[Classification of laryngeal papillomas and papillomatosis (author's transl)].

The clinical behavior of laryngeal papillomas is discussed by literature and by own 130 cases. Laryngeal papillomatosis is an epithelial proliferative lesion. Without prejudice to viral aetiology it can be interpreted in some cases as a dysplastic epithelial Tumor with the risk of cellular change to carcinoma. A histologic and cytologic classification of laryngeal dysplastic papilloma is described. The basic knowledge has been gained by the experiences in the diagnosis of oral-carcinom, portio-carcinoma and larynx-carcinoma. This classification is used for 48 selected cases of multiple and recurrence laryngeal papillomatosis in which sometimes also elder histological preparations were still available. The results and clinical value are discussed.

[A contribution to diagnosis, specificity and treatment of rhinoscleroma (author's transl)].

As a disease with a peculiar geographical distribution rhinoscleroma is a kind of chronic inflammation which may cause some diagnostic difficulties in non-endemic regions. In general, the diagnosis based on histological features and the microbiological identification of the causative agent. In case where a definite microbiological identification could not be achieved the diagnosis should be expressed in the following terms: "rhinosclerotic granuloma with features of specificity". The diagnosis is facilitated by the aid of cytological methods which are easy to perform and do not hurt the patient. The characteristic cells of the Mikulicz type may be observed in the smear. This paper presents the case of a 25-year old man with a rhinosclerotic granuloma of the larynx which caused dyspnea and aphonia. The diagnostic procedure was done by cytological, histological and ultrastructural methods. The patient was treated by surgery and by applications of antibiotics.

[Chemical preserved human trachea as prosthesis in covering tracheal defects--first experiences (author's transl)].

Basing upon animal experiments at the first time in two cases parts of a chemically conserved trachea were used for closing tracheal defects in reconstructive procedures. After layering a silastic foil on mucosal surface of the conservated tracheal piece in a first step this prepared transplant is implanted beneath the tracheal defect subcutaneously. Hitherto after 2 vs 3 months in a second step the paratracheal preimplants, now vascularized, were turned about the defect like a pedicle flap. Both transplants healed in the tracheal defect without disturbances. The new inner surface of the trachea now after an observation up to 6 months has remained plain without any granulations.

[Our experiences about the value of laryngeal cytology (author's transl)].

Cytology is a harmless and an important examination method in laryngeal cytology. Cytology controlled by biopsy and histology can lead with increasing experiences to diagnostic consequences. In a number of our cases cytology led to an early laryngofissur. In all these cases histology had justified the procedure.

[Laryngeal leukoplakias (author's transl)].

A histologic and cytologic classification of laryngeal dysplastic leukoplakias is described. The basic knowledge has been gained by the experience in the diagnosis of portio-carcinoma. Histologic and cytologic illustrations of different dysplastic stages are shown. The final classification including the clinical description is achieved by cytology and histology.

[Critical comments on the theory and practice of behavior therapy from the viewpoint of Marxist-Leninist philosophy].

By analyzing previous critical studies of the theoretical and methodological positions of behavioral therapy, which were conducted on Marxist-Leninist principles, the authors discuss the complex relationship of one-sided philosophical assumptions, simplifying elements of psychological theories, and radional moments of successful practical application in certain fields. Both critical delimitation from untenable theoretical interpretations and the inclusion of rational forms of therapy in a complex system of psychotherapy are considered essential.

[The anatomy of the laryngeal ventricle and its importance in carcinoma of the vocal cords (author's transl)].

An anatomical study was made of the sinus of Morgagni (laryngeal ventricle) in 10 normal larynges and 15 larynges with vocal cord carcinoma. The most striking difference in those patients with malignancy was the relatively greater goblet cell population found. It is assumed that these anatomical changes reflect a disturbed function of the laryngeal ventricle. The epithelial metaplasia and the high goblet cell count probably indicate an increased risk of vocal cord carcinoma.

[Improved differentiation of suspected chronic laryngeal lesions by highly dosed antibiotic therapy for a short time and comparing cytology (author's transl)].

The valuation of chronic laryngeal lesions is eased by a combination of a highly dosed antibiotic therapy for a short time and observation of the therapy procedure by cytology. The cytological estimated dyscaryoseindex gives reference if suspicion on a malignant lesion can be kept in mind. Above this the proceeding enables us for exact and useful biopsies respectively for primary diagnostic and therapeutic operations.

[Leukoplakias of the larynx (i. clinical and histological classification) (author's transl)].

A clinical and histological classification of leukoplakias of the larynx is described. Basis for this classification is the prognostically important systematical, histologic and cytologic knowledge gained in gynecology. The early clinical diagnosis limited to a macroscopic description of leukoplakias is completed by a prognostic attribute won by cytology and/or histology. Thus the final diagnosis of precancerous lesions of the larynx is classified into leukoplakias with low-graded dysplasias, middle-graded dysplasia, highgraded dysplasia and atypical epithelium. The old nomenclature regards dysplasias as facultative precancerous lesions and the atypical epithelium as an obligatory precancerous lesion.

Group B streptococci in neonatal deaths.

Clinical microbiologists are reminded to look for group B streptococci in neonatal infants. We report four deaths, one with meningitis, occurring among 14 such infected newborn infants at our institution; of the nursing staff, 31% were also carriers of group B streptococci.

Sleep-related growth hormone secretion is persistently suppressed in women with recurrent depression: a preliminary longitudinal analysis.

Growth hormone secretion was monitored during sleep in a group of 43 women with recurrent major depression who were participating in a 3-year maintenance therapy program. Patients were studied before acute treatment, after complete remission, and at 3-month intervals during maintenance treatment and the data generated were compared to those obtained in a control group of 14 age-matched healthy women studied once under identical conditions. When compared to the control group, the depressed patients secreted significantly less growth hormone before treatment. This reduction in growth hormone secretion, which was confined to the first half of the sleep period, persisted across the length of the maintenance study regardless of whether the subjects completed three years of therapy or experienced a recurrence.

Insulin-like growth factor 1 and growth hormone binding protein in depression: a preliminary communication.

This study was undertaken in order to advance our understanding of the distal growth hormone axis in depression. Insulin-like growth factor 1 (IGF-1) and growth hormone binding protein (GHBP) were measured in a group of 19 depressed women and a group of 16 healthy women. Using a generalized linear model, IGF-1 levels were negatively correlated with age (p = 0.0001), influenced by menstrual phase (p = 0.016), and significantly increased in the depressed group (p = 0.02). Using the same type of analysis, GHBP was significantly related to menstrual phase (p = 0.0001) and body mass index (p = 0.0001), but was not significantly different in patients and controls. IGF-1 and GHBP were positively correlated among healthy subjects (r = 0.46, p = 0.08), but not among depressed patients (r = -0.16, p = 0.51), although these correlation coefficients were not statistically significantly different from each other. These findings confirm the importance of several physiological factors in the regulation of IGF-1 and GHBP, and suggest that depression further influences this regulation.
