Studies on the bile salt stimulated lipolytic activity of human milk using whole milk as source of both substrate and enzyme. I. Nutritional implications.

The lipolytic activity of human milk was assayed using whole fresh milk as source of both enzyme and substrate. Bile salts and specifically glycine rather than taurine conjugates were essential for activity; a concentration of 2 mM being optimal. The lipid concentration and lipolytic activity increased significantly (P less than 0.01 and less than 0.02 respectively) during the course of a feed. The lipid concentration and lipolytic activity were compared in immature and mature milks (less and greater than 15 days postpartum respectively). The lipolytic activity remained constant whereas the lipid concentration was significantly greater in the mature than the immature milks (P less than 0.001). In none of the milks was the substrate concentration limiting. The % hydrolysis of ingested triglyceride in the immature and mature milks was calculated to be approximately 40 and 20% respectively in 2 h, which is the estimated transit time from the duodenum to ileum. The major lipolytic products were free fatty acids and in general the enzyme showed little specificity for different fatty acids of triglyceride.

Opioid inhibition of dopamine release from nervous tissue of Mytilus edulis and Octopus bimaculatus.

Morphine and D-Ala2-Met-enkephalin as well as other opioids suppress potassium-stimulated release of 3H-labeled dopamine from neurons tissue of two marine invertebrates, Mytilus edulis and Octopus bimaculatus. Naloxone reverses the inhibitory effects in both species. Potassium-stimulated release of 3H-labeled serotonin is not altered by opioids. It is postulated that opiate receptors and their endogenous effectors play a prominent role in regulation of transmitter release in invertebrates.

Coliform status of domestic swimming pools.

An investigation of maintenance, usage, and coliform status of water in 104 domestic swimming pools in 13 Brisbane suburbs was made in January, 1978. The results showed that domestic pools are often imperfectly serviced, and are potential infection sources. The need for health education of pool owners, and for more reliable methods of pool sanitization is emphasized.

Uniformity of human milk.

Compositon of human milk was studied during the feed in six women, through-out the day in one woman on 3 consecutive days and in three women on 39 occasions during established lactation. The total lipid content varied in the course of suckling as well as diurnally in a regular and predictable manner, but the fatty acid composition of the lipid remained constant and was similar in the subjects examined. Fatty acid pattern of milk lipid of one woman supplementing her diet with unsaturated fats was studied. The changes in the milk lipid compostion, induced by the altered diet, are discussed in relation to the mechanism of milk fat synthesis. The described changes in milk composition are specific and may be physiologically significant. The composition of lipid in milk of mothers on adequate and balanced diet shows only limited fluctuations forming a species-specific pattern. The variations that occur in extreme dietary conditions (malnutrition, undernutrition, supplementation) are not random and can be predicted.

Renal glutamine metabolism in rats fed high-protein diets.

The influence of protein intake on acid excretion and renal glutamine metabolism was investigated and compared to the effects of NH4Cl-induced metabolic acidosis. Rats fed a diet containing 55% casein excreted more ammonia, phosphate, sulphate, and chloride than did rats fed a 13% casein diet, but, when they were given an 0.1 M NaHCO3 solution to drink, ammonia excretion was no longer elevated. Renal phosphate-dependent glutaminase and phosphoenolpyruvate carboxykinase activities, ammoniagenesis by isolated mitochondria, and the rate of renal gluconeogenesis were all elevated in the rats fed the high-protein diet but not if these rats also drank the sodium bicarbonate solution. Increased glutaminase and phosphoenolpyruvate carboxykinase activities, mitochondrial ammoniagenesis, and gluconeogenesis were all evident in rats made acidotic with NH4Cl. It is concluded that these metabolic adaptations evident in the kidneys of rats fed the high-protein diet are due to the acidogenic effects of increased protein intake.

The transport and metabolism of glutamine by kidney-cortex mitochondria from normal and acidotic rats.

1. The oxidation of glutamine by kidney-cortex mitochondria from normal and acidotic rats was not inhibited by avenaciolide, which did inhibit glutamate uptake and oxidation. The oxidation of glutamine by these mitochondria was always greater than that of glutamate. Direct measurements of the metabolism of [1-14C]glutamine in the presence of glutamate, and of [1-14C]glutamate in the presence of glutamine, demonstrated that the uptake and metabolism of external glutamate is insufficient to account for the observed rate of glutamine uptake and metabolism. Thus the postulated glutamine/glutamate antiport does not play a quantitatively important role in the metabolism of glutamine by renal mitochondria. 2. Rapid swelling of these mitochondria was observed in iso-osmotic solutions of L-glutamine and L-glutamyl-gamma-monohydroxamate but not in D-glutamine or L-isoglutamine (1-amido-2-aminoglutaric acid). Thus a relatively specific glutamine uniport exists in these mitochondria. 3. The utilization of glutamine was increased about 3-fold in mitochondria from chronically acidotic rats. Thus mitochondrial adaptations play an important part in the renal response to metabolic acidosis.

Use of transcutaneous electrical stimulation in the control of postoperative pain.

Perception and relief of pain were studied in a varied group of fifty patients who had undergone laparotomy with upper abdominal midline incisions. Twenty-six patients were supplied with functioning transcutaneous electrical stimulators. Twenty-four received nonfunctioning machines. The controls were randomized by a nurse not otherwise involved in this study. Only seventeen of the total study group were judged failures and six of the patients had been receiving preoperative narcotics. Patient response was very favorable and pain relief was good to excellent, especially in one group with working stimulators (p is less than 0.008). Patients with known malignant disease did not respond as well as those with benign disease although anxiety may have been an important factor here. Use of the transcutaneous electrical stimulator would seem particularly worthwhile for patients undergoing elective abdominal operations for benign disease with no history of narcotic usage. Briefing the patient preoperatively on the potential value of transcutaneous electrical stimulation as a method of pain relief may improve end-results.

7q deletion syndrome (7q32 leads to 7qter).

Four independently ascertained children who presented with unusual facies and delayed mental and physical development were found to have a similar deletion of part of the long arm of chromosome 7 (46,XX or XY, del(7)(q32); 46,XX or XY,del(7)(pter leads to q32:)). Comparison of the findings of these four cases with one other case report of a similar deletion revealed similar dysmorphologic features in all five cases.
