Plasma exchange in bullous pemphigoid.

41 patients with pemphigoid entered a multicentre randomised study of the efficacy of plasma exchange. All patients received 0.3 mg/kg daily oral prednisolone, increased weekly if the disease remained active. 24 patients received plasma exchanges in addition (eight large-volume exchanges over 4 weeks), and 17 had prednisolone only. 4 patients, 2 in each group, were withdrawn from the study. The initial dose of prednisolone was effective in 13 of 22 patients receiving plasma exchange but in none of 15 patients receiving prednisolone only. Control of the disease was obtained with a mean daily prednisolone dose of 0.52 +/- 0.28 mg/kg in the plasma exchange group v 0.97 +/- 0.33 mg/kg in the other group and a mean cumulative dose of 1240 +/- 728 mg v 2770 +/- 1600 mg. This finding suggests that plasma exchange allows a substantial saving of corticosteroids in the management of pemphigoid. This sparing effect was observed whether or not serum anti-basement membrane antibodies had been detected before treatment.

[Hodgkin's disease complicating actinoreticulosis].

The authors describe the occurrence of Hodgkin's disease in a patient suffering from actinic reticuloid since 12 years. The severity of the disease (MED less than 1 mJ/cm2 UVB) imposed corticosteroids and immunosuppressive therapy (chlorambucil). The role of actinic reticuloid's pathogeny (persistent light reaction) and principally use of immunosuppressive drugs are discussed and may explain the occurrence of Hodgkin's disease.

[Bullous pemphigoid treated by plasma exchange. Open study in 10 patients].

Ten patients with bullous pemphigoid underwent low volume plasma exchange (1/3 to 1/2 of the plasmatic mass) 3 times a week for 2 to 6 weeks followed, in some of them, by maintenance corticosteroid therapy. Eight patients benefited from this treatment. Complete control of the disease was obtained with PE alone in 3: low-dose corticosteroids were sufficient to procure remission in 2, and dosage could be reduced in 3 patients who previously required high doses of corticosteroids. No serious side-effects were recorded. The best results were observed in patients who had been ill for less than 3 months. Thus, low volume plasma exchanges make it possible to obtain clinical improvement while avoiding the rebound phenomena sometimes observed after high volume plasma exchanges. Circulating immune complexes and pemphigoid antibodies usually decreased under treatment, but there was no close correlation between these changes and therapeutic effectiveness.

Neutrophil chemotaxis in psoriasis before and after PUVA therapy.

Chemokinesis and chemotaxis of neutrophil leucocytes were studied by migration under agarose in 35 patients with psoriasis and compared with 35 healthy controls. Eschericheae coli filtrate and lipopolysaccharide were used as chemo-attractants. Of these patients, 14 were tested before and after photochemotherapy. The chemotactic capacities of psoriatic serum before and after PUVA therapy were also investigated. The increase in the chemotactic activity of psoriatic polymorphonuclear leucocytes (PMNs) was not significant and remained unchanged after PUVA therapy. The activated psoriatic serum showed a slight increase in chemotactic activity when compared with normal activated serum. These results do not support an intrinsic abnormality of psoriatic PMNs, and their migration into psoriatic lesions could be chiefly due to the presence of chemotactic factors in involved epidermis.

[Hydatid cyst of the heart diagnosed with bidimensional echography. Apropos of a case].

A hydatid cyst of the left ventricular posterior wall was diagnosed by 2D echocardiography in a 36 year old North African patient presenting with exertional dyspnoea and atypical chest pain. Clinical examination was negative but the surface ECG showed inverted T-waves in the infero-lateral leads. Postero-anterior and lateral chest X rays were initially interpreted as normal but 2D echocardiography immediately revealed a cystic cardiac tumour which very probably was hydatid, given the patient's origin. Thorough investigation showed no other localisations and hydatid serology was also negative. Left ventricular and selective coronary angiography were performed to determine the vascular relationships of the cyst: ablation was carried out under cardiopulmonary bypass and pathological examination confirmed the diagnosis of hydatid disease.

[Kaposi's sarcoma during local and general steroid therapy for psoriasis (author's transl)].

The authors describe a new case of Kaposi's sarcoma associated with psoriasis. The patient (a 68-year-old Nord-African man) had suffered from psoriasis for 14 years and had applied local betamethasone or fluocinolone for 125 months when Kaposi's sarcoma occurred. The association between these two diseases is discussed. The role of local steroid therapy and the immunological abnormality occurring in psoriasis are analyzed in order to explain the association between psoriasis and Kaposi's sarcoma or other malignant diseases.

[Circulating immune complexes in skin disease patients. Study and literature data (author's transl)].

Circulating immune complexes (C.I.C.) were investigated in 244 patients with various skin diseases and 100 healthy subjects. C.I.C. were detected by the PEG-C4 assay, firstly proposed by Digeon et al. using the precipitation by polyethylene glycol (PEG 3,5 p. 100) and the determination by laser nephelometry of complement component C4 in sera and in precipitates. The percentage of C4 precipitated and of positive subjects were significantly increased in numerous cutaneous diseases: systemic lupus erythematosus, scleroderma, pemphigus, bullous pemphigoid, dermatitis herpetiformis, psoriasis, contact dermatitis and lichen planus. Two cases of dermatomyositis, 3 cases of post herpetic erythema multiformis and 2 cases of Kaposi-Juliusberg syndroma were also positives but no definite conclusion can be given because of the few patients tested. On the contrary, the values of precipitated C4 are normal in most cases of atopic dermatitis (the method does not detect IgE-C.I.C.) scabies, porphyria cutanea tarda, cutaneous epithelioma and discoid lupus. In chronic urticaria and in mycosis fongoides the mean values of precipitated C4 are significantly increased but the number of positive subjects is low and the significance of these results is uncertain because of the wide range of the values. The results of the present study are compared with the literature data. The value of C.I.C. determination in determining the evolutivity of skin diseases and their possible role in pathogenesis are discussed.
