 p less than 0.01). There was, however, a better correlation with total absorption of NaCl plus amino acids (r = 0.99, n = 7, p less than 0.01). In part II of the study separate isotonic solutions of NaCl, glucose, and the polymeric diet, Ensure were also studied. Net sodium secretion occurred during glucose and Ensure perfusion, as predicted from their low Na+ concentration. Owing to rapid sucrose absorption from Ensure there was substantial luminal disappearance of osmotically active particles and hence marked water absorption, which was accurately predicted using the regression equation for water absorption derived in part I, substituting sucrose absorption for amino acid absorption. We conclude that the marked Na+ and water secretion observed during Vivonex perfusion is not a unique property of this amino acid based diet but is due to its low Na+ content.

Comparison of perceived spatial frequency between the fovea and the periphery.

A grating viewed in the periphery usually appears to be of higher frequency than the same grating viewed at the fovea, for frequencies below the Nyquist frequency of the periphery. Systematic shifts in perceived frequency between fovea and periphery were found under several experimental conditions: spatially localized or extended sine-wave patterns, test stimuli presented in the superior or the inferior visual field, and stimuli of high or low mean luminance at several different contrasts. A multiple-spatial-frequency channels model can qualitatively account for our results, if it is assumed that each channel has a receptive-field size that increases as a function of eccentricity but has a perceived frequency label that does not change as a function of eccentricity.

Blue sclerae: a common sign of iron deficiency?

169 hospital inpatients were studied to assess the association of blue sclerae with iron-deficiency anaemia. Three observers independently graded the signs of blue sclerae and mucosal pallor as absent, equivocal, definite, or striking. Blue sclerae were seen more often in patients with iron-deficiency anaemia (40/46, 87%) than in those with other anaemias (2/28, 7%; p less than 0.001) or without anaemia (5/95, 5.3%; p less than 0.001). The specificity of blue sclerae in iron-deficiency anaemia was 0.94 with a sensitivity of 0.87. By comparison, mucosal pallor was noted in only 30% of patients with iron-deficiency anaemia, with a specificity of 0.96 and a sensitivity of only 0.20 (p less than 0.001). The presence of blue sclerae was unaffected by age, sex, or colour of iris. Blue sclerae appear to be a good indicator of iron deficiency and should become a regular part of clinical examination.

Microinjections of methyl-beta-carboline-3-carboxylate into the dorsal raphe nucleus: behavioural consequences.

Very small quantities (0.01-10 ng) of the inverse benzodiazepine receptor agonist, methyl-beta-carboline-3-carboxylate (beta-CCM) microinjected into the dorsal raphe nucleus (DRN) of the rat selectively reduced social interaction, an effect consistent with an increase in anxiety. Similarly, intraperitoneally injected beta-CCM, within a limited dose range, reduced social interaction without affecting locomotor activity. The benzodiazepine receptor antagonist, RO15-1788 (1 ng) microinjected into the DRN, reversed the suppression of social interaction induced by either intra-raphe or intraperitoneal beta-CCM. Histological examination of the beta-CCM microinjection sites showed that locations within the DRN were almost invariably associated with decreases in social interaction; microinjections failing to decrease social interaction were located primarily outside the DRN. We conclude that the DRN has a major role in expressing the anxiogenic effect of beta-CCM and it may therefore be an important area in the neuronal system controlling anxiety.

Coexistent ulcerative colitis and Crohn's disease.

This case report describes a patient with chronic ulcerative colitis and epithelial dysplasia of 17 y duration ultimately complicated by colonic carcinoma. At laparotomy, clinically unsuspected but typical Crohn's disease of the terminal ileum was also found. The macroscopic and histological features of the resected terminal ileum were characteristic of Crohn's disease.

Political orientation and occupational values of college youth, 1969 and 1981: a shift toward uniformity.

McFalls and Gallagher (1979) have found a strong relationship between the occupational values and political orientations of college students. Their study was based on the results of a sample survey conducted in 1969. However, the political climate on college campuses has changed dramatically, and so has the nature of the job market. A new survey was conducted in 1981 which was identical to the 1969 survey. Its objective was to determine if the same political group differentials in occupational values which existed in the politically tumultuous late sixties and early seventies still hold in the more placid 1980s. The findings are reported here.

The "winter born phenomenon" among schizophrenics: differences between blacks and whites.

Reviewed the literature on birth seasonality among schizophrenics and reported data on highly significant differences between the quarterly birth patterns of black and white schizophrenics (N = 724). The findings strongly indicate that specific subgroups of schizophrenics are especially likely to be winter-borns. Log-linear analyses uncovered a highly significant difference between the birth seasons of black and white patients. Black schizophrenics are especially likely to be winter-borns, while white schizophrenics had a birth peak in the fall. We speculate about the causes of this race-specific finding from a perspective that is sociological and neurophysiological.

The benzodiazepine receptor ligand CL218,872 has both anxiolytic and sedative properties in rodents.

The triazolopyridazine, CL218,872, inhibited the binding of [3H]flunitrazepam in the cerebellum more potently than in cortex, both in vitro and in vivo. The relationship between this phenomenon and the selectivity of benzodiazepine receptor subtypes is considered. In a range of behavioural tests the overall profile of CL218,872 was similar to that of diazepam in the rat and mouse, although CL218,872 was half as potent as diazepam in the rat and some 20-fold weaker in the mouse. In particular contrast to published observations, CL218,872 reduced locomotor activity in rats and mice at doses only slightly larger than those required to inhibit conflict in rats and footshock-induced fighting in mice. The only qualitative difference between the compounds that could be detected was in the mouse where large doses of CL218,872 did not produce the marked degree of motor incoordination seen after diazepam. However, no such difference was observed in the rat. The presently held view that CL 218,872 is a non-sedative anxiolytic agent needs to be revised in the light of these observations.

Effect of catheter tunnelling and a nutrition nurse on catheter sepsis during parenteral nutrition. A controlled trial.

In a three-year controlled trial of subcutaneous catheter tunnelling as a method of reducing total parenteral nutrition (TPN) catheter sepsis 99 silicone catheters (52 tunnelled, 47 untunnelled) were inserted into the subclavian (94%) or jugular (6%) veins under aseptic conditions. The influence of a nutrition nurse, who joined the nutrition team after 18 months, on catheter sepsis rate was also documented. Catheter sepsis was confirmed in 13 of 47 (28%) untunnelled catheters and only 6 of 52 (11.5%) tunnelled catheters (p less than 0.05). A nutrition nurse reduced sepsis rate from 33% (tunnelled 6, untunnelled 11) to 4% (0 tunnelled; 2 untunnelled) (p less than 0.001). There was no significant difference between tunnelled and untunnelled catheters in sepsis rates after the arrival of the nutrition nurse. Although 85% patients had concurrent internal sepsis, the pathogens implicated in catheter sepsis came from superficial sites in 16 of 19 cases (p less than 0.01). Rigorous aseptic nursing care is thus the most significant factor in the reduction of TPN catheter sepsis, but tunnelling can reduce sepsis rate when nursing care is suboptimum.

The roles of lactose and Clostridium difficile in the pathogenesis of enteral feeding associated diarrhoea.

This study investigated the influence of the disaccharide lactose on the incidence of clinically significant enteral feeding associated diarrhoea. In this double blind study both groups each of 25 patients were randomised to receive either a lactose containing diet Clinifeed 400 in 25 patients or a lactose free diet Ensure in 25 patients. Diarrhoea occurred with equal frequency in both treatment groups, even in those patients with symptomatic and biochemical evidence of impaired lactose handling. Although the onset of diarrhoea was significantly associated with antibiotic administration (p<0.01), Cl. difficile was not isolated from the stools of any patient with diarrhoea.
