Results of conservative surgery for middle ear cholesteatoma.

A retrospective study was carried out to analyze treatment results for cholesteatoma at the University of Iowa Hospitals and Clinics. All patients undergoing primary surgical treatment from January 1, 1969 to December 31, 1973 were followed through October 1976. Treatment failures were based on the occurrence of postoperative cholesteatoma. The probability of being disease free for periods up to five years after original surgery was then estimated for each type of operation. Postoperative cholesteatoma occurred after atticotomy in 17%, intact canal wall mastoidectomy in 35%, and modified radical mastoidectomy in 9%. The postoperative cholesteatoma rate in the group having intact canal wall mastoidectomy was more than twice as high for those patients under age nine. The likelihood of being disease free five years after intact canal wall surgery was estimated to be 36% using the life table method. Disease recurrence was significantly higher after intact canal wall surgery compared to other surgical methods. The effectiveness of this method should be evaluated in a randomized, prospective manner to minimize patient selection and treatment bias.

The risks of major head and neck surgery in the aged population.

A review of 162 major head and neck operative cases for cancer in patients over age 70 from 1963-1973 are reviewed. The major and minor surgical complication rates and the rate of medical complications are compared to 552 similar procedures in patients under age 70, during the same time period. The operative mortality figures for each group, as well as the causes of death, are examined. A plea is made for aggressive therapy in the elderly, both in the surgical planning and in their pre- and postoperative care.

Salivary gland neoplasms in children.

Salivary gland neoplasms in children represent an uncommon entity. However, the presence of such a tumor demands an accurate diagnostic evaluation in view of the commonplace occurrence of malignancy. Observation of fluctuant masses and surgical resection of firm masses are the recommended principles of management, with the majority of these neoplasms having a favorable prognosis.

Central vestibular compensation. Effect of the bilateral labyrinthectomy on neural activity in the medial vestibular nucleus.

An attempt was made to reconstruct the central events that occurred in the various stages of vestibular compensation after a bilateral labyrinthectomy in the cat. Bilateral labyrinthectomized cats showed no nystagmus, but had unsteady head movements and wide gaits. Neural activity in both sides of the medial vestibular nuclei (MVN) was depressed during the critical stage of compensation; however, the neural activity was full and normal during the acute and compensated stage. The experimental results suggest that removing crossed inhibitory influence and reducing the cerebellar inhibitory influence by bilateral labyrinthectomy enhances the process of recovering neural activity in the vestibular nuclei during central compensation, and that the contralateral vestibular end organ is not a source of the driving force responsible for regeneration of electrical activity in the deafferented MVN after a unilateral labyrinthectomy.

The osteo-mucoperiosteal flap in repair of cerebrospinal fluid rhinorrhea.

Repair of fistulas producing cerebrospinal fluid rhinorrhea, as with any reconstructive operations, should take into account the basic principles of reconstructive surgery for best results: 1. tissue needing replacement should be replaced by like tissue; and 2. a flap is always superior to a graft. In the case of this disease, the tissue sealing the fistula should be a flap containing not only mucosa or mucoperiosteum, but also pedicled bone. Two kinds of regional flaps are described which successfully closed the fistula in each of three patients. Follow-up is long term.
