Role of rectus abdominis myocutaneous flap in surgery for breast cancer.

Rectus abdominis myocutaneous flap has been used in four different therapeutic situations after mastectomy for breast cancer. These include (1) to obtain skin cover after radical mastectomy with liberal skin sacrifice for locally advanced cancer in 14 cases, (2) for skin cover to obtain primary wound healing in 10 cases treated by preoperative radiotherapy, (3) for delayed breast reconstruction in three patients and (4) for immediate postmastectomy breast reconstruction in 25 patients. Prosthesis was not used for breast reconstruction. The operative morbidity was minimal but 1-2 units of extra blood transfusion was required. Administration of adjuvant treatment, including chemotherapy and radiotherapy was not affected. Reconstruction was satisfactory.

Effects of 1,1-dichloro-2,2-bis[p-chlorophenyl]ethylene (DDE) on lactation in rats.

An inverse correlation between the concentration of DDE in human breast milk samples and the duration of breast feeding prompted the present study of the effects of DDE administration on the lactational performance of primiparous rats. Daily doses of 10 mg p,p'-DDE/kg body weight were given to virgin female Sprague-Dawley rats 5 d/wk for 5 wk prior to mating and continued throughout the gestation and lactation periods. Lactation capacity was determined by monitoring neonatal growth and by measuring milk production, milk composition (total protein, total lipid, and lactose), and mammary-gland weight and nucleic acid content on d 9 and 20 postpartum. Gross toxicity was assessed by monitoring clinical signs and body weight of the dams, and by measuring organ weights of the dams on lactation d 9 and 20. Histopathological evaluation of the mammary glands and selected organs in the dams and pups was also performed. The dose level of DDE employed was apparently not toxic to the dams and did not have a pronounced effect on neonatal mortality. No significant differences between DDE-treated and control groups were observed for any of the lactation parameters, even though the concentration of DDE in the milk of treated rats was approximately two orders of magnitude greater than the upper range of the DDE levels measured in human milk samples. These findings indicate that DDE does not adversely affect lactation or neonatal growth in Sprague-Dawley rats at the dose level used in this study.

Study of anti-inflammatory activity in the leaves of Nyctanthes arbor tristis Linn.--an Indian medicinal plant.

Nyctanthes arbor tristis Linn. (Harsingar) is widely used as a decoction in the Ayurvedic system of medicine for treatment of sciatica and arthritis, but it has not yet been screened scientifically. In the present study, the water soluble portion of the alcoholic extract of the leaves of Nyctanthes arbor tristis (NAT) was screened for the presence of anti-inflammatory activity. NAT inhibited the acute inflammatory oedema produced by different phlogistic agents, viz. carrageenin, formalin, histamine, 5-hydroxytryptamine and hyaluronidase in the hindpaw of rats. The acute inflammatory swelling in the knee joint of rats induced by turpentine oil was also significantly reduced. In subacute models, NAT was found to check granulation tissue formation significantly in the granuloma pouch and cotton pellet test. Acute and chronic phases of formaldehyde induced arthritis were significantly inhibited. NAT was also found to inhibit the inflammation produced by immunological methods, viz. Freund's adjuvant arthritis and PPD induced tuberculin reaction. Thus anti-inflammatory activity in leaves of Harsingar supports its use in various inflammatory conditions by the followers of the Ayurvedic system of medicine.

Test for reproducibility of metered-dose aerosol valves for pharmaceutical solutions.

Seven independent testing sites received metered-dose aerosols containing a standard test solution to assess a newly designed protocol for studying the reproducibility of metered-dose aerosol valves. In accordance with the rather small sampling protocol design, the amount of product dispensed per actuation was measured over the life of the aerosol at designated regions of actuation. A statistical analysis of the data collected at each testing site clearly indicated that the testing protocol is sufficiently reliable and workable for assessing the reproducibility of valve delivery for a given lot of metered-dose valves.
