Takayasu arteritis: angiographic findings and results of angioplasty.

Takayasu arteritis is a systemic disease characterized by occlusion of the aorta and its branches. We performed coronary angiography and thoracic and abdominal aortography on 47 patients with Takayasu arteritis. Angiographic findings included arterial stenosis, occlusion, or aneurysm. The left subclavian artery was involved in 26 cases (55%), the abdominal aorta in 25 cases (53%), the right renal artery in 21 cases (45%), the right subclavian and left renal arteries in 18 cases (38%), the descending thoracic aorta in 15 cases (32%), and the left common carotid artery in 14 cases (30%). The coronary arteriograms in the 47 patients showed coronary involvement in seven (15%). Proximal or osteal lesions were present in six cases. Percutaneous transluminal angioplasty was performed successfully in eight patients for treatment of three aortic lesions, nine renal artery lesions, one subclavian artery lesion, and one coronary artery lesion. In the three cases with recurrence after 4-7 months, repeated angioplasty was successful. For appropriate management of Takayasu arteritis, we suggest thorough angiographic evaluation and proper intervention, including percutaneous transluminal angioplasty in selected cases for revascularization.

Protein carbon-13 spin systems by a single two-dimensional nuclear magnetic resonance experiment.

By applying a two-dimensional double-quantum carbon-13 nuclear magnetic resonance experiment to a protein uniformly enriched to 26 percent carbon-13, networks of directly bonded carbon atoms were identified by virtue of their one-bond spin-spin couplings and were classified by amino acid type according to their particular single- and double-quantum chemical shift patterns. Spin systems of 75 of the 98 amino acid residues in a protein, oxidized Anabaena 7120 ferredoxin (molecular weight 11,000), were identified by this approach, which represents a key step in an improved methodology for assigning protein nuclear magnetic resonance spectra. Missing spin systems corresponded primarily to residues located adjacent to the paramagnetic iron-sulfur cluster.

Prenatal diagnosis of a heterozygote of salt wasting congenital adrenal hyperplasia due to 21-hydroxylase deficiency by genetic linkage analysis.

For the purpose of prenatal diagnosis of CAH, genetic linkage analysis by HLA genotyping with lymphocytes and cultured amniotic cells were performed in a family at risk in which two consecutive children had been affected with SW CAH. In addition, the response of serum 17-OHP to intravenous ACTH was determined in obligate carrier parents, and 17-OHP concentration of amniotic fluid was also measured at 16 weeks of gestation. As might be expected, the baseline levels of 17-OHP in obligate parents were significantly higher than that of normal control. Although the post stimulation response of 17-OHP to ACTH in the mother (I-2) was significantly higher than that of normal control, the post stimulation levels of 17-OHP were in normal range in the father (I-1). The 17-OHP level (5.7 ng/ml) in the amniotic fluid showed intermediate value compared to Pang's report (normal less than 30 ng/ml, CAH greater than 12.0 ng/ml) suggesting heterozygote of the fetus. Genetic linkage analysis by HLA genotyping with cultured amniotic cells revealed heterozygote in their fetus (II-3) who has received one chromosome No,6 containing HLA haplotype A24, B40, Cw3 (normal allele for 21-OH) from the father and the other chromosome No,6 containing HLA haplotype A2, Bw62, Cw4 (mutant allele for 21-OH D) from the mother. In conclusion, attempts to detect heterozygote for 21-OH deficiency by ACTH stimulation test were partially successful and prenatal diagnosis of CAH by the hormone studies in ammiotic fluid requires reliable values in normal, heterozygotes and patients group, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Percutaneous pulmonary valvuloplasty with a double-balloon technique.

Percutaneous pulmonary valvuloplasty with a double-balloon technique was used in eight patients, aged 10-40 years, in whom valvuloplasty with a single 20-mm balloon resulted in inadequate dilatation of the stenosis. The diameters of the additional balloon varied from 10 to 20 mm. Balloons were inflated manually at about 30-45 psi (206.7-310.1 kPa) and deflated immediately after disappearance of the balloon waist. The mean peak systolic pressure gradient was 85 mm Hg +/- 37 before valvuloplasty, and it decreased to 38 mm Hg +/- 28 afterward (P less than .05). Double-balloon dilation and simultaneous computed tomography were done in two cardiac specimens from adult cadavers. At inflations at 30 and 45 psi (206.7 and 310.1 kPa), the outflow tract was elongated, and the balloons also deformed into ovoid shape at the lower pressure. The double-balloon technique appears to be safe and effective for relief of pulmonary valvular stenosis in those cases in which single-balloon technique is inadequate, especially in older children and adults.

Magnetic resonance evaluation of mitral stenosis.

In order to study magnetic resonance (MR) abnormalities in mitral stenosis, electrocardiogram (ECG)-gated MR imaging was performed with a 2.0 T MR system in 41 patients with mitral stenosis before catheterization. Mean transverse diameter of the left atrium was 8.9 +/- 1.4 cm and anteroposterior diameter was 5.1 +/- 1.0 cm, indicating significant enlargement. Homogeneous or inhomogeneous flow-related signals in ventricular diastole were detected in the left atrial cavity in 32 cases (78%), and in the atrial appendage in 35 cases (85%). In 21 patients having a mean wedge pulmonary arterial pressure higher than 20 mm Hg, 20 patients (95%) showed flow-related signals in ventricular diastole. Other MR findings were mitral valve doming in diastole, flow-related signal in the pulmonary artery during systole, and left atrial thrombi.
