Simultaneous bilateral testicular germ cell tumors with dissimilar histology. Case report and review of the literature.

A 32-year-old male presented with simultaneous bilateral germ cell tumors of the testicles. Histological examination revealed dissimilar histology in both testes showing pure seminoma in the left side and mature teratoma with malignant transformation in the right testicle. A survey of the literature revealed a total of 151 previously described cases of synchronous bilateral germ cell tumors, the majority of which presenting as bilateral seminoma. Treatment of synchronous bilateral germ cell tumors with dissimilar histology should consist of bilateral orchiectomy and bilateral retroperitoneal lymph node dissection.

MR imaging of the gastrointestinal tract: value of Gd-DTPA.

To determine the value of gadolinium-DTPA (Gd-DTPA) as an MR contrast material for the gastrointestinal tract, we obtained T1-, T2-, and proton-density-weighted MR scans of the abdomen in 20 volunteers before and after oral administration of the compound (10 ml/kg of a 0.5- or 1.0-mM solution). Gd-DTPA was given either alone or with mannitol (15 or 30 g/l). Opacification of the small bowel after the administration of 1.0 mM Gd-DTPA was superior to that achieved after administration of the 0.5-mM solution. In the absence of Gd-DTPA, the pancreas could be delineated partly in 11 cases. After administration of Gd-DTPA, the pancreas could be well differentiated from stomach and duodenum in 16 of 20 cases. In the absence of mannitol, two of the five subjects had homogeneous opacification of the entire small bowel. When Gd-DTPA was given with either 15 or 30 g mannitol/l, homogeneous opacification of the entire small bowel was obtained in nine of 10 volunteers. Diarrhea not discomforting to the volunteers was noted in four of 20 cases. No abnormal blood tests were recorded after administration of Gd-DTPA. The results suggest that oral administration of Gd-DTPA (1.0 mM) given with mannitol (15 g/l) is effective as an MR contrast agent for the gastrointestinal tract. The formulation was found to be safe in a small series of 20 volunteers.

Adrenal myelolipoma: clinical, radiologic, and histologic features.

The adrenal myelolipoma is a benign, endocrinologically inactive tumor whose histologic structure consists of mature adipose tissue with foci of hematopoietic cells. A case is presented of a seventy-one-year-old woman in whom the diagnosis was established preoperatively by means of sonography, computerized tomography, and magnetic resonance tomography. In a review of the literature, the radiologic profile is discussed, and based on the analysis of 59 surgically treated cases a therapy recommendation is given.

Adrenal Gland Neoplasms↗

[Magnetic resonance tomography of ovarian tumors].

The results of 64 MR examinations in 52 patients with benign, malignant and inflammatory lesions of the adnexa are presented. In each case, surgery or laparoscopy was carried out, so that it was possible to compare the findings on MRT with the histological results. Multiplanar images of gynaecological lesions provide excellent demonstration of the pathological anatomy. High soft tissue contrast makes it possible to differentiate tissues, particularly in the case of serous, mucinous and haemorrhagic fluid collections, as well as solid and fat-containing tissue. High sensitivity may be expected in the demonstration of dermoid and endometrial cysts. The distinction between benign and malignant lesions must, however, be based on morphological criteria. The combination of T1 and T2 sequences is necessary for adequate MR examinations of gynaecological lesions.

Adnexa Uteri↗

[Imaging diagnosis of ovarian tumors. Magnetic resonance tomography in comparison with computerized tomography and sonography].

The present paper compares the results of MRT with sonography in 64 patients with tumours of the adnexa in 35 patients examined by CT. There was no difference between these three imaging methods as regards lateralisation of the lesion. MRT provided better differentiation because of the excellent demonstration of the uterus and of tumours of the adnexa. Detailed tissue characterisation, particularly as regards cystic lesions, provides improved diagnostic information. MRT has problems, however, because of its low spatial resolution and the difficulty in differentiation from bowel loops. At present sonography and CT is better at establishing a differential diagnosis. CT remains the method of choice for tumour staging.

[Oral contrast medium for magnetic resonance tomography of the abdomen. II. Phase I clinical testing of gadolinium-DTPA].

20 male volunteers were examined by MRI to determine safety and efficacy of Gd-DTPA formulations for gastrointestinal (GI)-tract contrast enhancement. Prior to and after administration of contrast spin-echo images were obtained. Volunteers were studied after administration of 10 ml contrast medium/kg (1.0 mmol Gd-DTPA/l vs. 0.5 mmol Gd-DTPA/l, no mannitol vs. 15 g mannitol/l vs. 30 g mannitol/l). Gd-DTPA provided high signal intensity in the GI tract. Contrast medium in gastric lumen and duodenum permitted improved delineation of pancreatic head and ventral outline in 16 of 20 subjects. No clinically relevant side effects were seen.

Conventional and rapid MR imaging of the liver with Gd-DTPA.

Twenty-three patients with malignant hepatic tumors underwent magnetic resonance (MR) imaging before and after intravenous administration of gadolinium-diethylene-triaminepentaacetic acid (DTPA). Two different doses were used, 0.1 mmol/kg and 0.2 mmol/kg. The larger dose proved to be more effective than the smaller dose. The signal-enhancement-to-noise ratio was significantly larger in the tumor than in the liver (2 alpha less than or equal to .05). In a moderately T1-weighted spin echo (SE) sequence (SE 400/30) (repetition time [TR] msec/echo time [TE] msec), the tumor was better defined 6 minutes after administration of Gd-DTPA. More strongly T1-weighted sequences--that is, SE 200/20 and inversion recovery 1,500/35/400 (TR msec/TE msec/inversion time, msec)--showed significantly worse contrast between tumor and liver (signal-difference-to-noise ratio [SD/N]) 10 and 15 minutes after administration (2 alpha less than or equal to .05). On the other hand, the low SD/N in the rapid MR imaging sequence was significantly improved (2 alpha less than or equal to .05). The most important indications for administration of Gd-DTPA in diagnosing hepatic tumors are the presentation of perfusion conditions and contrast optimization in rapid MR images.

Percutaneous transluminal treatment of varicoceles: where to occlude the internal spermatic vein.

Percutaneous transluminal sclerotherapy is an accepted treatment for varicoceles. It is administered by applying a liquid sclerosing agent to the internal spermatic vein. However, the optimal site for the placement of the occlusive material has not been evaluated previously. Therefore, the success rate of sclerotherapy was determined in two groups of patients. In the first group (34 patients), the liquid agent was applied in the cranial part of the internal spermatic vein; in the second group (113 patients), the agent was applied in the caudal part of the vessel at the level of the inguinal ring. The success rate determined 3 months after therapy by telethermography was significantly higher after treatment in the caudal part (82%) than in the cranial part (68%) of the vessel. Sclerotherapy at the level of the inguinal ring appears to be superior to therapy in the cranial part of the internal spermatic vein.

Ultrasound imaging in Peyronie disease.

Fifty-three patients with Peyronie disease were examined sonographically. The use of a water path facilitates good localization and measurement of the fibrotic plaques in the area of the tunica albuginea. Calcifications can be clearly detected. Penile sonography is painless, free of risk, and easy to perform and yields precise morphologic pictures. It is ideal for documentation and assessment of the anatomic alterations in Peyronie disease as well as for control of its course.

[Magnetic resonance tomography of focal liver lesions in comparison with computed tomography and sonography].

The results of magnetic resonance tomography (MRT) performed in 37 patients with predominantly solitary intrahepatic space-occupying masses are presented. Examinations were carried out with a superconducting magnet operating at a field strength of 0.35 Tesla. The majority of intrahepatic masses exhibited a well distinguishable signal amplification compared to normal hepatic parenchyma when T2 weighted sequences were used. For a further differentiation of the lesion, however, T1 and T2 weighted sequences have to be performed. In all cases diagnostic value of MRT is compared to that of sonography and computed tomography. The assessment of magnetic resonance tomograms of focal hepatic lesions is based on the sequence-depending on the signal behaviour of the mass - especially with regard to morphological criteria. The high soft-tissue contrast of magnetic resonance tomography compensates for the low spatial resolution, thus resulting in a sensitivity of focal lesions comparable to that of sonography and computed tomography. The good visualisation of vessels is an advantage. With the exception of haemangioma, however, MRT does not seem to improve diagnosis of the type of focal hepatic lesions.

[Noninvasive imaging procedures in the diagnosis and therapy control of varicoceles. I: Sonography and thermography in the diagnosis of varicoceles].

69 patients with clinically suspected varicoceles were examined thermographically and sonographically prior to testicular phlebography. The combination of sonography and thermography permitted precise differentiation into normal findings (7%), left-sided varicoceles (86%) and bilateral varicoceles (7%). Thermography was advantageous with an accuracy of 97.3%. Sonography had an accuracy of 90.5%. No clear correlation was found between varicocele size and degree of hyperthermia. The combination of sonography and thermography affords a high degree of accuracy in the diagnosis of varicoceles, including subclinical and infantile varicoceles.

[Non-invasive imaging procedures in the diagnosis and control of therapy of varicoceles. 2. Importance of thermography and sonography for therapy control following sclerotherapy of varicoceles].

In 41 patients with varicoceles, a combined thermographic and sonographic examination of the scrotum was carried out pretherapeutically as well as 14 days and 3 months after percutaneous sclerotherapy. In the differentiation of a successful sclerosing or a varicocele persistence, thermography shows a higher sensitivity than sonography with 34 posttherapeutic normal findings and 7 persisting hyperthermias. The sonographic sign of a successful varicosclerosation is the absence of venous dilation of the pampiniform plexus in an upright position with Valsalva's manoeuvre in 36 patients. After proximal sclerosing of the testicular vein sonography reveals a reduction in vascular size; thrombosed veins of the pampiniform plexus are demonstrated only after distal sclerosing. There are no sonographically detectable disorders of the testes after varicosclerosation. Thermographic and sonographic control after percutaneous sclerotherapy or surgical ligation of varicoceles is indicated in children and in doubtful clinical or spermatological cases.
