The authors review some of the factors that influence the perception of pulmonary lesions: the observer and his environment, the film or other image-recording and display media, the lesion and its qualities, and the technique or modality of examination used.

To determine the incidence and distribution of dorsal defect of the patella, the radiographs of 2,349 knees in 1,192 consecutive patients were evaluated. The defect was present in about 1% of the population. It may be found at any age from the preadolescent to the mature adult. Females were more commonly affected than males. The test population group was validated by concurrent tabulation and comparison with other series of two other bening lesions occurring at the knee: fabella and multipartite patella.

Four cases are reported, along with a review of the literature, to demonstrate the radiographic and clinial spectrum of bilateral anteromedial defect of the diaphragm in infants and children. Bilateral anteromedial defect of the diaphragm produces a characteristic radiographic pattern on herniation of abdominal structures through a single midline opening. The usually solid herniated structures elevate the heart and thymus producing a three-tiered snowman appearance. The lateral chest view substantiates the anterior location differentiating anteromedial herniation from eventration, partial or complete. Diagnosis can be confirmed by pneumoperitoneography but the high incidence of liver herniation allows less invasive determination by radionuclide scanning. While many patients will be asymptomatic, clinical symptoms can be severe due to pulmonary compression and hypoplasia or associated cardiovascular abnormalities.
A test series of chest radiographs containing pseudonodules was used to establish baseline sensitivity, specificity, and accuracy rates for individual observers. This experiment studies the effect of fatigue (as subjectively measured) and alcohol (as measured by blood alcohol levels) on observer perception and performance. The results indicate that the fatigue encountered in the course of ordinary professional activities has no significant effect on diagnostic accuracy. Modest quantities of alcohol may adversely influence diagnostic sensitivity, but the effect is unpredictable. Subjective estimates of the influence of fatigue or alcohol on performance by individual observers are frequently erroneous.
Scattered radiation doses to the eyes, thyroid and gonads of infants and children undergoing EMI head scans have been measured with thermoluminescent dosimeters. An average skin dose of 1.3 rad per scan was measured. Results are reported relative to incident skin dose and indicate that it is likely that there is no somatic or genetic hazard due to scattered radiation during an EMI head scan.
