Successful irrigated-tip catheter ablation of atrial flutter resistant to conventional radiofrequency ablation.

BACKGROUND: Catheter ablation of typical right atrial flutter is now widely performed. The best end point has been demonstrated to be bidirectional isthmus block. We investigated the use of irrigated-tip catheters in a small subset of patients who failed isthmus ablation with conventional radiofrequency (RF) ablation. METHODS AND RESULTS: Of 170 patients referred for ablation of common atrial flutter, conventional ablation of the cavotricuspid isthmus with >21 applications failed to create a bidirectional block in 13 (7.6%). An irrigated-tip catheter ablation was performed on identified gaps in the ablation line according to a protocol found to be safe in animals: a moderate flow rate of 17 mL/min and temperature-controlled (target, 50 degrees C) RF delivery with a power limit of 50 W. Bidirectional isthmus block was achieved in 12 patients by use of a mean delivered power of 40+/-6 W with a single application in 6 patients and 2 to 6 applications in the other 6. No side effects occurred during or after the procedure. CONCLUSIONS: Irrigated-tip catheter ablation is safe and effective for achieving cavotricuspid isthmus block when conventional RF energy has failed.

Altered expression of microtubule-associated protein 1B in cerebral cortical structures of pentylenetetrazole-treated rats.

Using Northern blot, immunoblotting, immunocytochemistry, and in situ hybridization, we show that a single administration of the convulsant pentylenetetrazole leads to robust, long-term changes in microtubule-associated protein 1B and its mRNA, in the adult rat brain. The first increases in MAP1B mRNA were detected at 15 hr following pentylenetetrazole administration in the temporal (Te2) and perirhinal cortex followed by increases in microtubule-associated protein 1B immunoreactivity at 72 hr postseizure. In contrast, the levels of microtubule-associated protein 1B mRNA and protein in layers I-II of the retrosplenial and parietal cortex (Par2) declined visibly by 24 hr and 72 h, respectively, post-seizure. The changes included loss of staining in layers I-II and development of structures resembling "strings-of-beads" along the fibers of projection neurons of layer V. The levels of microtubule-associated protein 1B mRNA in the entorhinal cortex peaked at later times (72 h), especially in layers II-III, and returned to control levels by 10 days. Whereas the levels of microtubule-associated protein 1B immunoreactivity in the retrosplenial and parietal cortex recovered by 5-10 days, it persisted at high levels through day 35 in layer V of the temporal cortex (Te2), layers II-III of the perirhinal cortex and layers I-II of the lateral entorhinal cortex. These results indicate that seizure activity leads to long-term upregulation of genes coding for structural elements that are characteristic of the immature brain such as microtubule-associated protein 1B.

Increased expression of NADH-ubiquinone oxidoreductase chain 2 (ND2) in preimplantation rabbit embryos cultured with 20% oxygen concentration.

In vitro culture of mammalian preimplantation embryos is associated with various developmental disorders such as retardation in development and cell damage. The molecular mechanisms underlying these processes are poorly understood. One of the possible reasons may be the unphysiologically high oxygen concentration used for culture. Four-day-old rabbit blastocysts were cultured with 5% O2 (physiologic oxygen concentration) or 20% O2 (usually used for in vitro culture) for 4 hr. Differences in gene expression were analysed by differential display reverse-transcriptase polymerase chain reaction (DD RT-PCR). Thirty-two differentially expressed RNA bands were found. Two of them revealed a high sequence homology to the equine NADH-ubiquinone oxidoreductase chain 2 (ND2), a subunit of complex I of the respiratory chain. mRNA expression of ND2 was increased in blastocysts cultured with the higher oxygen concentration. Increased expression of ND2 was confirmed by semiquantitative and semiquantitative competitive RT-PCR.

DNA ploidy abnormalities in rabbit preimplantation embryos are not increased by conditions associated with in vitro culture.

Possible adverse effects of in vitro culture-associated physical factors were studied in 3- and 4-day-old rabbit embryos. Laboratory conditions were mimicked by exposure to visible light (320-740 nm, 1600 lx) or decreased temperature (22 +/- 1 degree C). Embryos were exposed for a 24-hr period followed by either immediate evaluation or an additional 24 hr of standard in vitro culture (darkness, 37 degrees C) and evaluation thereafter. Effects were assayed by cytophotometric measurement of the DNA content in Feulgen-stained cell nuclei and by cell number. The incidence of DNA aneuploid embryos and DNA aneuploid cell nuclei per embryo, as well as the average nuclear DNA content, was not significantly different between exposed embryos and controls. Both in vitro culture and reduced temperature caused a decrease in cell number. The temperature-induced cell number decrease was reversible within 24 hr after return to 37 degrees C. These results demonstrate that physical factors associated with in vitro culture do not increase DNA ploidy abnormalities in cultured preimplantation embryos.

Epidermal growth factor receptor and ligands in elongating bovine blastocysts.

Preimplantation development depends on multiple interactions between mother and embryo. The Epidermal Growth Factor Receptor (EGF-R) and its ligands are potential components of the embryo-maternal cross-talk: Employing RT-PCR, in situ hybridization, and immunohistochemistry, we investigated on mRNA and protein level the expression of EGF-R, Epidermal Growth Factor (EGF), Transforming Growth Factor alpha (TGF-alpha), and Heparin-binding EGF-like Growth Factor (HB-EGF) in spherical and elongating bovine blastocysts between day 13 and day 16 of gestation, and in endometrium at day 13 of gestation. EGF-R mRNA and protein were detected in trophoblast and endoderm cells of all blastocyst stages that were studied, and in luminal and some glandular epithelial cells of the endometrium at day 13. EGF protein was detected in both blastocysts and endometrial epithelium. TGF-alpha transcripts and protein were present in blastocysts prior to and after elongation and in uterine glandular and luminal epithelium at day 13 of gestation. HB-EGF mRNA and protein was shown in the endoderm, and the protein also was detected immunohistochemically in about 45% of the blastocysts. This presence of the EGF receptor-ligand system in the endometrium and the preimplantation embryo at the time of blastocyst elongation suggests an important role for these growth factors during bovine preimplantation development.

[Clinical application of transcranial color-coded duplex ultrasound for detection of intracranial aneurysms].

We investigated 88 Patients with a total of 102 angiographically diagnosed intracranial aneurysms by means of transcranial colour coded Duplex sonography (TCCD) during a time period of 15 months. Both the size and the localization of the aneurysms were determined. Seventy aneurysms (77%) with a diameter of 16 +/- 8 mm (6-55 mm) were detectable, with excellent visualization in 36 (42%), moderate visualization in 34 (40%), and no sufficient visualization in 16 (16%) aneurysms, respectively. In another 16 cases (16%) there was no sufficient vone window. Thrombotic material inside the aneurysm was detectable in 16/20 cases (75%), visualization of coil embolized aneurysms in 12/25 patients (48%). TCCD allows the follow up of cerebral aneurysms, with the detection of thrombosis and treatment effects after embolization. The method is not valid for the detection of intracranial aneurysms.

Effects of pre-cues on voluntary and reflexive saccade generation. I. Anti-cues for pro-saccades.

Experiments on visual attention have employed both physical cues and verbal instructions to enable subjects to allocate attention at a location that becomes relevant within a perceptual or motor task some time later (cue lead time, CLT). In this study we have used valid visual peripheral cues (CLT between 100 and 700 ms) to indicate the direction and location of the next saccade. A cue is considered valid or invalid if its meaning with respect to the next saccade is correct or incorrect. A cue is called an anti- or pro-cue if the side of its presentation is opposite to or the same as the direction of the saccade required on a given trial. Correspondingly, a saccade is called an anti- or pro-saccade if it is directed to the side opposite to or the same as the stimulus presentation. A condition in which the cue and the stimulus are presented on opposite sides provides a simple way of dissociating voluntary attention allocation from automatic orienting. This paper considers the anti-cue pro-saccade task: the subjects were instructed to use the cue to direct attention to the opposite side, i.e. the location, where on valid trials the saccade target would occur. In the companion paper we have used the same physical condition, but we have reversed the instructions as to saccade direction and we have reversed the meaning of the cue, i.e. we designed a pro-cue anti-saccade task. In this first paper, the saccadic reaction times (SRTs) of pro-saccades of five adult subjects were measured in the gap paradigm (fixation point offset precedes target onset by 200 ms). With a CLT of 100 ms, valid anti-cues reduced the number of express saccades (i.e. saccades with SRTs in the range 80-120 ms) significantly compared with the control values (no cues). Valid anti-cues with increasingly long CLTs (100-700 ms) resulted in an increasing incidence of anticipatory saccades and saccades with longer SRTs (more than 120 ms), while the frequency of express saccades remained below the control value. When cue and saccade target were dissociated in location or in both location and direction, the effects of the cueing revealed a much lower spatial selectivity as compared to the effects that have been described for voluntary attention allocation by means of central cues. The results suggest that voluntary allocation of attention and cue-induced automatic orienting not only have different time courses but also have opposite effects on the generation of express saccades, and different spatial selectivities. A possible neuronal basis of these results is discussed considering related findings from electrophysiological studies in monkeys.

Effects of pre-cues on voluntary and reflexive saccade generation. II. Pro-cues for anti-saccades.

The reaction times of saccades (SRT) to a suddenly presented visual stimulus (pro-saccade) can be decreased and a separate mode of express saccades can occur when a gap paradigm is used (i.e. fixation-point offset precedes target onset by 200 ms). A valid peripheral cue, presented briefly (100 ms) before target onset, has been found to facilitate the generation of saccades to the target, thereby increasing the frequency of express saccades and decreasing the mean latency. This facilitation occurs only for cues that correctly indicate the direction of the subsequent target presentation (valid cues). The present study investigates the effects of valid cues on SRTs and error rate in the anti-saccade task (saccades in the direction opposite to the stimulus) by systematically varying the cue lead time (CLT) and using the gap and overlap conditions, i.e. fixation point remains on throughout the trial. For a CLT of 100 ms, both reaction times and error rates were significantly increased. With increasing CLT (200-500 ms), both the reaction times of the anti-saccades and the error rates returned to approximately control level, with CLT more than 200 ms in both the gap and the overlap condition. Additional experiments using non-informative cues in the overlap task showed that the reaction times of correct anti-saccades and the error rate were decreased when cue and stimulus appeared at the same side. Analysis of the erratic pro-saccades revealed that almost all of them were corrected, i.e. they were followed by a second saccade towards the required location. It is found that the correction times were usually very short, with intersaccadic intervals between 0 and 150 ms. We suggest that the orienting mechanism, elicited by a transient peripheral cue, relates to the command and the decision to make a pro- rather than an anti-saccade. The cue elicits pro-orienting towards its position when a pro-saccade is required, and anti-orienting when an anti-saccade is required. The orienting effect is transient and decays with CLTs of more than 200 ms; this result holds for both anti-saccades and pro-saccades. Since subjects reported that they could not prevent the erratic pro-saccades or were often not aware of them, we conclude that this orienting mechanism occurs automatically, beyond voluntary control.

Supplements as a source of micronutrient intake in middle-aged men in southern Germany: results of the MONICA dietary survey 1994/95.

Survey data on contemporary supplement intake in Germany are scarce, and the contribution of supplements to nutrient intake is especially unclear. In the MONICA Project Augsburg a dietary survey using 7-day weighed records was carried out in a subsample of 607 eligible men aged 45-64 years from the city of Augsburg (participation 430 men = 71%) in 1994/95. Participants also reported their intake of supplements. The German national food data base BLS (version 2.1), which was used to code the records and calculate the nutrients, had to be complemented by nutrient data of supplements and fortified foods. On a group level the maximum mean percentage contribution of supplements to the intake of a micro nutrient was 4.0% (vitamin C). On an individual level, however, up to 4.0% of the men were found to receive more than 50% of their intake of selected micronutrients from supplements. It is, therefore, recommended to ask for supplement intake in future dietary surveys within Germany.

Characterization of ion currents elicited by a stream of fluid during spontaneous and ligand-induced chloride current oscillation in Xenopus laevis oocytes.

During Ca2+-activated C- current oscillations a mechanical deformation of the Xenopus laevis oocyte by a fluid stream evokes transient inward currents of high amplitude (stream evoked inward current, Ii,st). This current can be observed either in native or RNA-injected oocytes expressing ligand-controlled ion channels from rat brain. Ii,st reversed at the equilibrium potential of chloride and was blocked by 9-anthracene carboxylic acid (2 mM). Power spectral analysis of the oscillations did not reveal a correlation between the features of the oscillations and the amplitude of Ii,st. Antagonists of stretch-activated cation channels [gadolinium (100 microM) and lanthanum (1mM)] did not block Ii,st. Calcium channel blockers [cobalt and manganese (10 mM)] did not inhibited Ii,st and Ii,st could also be elicited in calcium-free medium. Preloading oocytes with pertussis toxin (PTX) for 17 h prevented current oscillations and Ii,st caffeine (10 mM), an antagonist of the liberation of calcium from intracellular stores, inhibited Ii,st. Our results proride evidence for modulation of the mechanosensitivity of chloride currents by activation of intracellular second messenger cascades.

The antisaccade: a review of basic research and clinical studies.

The ability to suppress reflexive responses in favor of voluntary motor acts is crucial for everyday life. Both abilities can be tested with an oculomotor task, the antisaccade task. This task requires subjects to suppress a reflexive prosaccade to a flashed visual stimulus and instead to generate a voluntary saccade to the opposite side. This article reviews what is currently known about the neural structures and processes which are involved in the performance of this task. Current data show that a variety of brain lesions, neurological diseases and psychiatric disorders result in errors, i.e. prosaccades towards the stimulus, in this task. Brain imaging studies have shown that a widely distributed cortical and subcortical network is active during the generation of antisaccades. These findings are discussed and the potential of the antisaccade task for diagnostic purposes is evaluated.

A chemical approach to systematically designate the pyranopterin centers of molybdenum and tungsten enzymes and synthetic models.

The recent growth in the chemistry of the oxo-molybdenum enzymes has demonstrated the need for developing systematic methods for naming and abbreviating the novel pterin cofactors that bind to the metal ion via the sulfur atoms of an ene-1,2-dithiolate moiety. Historically, the term "molybdopterin" was coined to designate a special pterin that binds molybdenum and the molybdenum-bound form was termed the "molybdenum cofactor". However, recent studies have demonstrated that this novel pterin also binds tungsten. Furthermore, considerable variation has been found in the pterin entity itself. Taken together, these facts show that molybdenum- and tungsten-containing enzymes possess a family of cofactors rather than a single "molybdenum cofactor". This article proposes a unified methodology for describing these cofactors and their metal-free pterin units in light of recent results from protein crystallography. The various numbering schemes that have been used for this heterocycle are considered, as well as the IUPAC rules which are currently being used for related tricyclic compounds. A unified methodology for uniquely designating and abbreviating each cofactor is proposed. The available chemical and spectroscopic information on the pyranopterin entities that are present in the molybdenum and tungsten enzymes, the precursors to these centers, and synthetic pyranopterins are in part the basis of the systematic names and simplifying abbreviations.

[Value of spirometry-gated high resolution computerized tomography of the lung during inspiration and expiration].

PURPOSE: To compare mean lung density (MLD) of paired inspiratory and expiratory thin-section CT scans acquired after patient instruction or using spirometric gating. MATERIALS AND METHODS: 21 patients (13 m, 8 f. median age 59 years, two with normal lung function, 15 with obstructive, 4 with restrictive impairment) underwent thin-section CT. Paired inspiratory and expiratory scans were performed in the upper, middle and lower lung fields. They were acquired after automatic patient instruction with constant intervals between instruction and scan. Spirometrically gated scans were acquired within 4 days at 80% and 20% of vital capacity (VC) which has been determined on the CT scanner in supine position directly prior to the examination using the built-in spirometric option. Semiautomatic segmentation of the lung was performed and MLD was calculated. VC of pulmonary function testing was available for correlation. RESULTS: Inspiratory MLD was -828 +/- 71 HU with spirometric gating and -818 +/- 87 HU without. Expiratory MLD was -750 +/- 119 HU (increase 78 +/- 58 HU) with spirometric gating and -748 +/- 119 HU (increase 70 +/- 46 HU) without. All differences were non-significant. Intraindividual differences were 10 +/- 21 HU for inspiration and 2 +/- 36 HU for expiration. VC under CT-conditions was significantly smaller than that known from pulmonary function tests (1.9 +/- 0.71 vs. 2.7 +/- 0.81, p < 0.005). CONCLUSION: Spirometric gating does not lead to different results for MLD as compared with acquisition of paired inspiratory and expiratory thin-section CT after automatic patient instruction.

Only a truncated epidermal growth factor receptor protein is present in porcine endometrium.

The epidermal growth factor receptor (EGF-R) is a 170-kDa transmembrane protein, of which a truncated 100-kDa form (trEGF-R) lacking the cytoplasmic and the transmembrane domains, and an oncogenic 68-kDa form (v-erb) lacking the extracellular domain have been described. The trEGF-R is secreted and not able to transmit signals into the cell. Growth factors of the EGF family have been shown in porcine uterine fluids and blastocyst. Employing differential immunohistochemistry, we found the extracellular domain, but not the cytoplasmic domain, of the EGF-R in porcine endometrium on Days 9-11 of pregnancy. Blastocysts from Days 9 to 11 post coitum (p.c.) were positive for both antibodies, indicating the presence of the full-size receptor. Western Blotting of endometrial protein resulted in a 100-kDa band, but no 170-kDa band. No evidence for a coexpression of the 170- or 68-kDa forms of the receptor was found. ELISA analysis demonstrated trEGF-R in porcine uterine flushings. We conclude that 1) the trEGF-R is the only EGF-R form present in porcine endometrium, and 2) growth factors of the EGF family in porcine uterine fluids can exert their function via the EGF-R only on blastocysts, not on the endometrium.

Organ failure, infection, and the systemic inflammatory response syndrome are associated with elevated levels of urinary intestinal fatty acid binding protein: study of 100 consecutive patients in a surgical intensive care unit.

BACKGROUND: Intestinal mucosal ischemia and subsequent barrier dysfunction have been related to the development of organ dysfunction and death in the critically ill. We hypothesized that urine concentrations of intestinal fatty acid binding protein (IFABP), a sensitive marker of intestinal ischemia, might predict the development of the systemic inflammatory response syndrome (SIRS) and organ dysfunction. METHODS: One hundred consecutive critically ill patients were prospectively studied for the development of infectious complications, organ dysfunction, and SIRS. Urine was collected daily for measurement of IFABP. RESULTS: A total of 58 males and 42 females (mean age, 56 years; range,16-85 years) were studied. Of these 100 patients, 40 patients developed complications and 5 patients developed SIRS. IFABP was significantly elevated in all patients with SIRS, and IFABP levels peaked an average of 1.4 days (range, 0-7 days) before the diagnosis of SIRS. CONCLUSION: Elevated concentrations of urine IFABP correlated with the clinical development of SIRS. Studies to assess the utility of IFABP as a predictor of organ dysfunction and SIRS in the critically ill are warranted.

CTLA4 codon 17 dimorphism in patients with rheumatoid arthritis.

The genetic susceptibility to rheumatoid arthritis is conferred by genes in the human leukocyte antigen (HLA) region on chromosome 6, but additional genes may be involved to determine disease susceptibility. We have studied the distribution of the CTLA4 exon 1 polymorphism (49 A/G) in rheumatoid arthritis. This dimorphism at codon 17 results in an amino acid exchange (Thr/Ala) in the leader peptide of the expressed protein and was analyzed by PCR, SSCP and RFLP in 258 Caucasian rheumatoid arthritis patients and 456 controls. Rheumatoid arthritis patients were characterized by a decreased frequency of homozygotes for the Thr-17 substitution (32% versus 39%) and an overrepresentation of patients heterozygous for the Thr/Ala substitution (54% versus 46%). Gene frequencies for the Ala/Thr substitution differed only marginally from controls. In contrast, analyses of the CTLA4 exon 1 polymorphism with respect to HLA-DRB1*04 revealed significantly more patients with Ala in the homozygous (19% versus 15% controls) or heterozygous state (54% versus 39% controls) and less homozygous for Thr (27% versus 46% controls), with a particular increase of Ala/Ala genotypes among rheumatoid arthritis patients carrying the HLA-DRB1*0401 subtype. Among HLA-DRB1*04 negative rheumatoid arthritis patients, we observed no difference between the allele frequencies of the Ala-17 or Thr-17 substitution.
