Compression osteotomy of the tibia.

Compression osteotomy of the tibia is to allow knee motion postoperatively without case immobilization. Rigid internal stabilization by axial compression and tension band wiring enhances union. The rigid internal stabilization prevents loss of position of the osteotomy, often seen in other techniques. The benchmark for this osteotomy operation is no fractures of the osteotomy fragments, no position loss, no thromboemboli, and no deep wound infections.

[The new rotatory hip-joint endoprosthesis according to the unit construction system after Weber. Experience and results of follow-up checks after five years (author's transl)].

The new rotatory hip-joint total prosthesis after Weber with rotating metal head and polyethylene socket has yielded a very high percentage of good results to date, according to experience collected in the course of five years and a comprehensive followup study. It was possible to prove that the metallic rotatory joint does not entail any disadvantages, and that it is extremely resistant to wear. The almost complete absence of shaft fractures is remarkable (two cases after 2,500 operations) as well as the low incidence of loosening after primary operation. Complications occur much more frequently following exchange operations. The unit construction principle on which this type of prosthesis is based, offers great advantages in respect of surgical technique. It appears that the rotatory prosthesis is superior to other concepts with regard to wear and to stability of fixation.

[Conservative versus surgical treatment of spinal fractures].

Not only fractures of joints and shafts may benefit from internal fixation, but also certain fractures of the spine. Stable spine fractures still are suitable for closed treatment. Instable fractures, however especially those with neurological disorders, can be much improved in their results by adequate surgery, by open reduction, rigid fixation and decompression of the medullary canal in case of paraplegia. As simple as closed treatment of the stable fractures is, as difficult is surgery of instable fractures. For all the different types of instable fractures and different types of compression syndromes, individual surgical approaches, decompression and fixation techniques are to be familiar to the surgeon. The basics of such individualized treatment of fractures of the spine are described as it has become standard base on an experience with 750 fractures of the spine, 90 of them having had surgery.

Fibrous interposition causing valgus deformity after fracture of the upper tibial metaphysis in children.

The development of genu valgum in a child after a fracture of the upper metaphysis of the tibia with a medial gap is due to the interposition of a flap of fibrous tissue consisting of pes anserinus and periosteum avulsed from the lower fragment. The ensuing biomechanical disturbance induces bowing of the shaft and asymmetrical growth at both ends of the bone. Four cases of established deformity are reported, together with two cases of fresh fracture successfully treated by surgical clearance of the fibrous tissue from the gap.

[Assorted aluminium-oxide ceramic heads for prostheses of the hip-joint. A preliminary communication (author's transl)].

In the search for even more hard-wearing prostheses of the hip, a rotating total- and femoral head prosthesis and a total- and femoral head prosthesis with fixed detachable heads are described, the head of which is made from highly polished aluminium oxide ceramic, Biolox. For total prostheses the excellent gliding features between ceramic head and polyethylen acetabulum are being used. They will also permit return to proved metal-polyethylen prostheses, should the ceramic head fracture. The fracture prosthesis provides both such an exchange and conversion into a total prosthesis. Extending the present programme of rotation-prostheses to ceramic heads will require a wide assortment of heads. This will mean not only wide adaptability but also the need for detailed knowledge of this assortment, the use of which demand thorough preparation of the surgeon. In order to collect material of the use of these prostheses, the authors are very keen to hear of drawbacks- and without reticence-as soon as possible. Publication of our own experience will follow this preliminary communication in due course.

[Treatment of juvenile fractures: general introduction (author's transl)].

As fractures sustained during childhood are "different", the treatment for them is also different than that for fractures in adults. In general the treatment is conservative. A childhood fracture has in its favor the enormous potential of the growing skeleton for healing and correction, while against it is the possibility of impaired growth following certain injuries in growth areas. There are quite specific indications for operative treatment; in other words, some fractures require internal fixation, e.g. genuine epiphyseal fracture, fracture of the neck of the femur, displacement of the head of the radius.

[Correction of unstable osteosynthesis following intertrochanteric osteotomy].

When a tension-wire osteosynthesis with right angle plate after intertrochanteric osteotomy breaks down, one may achieve an undminished result and sufficient stability using the same principle -tension wiring -in another way. Improvements in the AO osteosynthesis-material might further raise the firmness of tension.

[Magneto-implants in surgery of the motorsystem. A project (author's transl)].

The high quality material of permanent magnetos permits various applications to the surgery of the motor system. When designing artificial joints, for certain osteosyntheses and osteotomies, for the stimulation of growth at the epiphyses and in orthopedic techniques. Encouraging attempts at "magneto-medicine" exist in ophthalmology, urology and general surgery. We propose to test permanent magnets for attraction and separation in orthopedic and traumatic surgery.
