Imipramine for the treatment of sterility in patients following retroperitoneal lymph node dissection.

The major long-term morbidity following retroperitoneal lymph node dissection is sterility resulting from sacrifice of the thoracolumbar chain or the hypogastric plexus during dissection of the iliac vessels at the bifurcation. Seven males who were devoid of sperm emission and ejaculation as a result of this kind of operation for testicular cancer were treated with imipramine 25 mg t.i.d. In 2 of the patients, antegrade ejaculation appeared during treatment with this drug. We feel that this success rate of 2 out of 7 calls for further studies, as the problem of these young men was considered hopeless until recently.

Experience with surgical management of ectopic ureterocele.

We evaluated 16 children with ectopic ureterocele diagnosed and treated between 1970 and 1980. The clinical presentation, radiological findings and operative management are presented and discussed. Since ectopic ureterocele has a broad spectrum of presentation and pathophysiology, treatment must be individualized. Upper pole heminephrectomy with partial ureterectomy, thus avoiding ligation of the ureteral stump, is the preferred surgical treatment. Transurethral uncapping of the ureterocele is indicated only for extremely ill infants with severe septicemia that does not respond to conservative treatment. Unsuspected ectopic ureterocele discovered incidentally during surgery is best managed either by upper pole nephro-ureterectomy or reimplantation of both ureters in their common sheath.

Prostatectomy in factor XI (plasma thromboplastin antecedent) deficiency.

A total of 10 patients with factor XI deficiency underwent prostatectomy: 3 who underwent suprapubic prostatectomy and 3 who underwent transurethral resection received fresh frozen plasma preoperatively, while 4 received fresh frozen plasma only in response to bleeding. We conclude that all patients who present with partial or severe factor XI deficiency should receive fresh frozen plasma preoperatively and for at least 10 to 14 days postoperatively. We believe that all Ashkenazi Jews who are about to undergo a urological operation should be screened for factor XI deficiency even when a history of a bleeding tendency is not present.

Zinc deficiency during intravenous hyperalimentation.

Two cases of patients developing symptoms and signs of zinc deficiency while on total parenteral nutrition are presented. Supporting causes for the appearance of early low serum zinc levels are discussed. These cases serve to further stress the need of adding trace elements to solutions given to patients on long-term total parenteral nutrition.

Partial hepatectomies with a hand-held laser knife: An experimental study.

Fifteen mongrel dogs underwent a left lateral non-anatomical partial hepatectomy using a carbon dioxide laser, diathermy or a surgical scalpel. An original prototype of a compact hand-held laser was used and its effects were compared with those of the other two instruments. An improved dry cat surface was achieved by placing a rubber snare proximal to the planned incision. While no significant differences in the blood loss or in the operating time were found among the experimental groups, the laser beam sealed vessels of up to about 6 mm in diameter without any complementary suture ligatures, which were needed in the other groups. Histological studies revealed less tissue trauma in the sections performed by the laser and only slight secondary inflammatory reaction.

Semen quality and hormonal status of patients following testicular torsion.

16 patients, 4 months to five years following unilateral torsion of the testis were evaluated as to semen quality and hormonal status. In patients operated within 12 hours of the onset of pain 44% had normal semen quality while in those operated following more than 12 hours only 20% had normal semen analysis. FSH, LH and testosterone levels were normal in 14 of the 16 patients. One patient had a low testosterone level and slightly elevated FSH, another patient had slightly elevated levels of both FSH and LH.

Radical retropubic prostatectomy in the treatment of localized prostate cancer: experience with 100 consecutive cases.

In recent years radical retropubic prostatectomy is used with increasing frequency to treat localized cancer of the prostate. We report our experience with 100 consecutive cases of prostate cancer that underwent this operation; there were no cases of mortality and a 7% rate of postoperative complications. More than 90% of the patients are socially continent postoperatively and 20% have retained sexual potency. Ninety-four percent of the patients are free of disease (mean follow-up period 18 months). In our opinion, such results support the use of radical retropubic prostatectomy for the treatment of localized prostate cancer in well-selected patients.
