[Treatment of the dry eye with vitamin A acid--an impression cytology controlled study].

To test the efficacy of topically applied tretinoin eyedrops (0.01%, twice daily, in a solution of peanut oil) in the treatment of squamous metaplasia of the conjunctival epithelium associated with dry-eye, the authors evaluated the impression cytology (IPC) of a total of 31 patients. The cytological findings were classified into four different stages with an overall range from 0 to 9 points. The average period of treatment was 12 weeks. The patients were classified into three groups: (1) Those with mucin deficiency (Mucin less than - greater than; n = 19), (2) those with Keratoconjunctivitis sicca (KSC; n = 6), and (3) a control group in whom pure peanut oil was used (n = 6). The IPC showed a considerable improvement of squamous metaplasia in the mucin less than - greater than group, by an average of 4.3 points (from 6.2 points before treatment to 1.9 points). No changes were seen in the IPC of the KCS patients or the control group. The results of this study show a good effect of tretinoin eyedrops in patients with a significant decrease in or loss of goblet cells seen in the IPC, a decreased break-up time and with no severe aqueous deficiency of tear film. These patients seem to have a "primary" mucin deficiency induced by the decrease in or loss of goblet cells. There is no indication for tretinoin treatment in patients with aqueous deficiency involving major or accessory lacrimal glands. The results of other previous and recent studies on tretinoin treatment are discussed.

Regression of oral hairy leukoplakia after orally administered acyclovir therapy.

To define the role of Epstein-Barr virus (EBV) in the pathogenesis of oral hairy leukoplakia, 13 human immunodeficiency virus-seropositive men with clinical and histologic evidence of oral hairy leukoplakia were enrolled in an open-label trial of orally administered acyclovir therapy (3.2 g/d for 20 days). Of six patients who received therapy, five exhibited clinical regression. Once therapy was discontinued, recurrences occurred in all responders. Among seven patients who refused therapy, no spontaneous remissions occurred. Before therapy, EBV replication within the leukoplakia was demonstrated by immunofluorescence tissue staining or electron microscopy in five patients who were studied. Human papillomavirus was not detected by immunocytochemistry or electron microscopy from tissue specimens of six patients. After therapy, biopsy specimens from two patients with complete responses revealed a normalization of histologic abnormalities and an inability to detect EBV in previously involved mucosa by immunofluorescence or in situ DNA hybridization assays. It was concluded that EBV replication within the epithelial cells of the tongue is necessary for the development of oral hairy leukoplakia.

Complex repetitive discharges in the iliopsoas muscle.

A prospective electromyographic investigation of lower limb muscles in patients with different neurogenic disorders showed that complex repetitive discharges (CRD) were observed predominantly and often only in the iliopsoas muscle. Analysis of the EMG findings in acute and chronic lesions shows that CRD are a feature of a chronic proximal motor axon lesion. Furthermore, the frequently focal occurrence of CRD in the iliopsoas muscle in clinically distal diabetic polyneuropathies suggests that this muscle and its nerves represent a locus minoris resistentiae.

Transcranial Doppler ultrasonography of carotid-basilar collateral circulation in subclavian steal.

The combination of a carotid-basilar and a vertebro-vertebral collateral circulation was verified directly in a patient with a complete subclavian steal by means of transcranial Doppler ultrasonography. The patient showed permanently reversed blood flow in the basilar artery. The subclavian steal influenced the hemodynamics of the circle of Willis at rest and during functional tests of the collaterally supplied arm. Our investigation provides the first direct experimental evidence of increased blood flow velocity in the carotid artery after decompressing the collaterally supplied arm.

[Thoraco-abdominal manifestation of diabetic neuropathy].

The differential diagnostic importance of the thoraco-abdominal manifestation of diabetic neuropathy, with special reference to the neurophysiological findings is underlined. The cases of six patients are described who suffered from severe, burning pain of varying location (area of the lower thoracic aperture, lower abdomen, scapulae, back). In all six patients pathological spontaneous activity of different location and intensity could be found bilaterally in the paravertebral muscles. Their segmental location correlated with the individual experience of pain.

The continuing problem of youthful solvent abuse in New York State.

The major finding in the comparison of surveys of New York State secondary school students is the increasing use of solvents in this population over time. What in 1974-75 seemed to be limited use, by 1983 was found to be of widespread use. Very much like the survey of 1974-75 and the findings in the literature, age of first use together with the recentness of use appears to cluster in the preteens or the early teen years and tapers off during the teen years. Of interest is the fact that, along with the general upward rate of use over the surveys, those 18 years or older also show an upward trend. Some evidence in the recent literature indicates that adults, too, are found to have a problem with inhalants. Similar to the 1974-75 survey, but unlike findings in the literature, males and females continue to show similar rates of solvent use. Although males do generally surpass females in these use rates, the differences are usually not significant. In light of the literature on solvent abuse among Hispanic youth, the 1978 and 1983 surveys indicated mixed findings. Hispanic students in New York City consistently showed intermediate levels of solvent use; whereas, in 1983, Hispanic students residing in areas of the state outside of New York City showed excessive rates of solvent use. To the extent that the problem of acculturation among poor Hispanic youth may contribute to solvent abuse, an explanation may be found in the contrasts between living in New York City and living in the rest of the State. In New York City, which has an extremely large Hispanic population (about 20 percent of the population), feelings of isolation and cultural distance may not be so profound. In the rest of the state, where the proportion of Hispanics is quite small (about 2 percent of the population), these cultural problems may be more of a factor. Reasons, however, for the extraordinary increase in solvent use among those Hispanic youth, specifically between 1978 and 1983, are difficult to offer. Again, very similar to the 1974-75 findings, the 1978 and 1983 surveys found a strong relationship between solvent use and poor academic performance. Furthermore, the early findings relating to solvent use to a lack of family cohesion was supported by the 1978 survey where comparable information was gathered. The pattern of drug-using or polydrug use found among solvent users in the 1974-75 survey was underscored by the findings in the subsequent surveys.(ABSTRACT TRUNCATED AT 400 WORDS)

Mast cells in sun-exposed and non-sun-exposed skin. An autopsy study.

One hundred ten skin biopsies were taken from 55 consecutive autopsies to evaluate the number and location of mast cells in the dermis. For each autopsy, one biopsy was taken from the V of the neck (sun-exposed area) and the other from the upper thigh (non-sun-exposed area). Normal-appearing skin was biopsied. There were 28 men and 27 women ranging in age from 16 to 94 years. Fifty-three patients were Caucasian and two were Negro. Mast cells were counted in 10 random high-power fields in the papillary dermis only. The average number of mast cells per high-power field (X 400) in sun-exposed skin for both men and women was 8.19 (65/mm2 or 13,000/mm3) with one standard deviation of 4.08, while that of non-sun-exposed skin was 7.52 (60/mm2 or 11,900/mm3) with one standard deviation of 3.62. The difference between the number of mast cells in sun-exposed and non-sun-exposed skin was not statistically significant. In addition, no statistically significant differences were observed for the average number of mast cells per high-power field in regard to sex or the presence of malignancies.

Metabolism and placental transfer of 125I-proinsulin and 125I-tyrosylated C-peptide in the pregnant rhesus monkey.

125I-Proinsulin or 125I-tyrosylated-C-peptide (125I-tyr-CP) was administered to pregnant Rhesus monkeys by bolus followed by constant infusion to examine placental transfer of these peptides. At the end of each infusion, fetuses were exsanguinated in situ via the umbilical vein. The bolus-constant infusion technique produced a steady state in maternal plasma of immunoprecipitable label, measured using excess insulin or C-peptide antiserum. In animals infused with 125I-proinsulin, analysis of umbilical venous plasma revealed no apparent transfer to the fetus of immunoprecipitable label. In animals infused with 125I-tyr-CP, 3-13% of the umbilical venous plasma radioactivity was immunoprecipitable, representing 1.4-5.8% of the immunoprecipitable radioactivity in maternal plasma at delivery. Gel filtration chromatography of umbilical venous plasma revealed that the immunoprecipitated moiety was a fragment of 125I-tyr-CP. Analysis of maternal plasma showed that the predominant peak of radioactivity represented intact C-peptide. A peak corresponding to the fetal immunoprecipitable peak was also present. Analysis of simultaneous maternal arterial and uterine vein plasma samples showed that degradation of 125I-tyr-CP occurred across the uterus. Studies in one nonpregnant and three postpartum animals indicated that pregnancy increased the rate of metabolism of 125I-tyr-CP. When 125I-tyr-CP was incubated with trophoblastic cells in culture, degradation to a species corresponding on gel filtration to the immunoprecipitable fetal metabolite was found. We conclude that proinsulin, like insulin, does not traverse the placenta. Immunoreactive fragments of C-peptide do cross, however, and pregnancy alters the metabolism of 125I-tyr-CP, probably owing to placental degradation.

Evidence for intrinsic regulation of met-enkephalin-immunoreactivity in gastroenteropancreatic tissues of the rat.

In a study whether gastrointestinal endogenous opioids can be modified by vagal denervation or by pharmacological application of an opiate, we examined met-enkephalin-immunoreactivity in gastrointestinal tissue in rats with and without truncal vagotomy and with and without subcutaneously implanted morphine pellets. The immunoreactivity of the tissue extracts gave dose-response lines in the radioimmunoassay for met-enkephalin which were near parallel to that for the standard. On Sephadex chromatography the met-enkephalin immunoreactivity eluted at a position similar to synthetic met-enkephalin. Tissue concentration of met-enkephalin immunoreactivity was not significantly different from the respective control after vagotomy and after morphine treatment. Total gastric met-enkephalin immunoreactive content increased significantly after vagotomy in line with gastric hypertrophy occurring after vagotomy without a drainage procedure. From these results it is concluded that met-enkephalin immunoreactivity in the rat gastrointestinal tract is regulated intrinsically, it is neither altered by vagal denervation nor by exogenous opiate administration.

Ureteral stone management: emerging concepts.

A total of 206 procedures was performed on 193 patients with ureteral calculi. The success rate was 62 per cent for stones proximal to the pelvic brim and 94 per cent for stones distal to the pelvic brim (87 per cent success over-all). Of the procedures 61 were done for fragments lodged in the ureter following extracorporeal shock wave lithotripsy. Ureteroscopy was done in 147 cases. Our current management concepts (including extracorporeal shock wave lithotripsy) for proximal and distal ureteral calculi are presented.

Bowen's disease, Paget's disease, and malignant melanoma in situ.

The histologic differential diagnosis of skin lesions characterized by large, atypical, clear cells in the epidermis includes Bowen's disease, Paget's disease (mammary and extramammary), malignant melanoma in situ (pagetoid precancerous melanosis), mycosis fungoides, Spitz nevus, and artifact. Our experience with these lesions indicates that these diseases can be differentiated immunohistologically by the standard peroxidase-antiperoxidase technique, using antibodies directed against keratin (KER), carcinoembryonic antigen (CEA), and S-100 protein (S-100). Based on a study of 11 cases of Bowen's disease, eight cases of Paget's disease, and nine cases of malignant melanoma in situ, we conclude that the atypical clear cells of Bowen's disease stain only with antibodies to KER; those of Paget's disease, exclusively with antibodies to CEA; and those of malignant melanoma in situ, with antibodies to S-100. Additionally, we report a case in which clinical and histologic findings suggested Bowen's disease, but immunohistologic findings supported the diagnosis of Paget's disease.

Use of biosynthetic human C-peptide in the measurement of insulin secretion rates in normal volunteers and type I diabetic patients.

We undertook this study to examine the accuracy of plasma C-peptide as a marker of insulin secretion. The peripheral kinetics of biosynthetic human C-peptide (BHCP) were studied in 10 normal volunteers and 7 insulin-dependent diabetic patients. Each subject received intravenous bolus injections of BHCP as well as constant and variable rate infusions. After intravenous bolus injections the metabolic clearance rate of BHCP (3.8 +/- 0.1 ml/kg per min, mean +/- SEM) was not significantly different from the value obtained during its constant intravenous infusion (3.9 +/- 0.1 ml/kg per min). The metabolic clearance rate of C-peptide measured during steady state intravenous infusions was constant over a wide concentration range. During experiments in which BHCP was infused at a variable rate, the peripheral concentration of C-peptide did not change in proportion to the infusion rate. Thus, the infusion rate of BHCP could not be calculated accurately as the product of the C-peptide concentration and metabolic clearance rate. However, the non-steady infusion rate of BHCP could be accurately calculated from peripheral C-peptide concentrations using a two-compartment mathematical model when model parameters were derived from the C-peptide decay curve in each subject. Application of this model to predict constant infusions of C-peptide from peripheral C-peptide concentrations resulted in model generated estimates of the C-peptide infusion rate that were 101.5 +/- 3.4% and 100.4 +/- 2.8% of low and high dose rates, respectively. Estimates of the total quantity of C-peptide infused at a variable rate over 240 min based on the two-compartment model represented 104.6 +/- 2.4% of the amount actually infused. Application of this approach to clinical studies will allow the secretion rate of insulin to be estimated with considerable accuracy. The insulin secretion rate in normal subjects after an overnight fast was 89.1 pmol/min, which corresponds with a basal 24-h secretion of 18.6 U.

Substance use among females in New York State: catching up with the males.

Epidemiologic surveys of the New York State population over the past several years show substance use rates for females that are approaching the higher use rates for males. Surveys of the secondary school population show similar rates of use for girls and boys over time as well as a similar intensity of involvement. Surveys of adults show a dramatic increase of use for females, ages 18 to 34 years, in recent years, and a more modest increase for females, ages 35 years and older. An analysis of substance use by sex, age, marital status and employment identifies disparate rates of use among subgroups. For instance, among younger adults, single full-time employed women have considerably higher rates of use than housewives for illicit substances as well as psychoactive prescription drugs used medically as well as nonmedically.

The limitations to and valid use of C-peptide as a marker of the secretion of insulin.

The accuracy with which the secretion rate of insulin can be calculated from peripheral concentrations of C-peptide was investigated in conscious mongrel dogs. Biosynthetic human C-peptide and insulin were infused intraportally and their concentrations measured in the femoral artery. During steady-state infusions of C-peptide, the peripheral concentration changed in proportion to the infusion rate and the metabolic clearance rate (5.2 +/- 0.3 ml/kg/min) remained constant over a wide range of plasma concentrations. Application of a two-compartment mathematical model, in which the model parameters were estimated from analysis of C-peptide decay curves after intravenous bolus injections, allowed the intraportal infusion rate of C-peptide to be derived from peripheral C-peptide concentrations, even under non-steady-state conditions. Estimates of the intraportal infusion rate based on this model were 102.4 +/- 2.6% of the actual infusion rate as it was increasing and 102.3 +/- 5.5% of this rate as it was falling. The peripheral C-peptide: insulin molar ratio was influenced by the rate at which equimolar intraportal infusions of C-peptide and insulin were changed. The baseline C-peptide: insulin molar ratio (4.1 +/- 0.9) increased to peak values of 8.2 +/- 0.6, 10.3 +/- 2.0, and 14.9 +/- 1.3 when the infusion rate was increased and then decreased rapidly. Peak values of only 5.7 +/- 1.2 were found if the intraportal infusion rate was changed slowly.(ABSTRACT TRUNCATED AT 250 WORDS)
