Management of invasive frontoethmoidal sinus mucoceles.

Invasive frontoethmoidal sinus mucoceles extending into the anterior cranial fossa or orbits are difficult to manage and can lead to lethal complications. In the past four years, nine cases of frontoethmoidal mucoceles were treated at the University of Texas Medical Branch, Galveston. Five cases were complicated by anterior cranial fossa invasion, orbital invasion, or both. The choice of surgical procedures used to manage these cases depended on mucocele extent and location, which were best determined by computed tomography. Two cases required craniotomy, one of which required an inferior-based pericranial flap for reconstruction of the floor of the anterior cranial fossa. Two cases were managed by osteoplastic flap and fat obliteration procedures. Intranasal drainage was the procedure used in one case that had extensive orbital involvement. Surgical complications included an intracranial abscess in one patient and a cerebrospinal fluid leak in a second patient.

Bacteriologic evaluation of the Servo 150 hygroscopic condenser-humidifier.

The Servo 150 hygroscopic condenser-humidifier was evaluated during use to determine if the inner foam core became contaminated and if a bacteria-laden aerosol was produced during the inspiratory cycle of the patient's mechanical ventilator. Cultures from the core and of inspired gas were obtained from seven patients with known culture-positive sputum, after 4, 8, 12 and 24 h of humidifier use. In each case, the inner foam core was grossly contaminated after 4 h of use and colony counts increased during the 24-h testing period. The bacteria recovered were the same as those cultured from sputum. Despite the core's heavy bacterial growth, a bacteria-laden aerosol occurred in only 43% of the samples obtained during humidifier use. The Servo humidifier does not appear to increase the risk of airway exposure to airborne bacteria during mechanical ventilation.

Epithelial-myoepithelial carcinoma of the parotid gland.

The EME carcinoma of intercalated duct origin is now recognized as a distinct salivary gland malignancy. A case has been reported that typifies clinical and pathologic features. Both the surgeon and pathologist should learn not only to recognize its dual cellular characteristics, but also to be aware of the potential destructive nature and possibility for metastases with this tumor.

Tumor responsiveness of squamous cell carcinoma to cisplatin.

The addition of chemotherapy to planned multiple modality treatment of patients with advanced head and neck squamous cell carcinomas may improve survival rates, but individual tumor response is unpredictable. An assay for determining tumor responsiveness to specific chemotherapeutic agents would facilitate treatment selection. This study used the subrenal capsule (SRC) assay to test responsiveness of squamous cell carcinoma to cisplatin. Tumor xenografts obtained from ten patients were implanted beneath the renal capsules of control and treatment mice. Tumor responsiveness was determined by using the paired Student's t-test to compare base-line and post-treatment measurements in the cisplatin-treated mice. Results indicated six of ten tumors were sensitive to cisplatin. In conclusion, tumor sensitivity to cisplatin and other drugs may be predictable with the SRC assay, but to be clinically useful, studies will be necessary to correlate assay results with responses observed in patients.

Nasal dermoid with intracranial involvement.

Nasal dermoids are unusual lesions resulting from embryopathology. They are diagnosed easily by physical examination. Treatment is complete surgical excision, aided by microsurgical techniques. Involvement of the skullbase is common with "deep-seated" ND, and intracranial involvement is not uncommon. The high-resolution CT scanner is believed to be valuable in diagnosing deep involvement, including intracranial extension. The finding of a bifid crista galli is suggestive of intracranial involvement. Neurosurgical consultation is mandatory for all cases of ND when deep extension is suspected.

Carcinoma of the nasal septum.

Twenty-two patients with nasal septal carcinoma have been treated at the University of Texas Medical Branch, Galveston, during the 18-year period from 1961 to 1979. Squamous cell carcinoma of the nasal septum was found in 18 patients (82%), with single instances of reticulum cell sarcoma, basal cell carcinoma, histiocytic lymphoma, and transitional cell carcinoma. One of the squamous cell carcinoma group had palpable cervical metastasis at diagnosis, with metastatic neck disease developing in eight patients from three to 25 months after treatment. Squamous cell carcinoma of the nasal septum is aggressive and often undertreated. Our experience suggests that patients with septal carcinoma of any size should be treated by wide surgical excision (via lateral rhinotomy) and irradiation to the primary site and neck. Control of the primary tumor was achieved in 17 of 18 patients, with neck control in ten patients. The five-year absolute survival rate for squamous cell carcinoma of the nasal septum was 66%.

Calcium sulfate as a mandibular implant.

Mandibular reconstruction has posed significant problems, particularly when dealing with loss of mandibular substance. Various implant substances have been utilized in attempts to repair mandibular defects. These have included metals, synthetic material, and organic substances. All have posed significant problems, particularly with rejection and inability of the implant to develop a stable fusion with the surrounding bone. Recently, calcium sulfate has been successfully used as an implant in frontal sinuses, mastoid cavities, and, in one report, as a mandibular implant. The present study looks at the ability of calcium sulfate to induce osteoneogenesis in canine mandibles with and without the presence of a periosteal covering around the implant. Infection somewhat limited the success of the study, but in those animals without major infections, successful replacement of the calcium sulfate by normal bone occurred both with and without the presence of periosteum.

National organization for interinstitutional cooperative studies in otolaryngology.

Cooperative clinical investigation, the sharing of ideas and the pooling of resources among several institutions, is a valid, useful, and powerful research mechanism for almost any type of prospective clinical study. The strengths, weakness, and principles of cooperative studies are reviewed in detail. I propose that a national office for research activities in otolaryngology be established in the headquarters of the American Academy of Otolaryngology-Head and Neck Surgery, Inc., in Washington, DC, at the earliest possible date.
