Effects of internal cleavages and mutations in the C-terminal region of NIa protease of turnip mosaic potyvirus on the catalytic activity.

The nuclear inclusion protein a (NIa) of turnip mosaic potyvirus is a protease processing the viral polyprotein into functional proteins. It has been shown that the NIa C-terminal 27-kDa protease cleaves itself between Ser-223 and Gly-224 to generate a 25-kDa protein lacking the C-terminal 20 amino acids. We have found a second internal cleavage near the C-terminus resulting in the degradation of the 25-kDa protein into a 24-kDa protein. Substitution of the active site Asp-81 or Cys-151 with Asn or Ser, respectively, prevented the second cleavage, suggesting that the internal cleavage is also due to the proteolytic activity of the NIa protease. This second internal cleavage was found to occur between Thr-207 and Ser-208, eliminating the C-terminal 36 amino acids from the 27-kDa protease. The proteolytic activity of the 24-kDa protein was not detected at all when it was measured using a nonapeptide containing the cleavage site between 6K1 and Cl as a substrate, suggesting that the C-terminal region between residues 208 and 223 contains essential amino acids for the processing of 6K1-Cl polyprotein. The deletion analyses of the C-terminal region revealed that at least 217 amino acids from the N-terminus are required for the catalytic activity of the NIa protease. The point mutation of Trp-212 to Ser, Gly-213 to Ser, or Ile-217 to Asp drastically abolished the catalytic activity, demonstrating that Trp-212, Gly-213, and Ile-217 are important for the processing of 6K1-Cl polyprotein.

Characterization of Nla protease from turnip mosaic potyvirus exhibiting a low-temperature optimum catalytic activity.

Nuclear inclusion protein a (Nla) protease of turnip mosaic potyvirus is responsible for the processing of the viral polyprotein into functional proteins. The Nla protease was found to exhibit its optimum catalytic activity at approximately 15 degrees and a bell-shaped pH-dependent activity profile with a maximum at approximately pH 8.5. Kinetic studies showed that both Km and V(max) values were lower at 12 than at 25 degrees in all three different pH conditions, pH 7.0, 7.4, and 8.3, indicating that the higher activity at 12 degrees is due to the lower value of Km. Interestingly, the self-cleavage of the 27-kDa protease to generate the 25-kDa protease occurred more rapidly at 25 than at 12 degrees, implying that the C-terminal self-cleavage site may interfere with the binding of the peptide substrate to the active site of the protease. Mutations and deletions at the C-terminal cleavage site had no effect on the temperature dependence of the proteolytic activity, demonstrating that the C-terminal self-cleavage is not related to the low-temperature optimum catalytic activity. The fluorescence measurement of the Nla protease upon temperature variation revealed that the protease undergoes a large conformational change between 2 and 42 degrees and a drastic transition near 45 degrees, suggesting that the low-temperature optimum catalytic activity is due to the highly flexible structure of the Nla protease.

Lung epithelial cell-released nitric oxide protects against PMN-mediated cell injury.

A calcium-independent type II nitric oxide (NO) synthase has been localized in lung epithelial cells; however, the function of NO. released by epithelial cells is unclear. We hypothesized that epithelial-derived NO may affect the interactions between polymorphonuclear neutrophils (PMN) and the alveolar epithelium and studied PMN adhesion and cytotoxicity to lung epithelial cells. A dose- and time-dependent production of NO. by L2 cells can be induced by a mixture of inflammatory mediators (cytomix) containing lipopolysaccharide, tumor necrosis factor-alpha, and interferon-gamma. Increased NO. production by L2 cells was associated with decreased 51Cr release by the epithelial cells after they were incubated with activated PMN. Addition of NG-monomethyl-L-arginine or oxyhemoglobin reversed the protective effects of cytomix, suggesting that increased NO. production by L2 cells was responsible for the decreased 51Cr release. However, PMN adhesion and intercellular adhesion molecule-1, a major adhesion molecule involved in PMN adhesion to epithelium, were increased by cytomix. We conclude that NO. released by lung epithelial cells was involved in protecting epithelial cells from PMN-mediated cytotoxicity. NO.-mediated protection of lung epithelial cells occurred in spite of PMN adhesion being increased, suggesting that reduced adhesion is not required for NO.-mediated inhibition of PMN cytotoxicity.

Contrasting response of lung parenchymal cells to instilled TNF alpha and IFN gamma: the inducibility of specific cell ICAM-1 in vivo.

Induction of intercellular adhesion molecule-1 (ICAM-1) by proinflammatory cytokines during inflammation plays an important role in regulating polymorphonuclear neutrophil (PMN) migration and localization. In this report, we examined the effects of tumor necrosis factor-alpha (TNF alpha) and interferon-gamma (IFN gamma) on specific lung cell expression of ICAM-1 in vivo and the accompanying morphological changes. Balb-c mice were treated with phosphate-buffered saline (PBS) alone or with PBS containing 5 micrograms TNF alpha or IFN gamma through intranasal instillation. Twenty-four hours after treatment, their lungs were processed for immunoblot analysis and electron microscope immunocytochemistry. In the normal lung, the ICAM-1 level is high on type I alveolar epithelial cells, medium on arterial and venous endothelial cells, low on type II epithelial cells and capillary endothelium, and not detectable on bronchial epithelium. Topical treatment of the lung with either TNF alpha or IFN gamma induced a 50-60% increase in total lung and alveolar ICAM-1. A dramatic increase of alveolar type II cell surface ICAM-1 was observed (> 20-fold). Both cytokines caused 2-3-fold higher ICAM-1 expression on capillary endothelial cells and a 40% increase of ICAM-1 on alveolar type I cells that was not uniform. However, due to the large total surface area of type I epithelium, type I cells contribute 70-86% of total alveolar septal ICAM-1 and > 90% of alveolar surface ICAM-1 in either treated or normal mouse lungs. Increased ICAM-1 expression was also observed on nonparenchymal endothelial and epithelial cells. Margination and sequestration of PMN in cytokine-treated lungs were observed by histologic examination, measurements of total lung myloperoxidase activity, and number of neutrophils recovered in bronchoalveolar lavage fluid. These results showed that TNF alpha and IFN gamma induce ICAM-1 expression and infiltration of neutrophils in the lung. The response of specific lung cells in terms of induction of ICAM-1 in response to cytokine stimulation varied significantly, particularly between type I and type II epithelial cells.

Prudent expert systems with credentials: managing the expertise of decision support systems.

'Black box' expert systems (ES) are mistrusted by clinicians. Errors generated by medical ES are also a significant cause for concern. We report new ES properties--prudence and credentials--that improve error management and underpin a new approach for improving the credibility of ES for clinical users. Prudent ES modify their output according to past experience. For a knowledge base built from 1610 cases, feature exception prudence (FEP) detected all interpretation errors (100% sensitivity for error detection). Although the false positive rate for FEP was high (47%), the 100% sensitivity meant that the 53% of cases that did not produce flags could be exempted from human validation. As more cases are processed, fewer cases should need human validation. Feature recognition prudence (FRP), a property of ripple down rules (RDR), proposed the correct alternative conclusion in 14% of incorrectly interpreted cases. Human expert validation of the flagged cases enabled context-sensitive credentials (accuracy, incidence and specificity of a given conclusion) to accumulate. Credentials should enable the user to judge the credibility of the ES output. An error management strategy based on credentialed, prudent ES should reduce the impact of error in the clinical environment. The empowerment of clinicians to critically evaluate ES credibility may facilitate greater confidence in, and acceptance of, ES by clinicians.

Immunocytochemical localization of the sites of superoxide dismutase induction by hyperoxia in rat lungs.

BACKGROUND: Rats exposed to 85% O2 can survive subsequent exposures to 100% O2. The development of this tolerance to oxygen toxicity is associated with an increase in total pulmonary content of both CuZn and Mn superoxide dismutases (SOD). It is not known, however, in which cells increases in these antioxidant enzymes occur. We carried out an electron microscopic immunocytochemical study to define the distribution of CuZnSOD and MnSOD and their sites of induction in rat lungs. EXPERIMENTAL DESIGN: The lungs of rats exposed to either air or 85% O2 were fixed by instillation of fixative 7 and 14 days after the exposures were started. Cryoultrathin sections were prepared from randomly selected, frozen tissue blocks and were immunolabeled for either CuZnSOD or MnSOD. The labeling densities of the two enzymes in the major alveolar parenchymal cells (type I and type II epithelial cells, interstitial fibroblasts, and endothelial cells), were evaluated. RESULTS: All of the alveolar septal cells studied displayed similar concentrations of CuZnSOD. After 7 or 14 days of exposure to 85% O2, the total lung content of CuZnSOD was increased because of cell hyperplasia and hypertrophy, but the intracellular concentrations of CuZnSOD did not increase. MnSOD was present in the mitochondria of all lung cells in control rat lungs but was highest in alveolar type II epithelial cells. Exposure to 85% O2 increased the concentration of MnSOD in the mitochondria of interstitial fibroblasts by 197 and 139% after O2 exposure of 7 days and 14 days, respectively. A significant induction of MnSOD also occurred in the mitochondria of alveolar type II cells after 7 days of hyperoxia exposure. Biochemical analyses showed that a significant fraction of the hyperoxia-induced MnSOD was in the form of an enzymatically inactive protein. CONCLUSIONS: Hyperoxia induced total lung CuZnSOD in proportion to lung cell hypertrophy and hyperplasia. Total lung MnSOD was increased in association with specific inductions of MnSOD protein expression in the mitochondria of type II alveolar epithelial cells and interstitial fibroblasts. Hyperoxia appeared to result in inactivation and accumulation of the inactive MnSOD as well as induction of enzymatically active MnSOD. The ability of fibroblasts and alveolar type II cells to replace inactive MnSOD or to augment active MnSOD in the mitochondria appears to play important roles in the development of tolerance to hyperoxia by rats.

Origins of sympathetic innervation to the mesenteric vein vessel in cats.

Physiological and histochemical studies have clearly demonstrated the control and innervation of the sympathetic nerve on the splanchnic veins. However, the origins of postganglionic fibers remain to be determined. In this study, we used the retrograde transport of horseradish peroxidase (HRP) technique to trace the origins of postganglionic neurons in pre- and paravertebral sympathetic ganglia that innervate the mesenteric vein. A segment (6-8 mm) of mesenteric vein close to the duodeno-jejunal junction was isolated. HRP was applied externally on the vein segment to allow uptake into the nerve endings. In 10 cats, there was a total of 9275 HRP-labeled neurons in the pre- and paravertebral ganglia. The neuron distribution (mean +/- S.E.M.) was as follows: 452 +/- 77 (49%) in the superior mesenteric ganglion (SMG), 248 +/- 62 (27%) in the right celiac ganglion (RCG), 111 +/- 23 (12%) in the left celiac ganglion (LCG) and 58 +/- 16 (6%) in the splanchnic ganglia. Although the appearance of HRP neurons in the paravertebral ganglia (T10-L3) was relatively sparse, there were still significant numbers in L1 (26 +/- 13, 3%), T13 (15 +/- 5, 2%) and other ganglia. The results indicate that more than 90% of the presynaptic sympathetic neurons innervating the mesenteric vein make their synaptic connections in the prevertebral ganglia, mostly SMG and then RCG as well as LCG. Postsynaptic neurons arising directly from the paravertebral ganglia (mostly L1 and T13) constitute to about 6% of the total.

Primary and immortalized (BEAS 2B) human bronchial epithelial cells have significant antioxidative capacity in vitro.

Antioxidant enzymes located in the bronchial epithelium can be expected to be important in protecting these cells against both endogenous and exogenous oxidants. In this study, human bronchial epithelial cells were isolated and cultured from specimens obtained from donors for lung transplantation. The levels and relative importance of different antioxidant enzymes were also assessed using an immortalized human bronchial epithelial cell line (BEAS 2B cells). Immunocytochemical studies showed a similar pattern of intracellular localization with the moderate degrees of labeling for Mn superoxide dismutase (SOD), CuZn SOD, and catalase in freshly isolated bronchial epithelial cells, bronchial epithelial cells in primary culture, and BEAS 2B cells. CuZn SOD and catalase decreased in labeling density whereas Mn SOD was unchanged when bronchial epithelial cells were placed in primary cultures. In contrast, Mn SOD and catalase were decreased in BEAS 2B cells compared with primary cultures. Although Mn SOD was low in BEAS 2B cells, it could be significantly induced by tumor necrosis factor treatment. Biochemical analysis showed remarkably similar catalase and glutathione reductase activities in primary cultured epithelial cells and BEAS 2B cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Intercellular adhesion molecule-1 expression on the alveolar epithelium and its modification by hyperoxia.

The distribution of intercellular adhesion molecule-1 (ICAM-1) on alveolar epithelial cells and the effects of exposure to 100% O2 on ICAM-1 expression in mouse lungs were studied by EM immunocytochemistry and immunoblot analysis. Cryoultrathin sections from mouse lungs exposed to air or 100% O2 for 84 h were labeled with a monoclonal rat anti-mouse ICAM-1 antibody. In the normal lung, abundant ICAM-1 expression was found on the alveolar surface of type I epithelial cells. Furthermore, ICAM-1 is highly concentrated on the surfaces near cell junctions. ICAM-1 was also found on the capillary surface of endothelial cells and alveolar surface of type II cells at densities considerably lower than that found on type I epithelial cells. After exposure to O2, the labeling density of ICAM-1 on the central surface of type I epithelial cells was not changed significantly. However, the gradient of ICAM-1 on the surfaces near cell junctions was nearly abolished. ICAM-1 labeling on the capillary surface of endothelial cells remained low. ICAM-1 was also markedly induced on the alveolar surface of type II epithelial cells after hyperoxic exposure. These results show that ICAM-1 is expressed primarily on type I epithelial cell surfaces near cell junctions. Exposure to hyperoxia causes a dramatic change in the distribution pattern of ICAM-1 on alveolar type I epithelial cells and induces expression of ICAM-1 on alveolar type II epithelial cells. These hyperoxia-induced changes may influence the associated neutrophil invasion/retention in the alveolar air spaces or alveolar walls.

An acute toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

The acute toxicity of a recombinant human interferon alpha A (code name: LBD-007) was evaluated in both sexes of Sprague-Dawley rats, 5 weeks old, by the oral, subcutaneous and intravenous routes of administration. Based on the results, LBD-007 was not considered to induce any toxicological effect on the rats in mortalities, clinical findings, body weights and gross findings. It is suggested that LD50 values in rats would be > 48 x 10(8) IU/kg in the study of oral administration and > 24 x 10(8) IU/kg in the study of subcutaneous or intravenous administration.

A 4-week subcutaneous toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

Recombinant human interferon alpha A (code name: LBD-007) was subcutaneously administered to both sexes of Sprague-Dawley rats at the doses of 0, 6, 12 and 24 x 10(6) IU/kg of body weight five days per week for 4 weeks to evaluate the subchronic toxicity. 20 to 50% of rats except the male control group showed minimal to mild, focal to multifocal renal mineralization. Whether renal mineralization is related to the test substance is not elucidated. Male rats dosed at 24 x 10(6) IU/kg showed the decrease of the absolute kidney weights and the increase of the brain relative weights. Female rats dosed at 24 x 10(6) IU/kg showed the increase of the absolute and relative ovary weights, and the decreases of specific gravity, bilirubin and urobilinogen in urine. However, no drug-related changes were noted in clinical findings, body weights, food consumption, water consumption, hematology, blood clinical chemistry and necropsy findings. Based on the results it is concluded that the estimated subcutaneous non-toxic dose of the recombinant human interferon alpha A (LBD-007) in rats is (6 approximately 12) x 10(6) IU/kg.

A 6-month subcutaneous toxicity study of recombinant human interferon alpha A (LBD-007) in Sprague-Dawley rats.

Recombinant human interferon alpha A (code name: LBD-007) was subcutaneously administered to both sexes of Sprague-Dawley rats at the doses of 0, 3, 6 and 12 x 10(6) IU/kg of body weight six days per week for 6 months to evaluate the chronic toxicity. Male rats dosed at 12 x 10(6) IU/kg for 26 weeks showed a decrease of ketone body in the urine, and female rats dosed at 12 x 10(6) IU/kg for 26 weeks showed a decrease of serum alkaline phosphatase. Microscopic changes with remarkable incidence were nephropathy, renal mineralization and ultimobranchial cysts of the thyroid gland in all groups of both sexes of rats. These changes were not considered to be drug-related toxicity because they appeared to be minimal, occasionally mild in severity and did not have dose-dependency as well as clinical significance. In general, no drug-related change was noted in clinical findings, body weights, food consumption, water consumption, hematology, organ weights and necropsy findings. Based on the results, it is concluded that the estimated subcutaneous non-toxic dose of the recombinant human interferon alpha A (LBD-007) in rats would be 6 x 10(6) IU/kg.

Protocol for out-patient management of dengue illness in young adults.

A prospective study on the practicality of an out-patient management protocol for dengue infection in adults was carried out during a 2-month period. Doctors were requested to follow the protocol and assessment was done on the patients' outcome, the admission rate, and the compliance to the protocol by doctors and patients. One hundred and sixty-two patients (mean age 27.3 years) were clinically diagnosed to have dengue illness. Among them, 82.7% had dengue fever (DF); 13.0% had dengue haemorrhagic fever (DHF) and 4.3% had dengue shock syndrome (DSS). Dengue aetiology was confirmed in 69.4% of the DF group and 85.7% of the DHF and DSS groups. There were no deaths among the 162 patients. The admission rate was 43.8%. A high compliance rate of 86.4% by the doctors and a low patient default rate of 16.4% showed that the protocol was practical.

Streptococcal pharyngitis in a primary care clinic.

A study to determine the sensitivity and specificity of the Directogen Group A Streptococcal Test was carried out in a primary care clinic in University Hospital, Kuala Lumpur. The study also looked at the prevalence of streptococcal pharyngitis in the clinic. We found that the rapid test has a sensitivity of 90.9% and a specificity of 90.7%. Viral pharyngitis is the commonest form (63%) of pharyngitis followed by Group A Beta haemolytic streptococcal pharyngitis (14.2%). The presence of exudates, tonsillar enlargement and the presence of tender cervical lymph nodes have a significantly higher association with streptococcal pharyngitis.

Hyperthyroidism following hypothyroidism--a case report.

A 25 year old female presented with hypothyroidism which was followed by the development of hyperthyroidism about 1 1/2 years later. This uncommon phenomenon is postulated to result from changes in the relative amounts of stimulatory and inhibitory TSH receptor antibodies. This case illustrates the possible continuum between Graves' disease and Hashimoto's thyroiditis within the broad spectrum of autoimmune thyroid disease.

Baroreflex adaptation and resetting on the control of arterial pressure.

In anesthetized and vagotomized dogs, the carotid sinuses were vascularly isolated to control and adjust the carotid sinus pressure (CSP). In 10 animals, we first observed the changes in systemic arterial pressure (SAP) during CSP holding at different levels. Thereafter, the CSP was lowered to 40 mmHg and increased to 180 mmHg in a ramp fashion to obtain the CSP-SAP baroreflex function curves. The system was initially stabilized at a sinus holding pressure (HP) of 100 mmHg and an average SAP of 107.5 mmHg. During a 15 min HP at 50 mmHg, the SAP elevated to 119.4 mmHg at the beginning and then adapted downward to a stable level of 107.2 mmHg in 5 min. On the other hand, a change of HP to 150 mmHg caused a SAP decrease to 69.0 mmHg and then adapted upward to a quasi-steady level of 87.5 mmHg in 5 min. The CSP-SAP curves constructed by logistic function analysis from the ramp protocol were also significantly affected by the HPs. When the CSP-SAP curve following a HP of 100 mmHg was used as a control, the curve was shifted to the left and to the right following a HP of 50 and 150 mmHg, respectively. There were significant increases in the threshold, midpoint and saturation pressures as the HP was elevated. The degree of acute resetting (delta threshold pressure/delta HP) was calculated to be 33.0% (16.5/50) and 55.4% (27.7/50) for a change of HP from 100 mmHg to 50 mmHg and to 150 mmHg, respectively. However, the resetting was characterized by a parallel shift of the CSP-SAP curves without significant changes in the baroreflex sensitivity or gain. As the acute adaptation and resetting are correlated using a graphic analysis, we hypothesize that the baroreceptors can recognize a new pressure level within minutes. The inherent ability incurs downward and upward adaptation as well as resetting at lower and higher holding pressure, respectively.

Isolation of water-soluble cytochromes from cyanobacteria by adsorption chromatography.

A simple procedure using ammonium sulfate to fractionate water-soluble cytochromes c553 and c550 on Sephacryl S-200 gel is described. The usefulness of this procedure has been studied using the crude extracts of mesophilic cyanobacteria. It was found that almost all the cytochromes were adsorbed on to the gel at 2.34 M ammonium sulfate and were eluted at decreasing salt concentrations. The cytochromes were free of interfering phycobiliproteins and thus were suitable for the study of isoelectric points. It was also found that this procedure allowed a clear separation of the cytochromes based on their hydrophobicities. The order of elution was cytochrome c553, then cytochrome c550, indicating that c550 is more hydrophobic than c553. All these results show that this procedure provides both a simplified and an efficient purification of the cytochromes and insight into their surface properties. The cytochromes of Microcystis aeruginosa were purified to homogeneity using this procedure and other existing ones. Homogeneous cytochromes c553 and c550 were chromatographed on Sephacryl S-200 at 1.75 M (NH4)2SO4 and found to elute in the same order as reported earlier for the cytochromes in the crude extracts. In addition, cytochrome c550 was found to be more heat resistant and less water soluble than cytochrome c553.
