A long term treatment with a venotropic drug. Results on efficacy and safety of Daflon 500 mg in chronic venous insufficiency.

UNLABELLED: Drugs for long term administration have to prove their efficacy and safety. Previously published double blind controlled studies (from single dose to two months treatment) have already demonstrated the phlebotropic activity of Daflon 500 mg in chronic venous insufficiency (CVI). The aim of the study was to investigate the safety of this agent during one year of continuous administration. Two-hundred and fifteen out-patients suffering from CVI received Daflon 500 mg, 2 tablets per day. Therapeutic activity was evaluated every 2 months on: 1) venous symptoms (functional discomfort, cramps, evening oedema) assessed by a 0 to 4 scale; 2) supramalleolar and calf circumferences; 3) overall assessment of efficacy (excellent, useful, nil). Acceptability was assessed by recording the side effects and measuring laboratory parameters. RESULTS: 170 patients completed the study. Functional symptoms were statistically significantly improved as shown by the following: functional discomfort: 0.55 +/- 0.06 vs 2.63 +/- 0.06, supra-malleolar circumference in cm: 22.5 +/- 0.2 vs 23.1 +/- 0.2, and calf circumference in cm: 34.7 +/- 0.3 vs 35.2 +/- 0.3. This improvement in the symptoms quickly appeared from the first control (M2) and reached about 50% of the total decrease. Overall assessment of efficacy was evaluated as follows: excellent = 58%, useful = 33%, nil = 9% of the cases. Laboratory parameters remained constant during the 12 months. Side effects were essentially gastralgia (n = 7). According to these results, it appears the efficacy and safety of Daflon 500 mg are corroborated even after a one year administration.

Skin perfusion pressure in leg ulcers assessed by photoplethysmography.

The aetiology of lower limb ulcers is often the result of intricate vascular pathology and the quality of the peri-ulcer microcirculation is a major factor in the prognosis of the disease. We measured the skin perfusion pressure (S.P.P.) using an infra-red photoplethysmograph of the PPG type (ESM, Mauguio, France). The study included 30 healthy subjects, 25 patients with leg ulcers of varied etiology (venous: 14, arterial: 11), and 15 patients with uncomplicated varicose veins. Measurements were undertaken in dorsal decubitus position after a 15 minute rest. The S.P.P. was expressed as the percentage of humeral blood pressure. The results showed an important decrease of S.P.P. in patients with arterial ulcers when compared to patients with healthy legs (39.9 +/- 16.4% and 82.8 +/- 10.0% respectively). The S.P.P. values were intermediate (54.4 +/- 19.3%) in patients with venous ulcers while they were normal in the patients with uncomplicated varicose veins (87.3 +/- 12.3%). Among this population, 18 patients (11 with uncomplicated varicose veins and 7 with venous ulcers) were investigated before and after a one month treatment with a flavonoid (Daflon 500 mg, 2 tablets per day). An increase of S.P.P. to normal values after treatment was found in patients with venous ulcers though not in patients with varicose veins. This non invasive procedure appreciating the microcirculation status in peri-ulcerous skin could be of predictive value for the healing potential. It could be a good method to assess the efficacy of vasculotropic agents.

[Acute methotrexate poisoning: apropos of 16 cases reported to the Paris Poison Control Center and review of the literature].

16 cases of acute methotrexate (MTX) poisoning were reported to the Paris Poison Control Centre and 62 others were published between 1974 and 1995. Until 1992, MTX was mainly prescribed for neoplastic diseases. Clinical features involve acute renal failure, pancytopenia, and cutaneous or mucous injury. These cases emphasise major risk factors responsible for misuse and overdosage. Since 1992, MTX has been increasingly used in rheumatological or pulmonary diseases (rheumatoid arthritis, steroid-dependent asthma). Most of the overdosages were due to a medical misuse (in the dosage regimen or a misunderstanding of the prescription by nurse, pharmacist or patient). All patients had a bone marrow suppression. The treatment is based especially on early administration of folinic acid rescue.

[Pharmacovigilance off the beaten track: herbal surveillance or pharmacovigilance of medicinal plants].

For several years, herbal medicines have been increasingly consumed by patients without prescription. They are traditionally innocuous but as medicinal products they require drug surveillance in order to identify their risks. However, the first concern is to give them a legal status, to evaluate their efficacy and to control their safety. Published data show that the risk is due either to a contaminant or to an added drug or falsification. Since 1985, the Regional Pharmacovigilance Centres have received about 341 reports of undesirable effects due to herbal medicines: among them 30 cases of hepatitis were reported associated with germander which has been now withdrawn from the French market. This example well illustrates the role of the national system and its responsibility for collecting and evaluating ADRs due to herbal medicines.

Papular mucinosis with rapid spontaneous regression in an HIV-infected patient.

Papular mucinosis is a rare, papular eruption caused by the dermal deposition of mucin. Three clinical subtypes have been described. The evolution is usually chronic and no effective treatment is available. Papular mucinosis has recently been reported in patients with AIDS. We describe another case of papular mucinosis in an HIV patient with an uncommonly rapid, total regression.
