[Granular cell tumor of the breast].

Report about a 46 year old women with granular cell tumor of the breast. Clinical features, mammography and sonography has appearances highly suspicious for breast cancer. Tumorectomy and axillary lymphonodectomy were performed. Estrogen and progesterone receptor both were histochemically and biochemically negative. Estradiol and prolactin levels in serum were normal. Cathepsin D in cytosol was slightly elevated, DNA-flow-cytometry revealed no signs of malignancy. Evident was the high content of protein S100 in cytosol of tumor cells.

[Treatment of acute salpingitis with tetracycline/metronidazole with or without additional balneotherapy, Augmentin or ciprofloxacin/metronidazole: a second-look laparoscopy study].

110 patients suffering from laparoscopical verified salpingitis and desire for a baby, were treated with tetracycline (oxytetracycline or doxycycline; TC)/metronidazole (n = 67), augmentan (n = 22) or cipropfloxacin/metronidazole (n = 21). After an average period of 11.6 weeks, all patients underwent second-look laparoscopy with dye insufflation. In 34 patients treated with TC/metronidazole, the effects of additional physio-therapeutical measures were examined under conditions as they prevail in a Spa. 33 patients without balneotherapy served as controls. All the 4 groups were comparable (p greater than 0.05) in respect of mean age, percentage, share of nulliparous women, salpingitis gonorrhoica, contraceptive behaviour and also of the stage of salpingitis. All antibiotic regimens used resulted in a prompt decrease of inflammatory clinical signs after five days (temperature, blood sedimentation rate, leukocytes). Only 2 of 34 patients treated by additional cure at a Spa reported complaints, whereas complaints were reported by 14 of 33 control patients (p less than 0.01), 7 of 22 (p less than 0.01) treated with augmentan and to 7 of 21 (p less than 0.01) treated with ciprofloxacin/metronidazole. The tubal occlusion rates amounted to 33.3% (TC/metronidazole), 32.3% (TC/metronidazole and balneotherapy), 22.7% (augmentan) and 23.8% ciprofloxacin/metronidazole. The differences did not attain statistical significance (p greater than 0.05). With regard to adhesions, there were, likewise, no significant differences between findings at first laparoscopy and second look-laparoscopy, respectively. It is concluded, that additional physiotherapeutic measures, after antibiotic therapy of acute salpingitis, reduce the frequency of lower abdominal pain, but do not result in an improvement of tubal occlusion and reduction of adhesions.(ABSTRACT TRUNCATED AT 250 WORDS)

Possible dual effect of synapses that are putatively purely excitatory or purely inhibitory: bases in stability theory and implications for neural network behavior.

Depolarization of an excitable membrane has a dual effect; excitatory in that it causes rapid opening of calcium and/or sodium channels but inhibitory in that it also causes those channels to inactivate. We considered whether apparently paradoxical or dual behavior might be exhibited by excitatory and inhibitory synaptic inputs. We used the classic Hodgkin-Huxley model for voltage-gated channels plus leakage channels of appropriate selectivity for ligand-gated postsynaptic channels. We summarize a model cell's behavior by calculating elicited firing frequency as a function of reversal potential and conductance of summed synaptic inputs, using stability theory and direct simulations. Dual behavior is elicited in the model with reasonable densities of ligand-gated channels. Thus a particular synaptic input to a neuron may be either excitatory or inhibitory depending on simultaneous activity of other synaptic inputs to the cell. This input-output map may give rise to biologically realistic and rich behaviors as an element of computed neural networks, and still be computationally tractable.

[Late metastasis of a breast carcinoma to the uterine corpus and cervix].

In a 55-years-old woman 11 years after radical operation of a left-sided breast cancer a late metastasis in the uterine corpus was diagnosed and treated by subtotal hysterectomy. In the further course a carcinoma developed in the opposite breast, which may be most probably a second primary breast cancer. However a contralateral metastasis of the earlier treated breast cancer cannot certainly be excluded. Finally, a metastasis of the left sided-breast cancer was diagnosed in the uterine cervix. This case shows, that gynaecological bleedings also may be caused by atypical metastasis of a breast cancer.

[Image analytic DNA cytometry--a possibility for assessment of prognosis after resection of ductal pancreatic cancers?].

In this study we analysed DNA-ploidy as a potential prognostic parameter in ductal pancreatic carcinoma. Paraffin embedded histological material, obtained by resection from 34 patients with a ductal pancreatic carcinoma, was selected for analysis. Tumor areas within the paraffin embedded material were identified by HE-stained reference sections. One 50 microns section was dewaxed, rehydrated and mechanically and enzymatically prepared to form a suspension of 10000 cells/ml. One milliliter of the suspension, which contained bare nuclei with small rests of cytoplasma, was centrifuged on glass slides. The fixed nuclei were air-dried and stained by Feulgen SITS technique, which allows for the quantitative measurement of DNA. The DNA analysis was carried out with a computer-controlled single-cell cytophotometry. In contrast to using flow cytometry, only the tumor cells were measured by image-cytometry. Overlapping nuclei, dirt and other artifacts as well as inflammatory cells were efficiently eliminated. With DNA image-cytometry, we could differentiate between hypotriploid, triploid, hypertriploid and tetraploid tumors. The DNA-content provided the strongest influence on prognosis in the multivariate analysis.

The decision to execute a durable power of attorney for health care and preferences regarding the utilization of life-sustaining treatments in nursing home residents.

One hundred three nursing home residents were interviewed regarding their preferences for the choice of an agent for health-care decision making while being offered the opportunity to execute a Durable Power of Attorney for health care. They also completed a questionnaire that tapped their preferences regarding the use of four types of life-support treatment under three hypothetical levels of future cognitive functioning. Factors that might influence these preferences, such as previous experiences with life-sustaining treatments, religious beliefs, and personal values, were also examined. Participants tended to choose their son or daughter as their agent for future health-care decision making. They had clear and consistent patterns of preferences regarding the utilization of life-sustaining treatment. Generally, participants opted not to be treated, although there was variability among participants. They were even less inclined to opt for treatment as their perceived level of future cognitive functioning declined, or when the life-sustaining treatment involved permanent rather than temporary procedures.

[Endometrial findings in long-term treatment with synthetic sex steroids in Turner syndrome].

In 9 patients with Turner's syndrome (karyotype 45/X0) aged from 23 to 50 years (average age 34 years) a curettage was performed. In these women there has been a treatment with mestranol 80 micrograms or ethinylestradiol 50 micrograms and chlormadinome acetate 2 mg as a sequential therapy in a cyclic manner for 5 to 22 years. Additionally in 3 patients a biopsy from the uterine cervix was performed because of abnormal colposcopic findings. Only in one patient an atrophic endometrium was found, whereas in the other patients the endometrium showed a weak proliferation or a slight secretion. Ectasia of the endometrial glands was observed in 4 patients. A hyperplastic endometrium wasn't found in any patient.

Hypoxic modulation of systolic blood pressure and urinary sodium excretion in spontaneously hypertensive rats after carotid body denervation.

To explore the role of arterial chemoreceptors, the effect of hypobaric hypoxia on urinary sodium excretion and systolic blood pressure was investigated in conscious spontaneously hypertensive rats (SHR) with carotid body denervation (CBD) or after sham-operation (SO). Denervation of the carotid bodies was performed by section of the carotid sinus nerves. Exposure to hypobaric hypoxia equivalent to high altitude of 4000 m led to a more pronounced decrease in systolic blood pressure in CBD-rats than in SO-rats. The pattern of urinary sodium excretion observed on the first two days of hypoxia in both groups was not affected by the chemodenervation. It is being suggested that arterial chemoreceptors do not play a critical role in blood pressure and natriuretic responses to hypobaric hypoxia in conscious SHR.

[Applicability of the subrenal capsule assay (SRCA) in primary breast carcinoma].

We have examined the application of the subrenal capsule assay (SRCA) by use of normal immunocompetent (NIC) mice for chemosensitivity testing of primary breast cancers. In 18 out of 20 carcinomas an evaluation was possible. AB mice (10 breast cancers) and B6D2-F1 mice (10 breast cancers) were taken as recipients. Measurement of the size and the histological examination of all transplants were obligatory performed before and after finishing the assay on day 6. By use of AB mice as well as B6D2 mice in each group in 6 out of 9 evaluable cancers occurred a significant growth of the transplanted tissues after 6 days (p less than 0.05). It could be demonstrated by the histological examination that the enlargement was a consequence of the immunological reaction of the NIC mouse to the xenogeneic material. In most cases (76 of 97 used mice) the transplants consisted of inflammation cells only. Vital tumor cells were seen only in few cases (21 of 97 used mice). It is concluded, that the SRCA is not suitable for chemosensitivity testing of human breast cancer, if NIC mice as recipients are used.

[Ablative hormone therapy and chemotherapy of metastatic breast cancer].

Case report of a 45-year-old woman with breast cancer who underwent mastectomy with axillary lymphonodectomy and radiation therapy. Two years after primary therapy liver metastases have been proved by sonography, liver scintigraphy and computerized tomography scanning. A nearly complete remission was achieved by ablative hormone therapy and CMF-chemotherapy.

Effect of hypobaric hypoxia and almitrine on voluntary salt and water intake in carotid body denervated spontaneously hypertensive rats.

The effect of moderate hypobaric hypoxia and almitrine bismesylate (almitrine) on salt and water intake was investigated in carotid body denervated (CBD) and sham-operated (SO) spontaneously hypertensive rats (SHR). The animals were kept singly in metabolic cages and given free access to food, water and a 2.5% NaCl-solution. Oral administration of almitrine and exposure to hypobaric hypoxia for five days evoked a suppression of voluntary salt intake in SO-SHR, but not in CBD-SHR. Exposure of SO and CBD-SHR to hypobaric hypoxia resulted in a significant decrease in water intake in animals of both groups on the first day of hypoxia. Stimulation of the arterial chemoreceptors by almitrine induced no change in water intake similar to that evoked by hypobaric hypoxia. These results prove that stimulation of the arterial chemoreceptors has a direct effect on salt appetite but not on water intake in SHR.

[Effectiveness of perioperative preventive use of antibiotics with metronidazole or doxycycline in vaginal hysterectomy].

To prove the effectiveness of perioperative antibiotics prophylaxis (PABP) in prevention of postoperative infections after vaginal hysterectomy the efficacy of rectale Metronidazole application (5 times 500 mg; n = 192) was compared with that of Doxycyclin (2 times 200 mg intravenously; n = 116). The infectious morbidity after both prophylactic antibiotic regimes was compared to that of an untreated group (n = 186). In spite of additionally colporrhapies the feverish standard morbidity without prophylaxis was 13.4%, after Metronidazole application 13.5% and after Doxycyclin 2.6% (p less than 0.05). Simultaneously the rate of necessary antibiotic treatment decreased significantly. Pelvic infections complicated 16.7% of vaginal hysterectomies having no prophylaxis, but only 4.3% after Doxycyclin medication (p less than 0.05). Postoperative urinary tract infections (UTI)--mostly asymptomatic bacteriuria--were not influenced by any PABP. Whereas UTI after bladder catheter duration shorter than two days occurred in 26.0% (without PABP), 29.9% (Metronidazole) respectively 18.2% (Doxycyclin; p greater than 0.05), the rates after a longer stay were 69.6%, 76.8% and 65.7%. It is concluded, that the febrile standard morbidity and pelvic infections were reduced as well as the rate of necessary antibiotics therapies by a prophylactic application of Doxycyclin, but not with Metronidazole. The incidence of postoperative UTI was not reduced by prophylaxis.

[Detection of steroid hormone receptors in patients with mastopathies and fibroadenomas].

Estradiol and progesterone receptors have been determined in 74 women with mastopathy and 33 women with fibroadenomas in the period from 1986-1988. Mastopathy was classified according to Prechtel (stage I-III). A negative steroid-hormon-receptorstatus (less than 10 fmol/mg protein) was predominant in the group with Prechtel I and II. In our study the progesterone receptor was detectable more frequently than the estradiol receptor in patients with mastopathy or fibroadenomas.

[Effectiveness of perioperative preventive use of antibiotics with ampicillin/gentamycin or cefotiam in abdominal cesarean section].

To prove the effectiveness of perioperative antibiotics prophylaxis (pabp) in prevention of postoperative infections after caesarean section the efficiency of a Ampicillin/Gentamycin combination was compared with that of Cefotiam (Halospor). The infectious morbidity after both prophylactic antibiotic regimes was compared to that of an untreated sectioned group. The feverish standard morbidity (fsm) of all caesarean sections (between 1. 7. 1986-30. 6. 1988; n = 354) decreased by selective pabp from 25.3% to 16.7%. Compared with untreated patients (no infectional risk; fsm: 22.4%) the infectional morbidity after caesarean section was reduced by Ampicillin/Gentamycin (fsm: 10.0%; p less than 0.05) and also by Halospor (fsm: 13.7; p greater than 0.05). Uterine (14.5%) and urinary tract infections (6.6%) were the most frequent causes of fever during puerperium. After pabp the frequency of this causes decreased evidently. The rate of postoperative antibiotics therapies went down from 26.3% without pabp to 13.3% (p less than 0.05) with Ampicillin/Gentamycin respectively 13.7% (p less than 0.05) with Cefotiam. It is concluded, that the feverish standard morbidity was reduced by a selective papb both with Ampicillin/Gentamycin and Halospor. As a consequence of our results we are performing the pabp with a single dose of 2 g Halospor combined with 0.5 g Metronidazol in all sections generally.

Effect of chronic hypoxia on 45Ca and 86Rb uptake in aortic smooth muscle from spontaneously hypertensive rats (SHR).

The effect of chronic hypobaric hypoxia equivalent to a simulated high altitude of 4000 m was investigated on the Ca2+ and Rb+ uptake in vascular smooth muscle. The decline in systolic blood pressure in SHR due to hypoxia was associated with a significant decrease in 45Ca uptake and ouabain-insensitive 86Rb uptake as well as the tissue Ca2+ and K+ content. It seems likely that the reduction of the higher vascular tone in SHR by chronic hypoxia is due to the alteration of the transmembrane ion fluxes in the vascular smooth muscle cells. However, the meaning and the nature of the active and passive ion fluxes involved in this problem remains to be clarified.
