Detection of multivessel coronary artery disease by early exercise testing after first myocardial infarction.

The ability of a symptom limited treadmill exercise test (TMT) to predict the presence of multivessel coronary artery disease (MVD) three weeks after a first myocardial infarction was assessed in 76 male patients aged 25-65 (mean 40.9 +/- 10.1) years. There were 6 patients with normal coronaries, 36 with single vessel disease (SVD), 20 with double vessel disease (DVD), and 14 with three vessel disease (TVD). TMT was positive in 20/34 (58.8%) patients with MVD, and 4/42 (9%) patients with SVD or normal coronaries. The predictive accuracy of a positive TMT in detecting MVD was 83.3%, and the predictive accuracy of a negative TMT in excluding MVD was 73.1%. It is concluded that TMT is useful test for predicting the presence or absence of MVD in survivors of a first transmural myocardial infarction.

Acceleration of heat transfer in vial freeze-drying of pharmaceuticals. I: Corrugated aluminum quilt.

The medium offering the greatest resistance to heat transfer from the freeze-dryer shelf to the moving and subliming interface within the product is the space between the flat shelf top and the concave vial bottom. The resistance to heat transfer can be greatly reduced by improving the thermal conductivity of this intervening space. Several heat transfer augmentation devices, which fill this gap, are described. The devices are inexpensive and easy to use. Experimental data show that the resistance of the intervening space is reduced appreciably and the drying time is greatly reduced.

Effect of garlic pretreatment on isoprenaline-induced myocardial necrosis in albino rats.

Albino rats were pretreated orally for seven days with different doses of the essential oil and juice of garlic. These animals were challenged with isoprenaline (85 mg/kg, s.c.) on day six and seven to induce myocardial necrosis. The same rats were also subjected to swimming test before and after forty eight hr of first injection of isoprenaline. Pretreatment with essential oil (2.0 and 4.0 mg/kg) and juice (2.0 and 4.0 ml/kg) significantly protected the myocardium from isoprenaline-induced necrosis. These animals also exhibited better swimming performance. In separate group of animals, pretreatment with garlic juice (4.0 ml/kg) or essential oil (4.0 mg/kg) improved the chances of survival in isoprenaline-challenged animals.
