Technology evaluation roundtable discusses role of private sector.

Working from the premise that health care technology should be studied from a multiperspective approach rather than a unidirectional approach, a group of health care opinion leaders gathered to discuss how private or public-private sector efforts could lead to this broad-based approach and to more effective and efficient utilization of health technology.

Production of maximally acid urine by the isolated dog kidney.

The isolated kidney has not been reported to acidify urine maximally. To study this defect, kidneys from dogs fed NH4Cl were perfused with autologous blood. Perfusate pH was 7.20 +/- 0.03 [HCO3] was 14 +/- 1 mEq/L, and urine pH was abnormally high, 6.60 +/- 0.08. When corrected for difference in GFR, UNH4+V was similar to that seen in vivo, but UTAV and UNet H+V were low. FEHCO3- was 2.3% +/- 0.8% and HCO3- excretion persisted to a small degree at perfusate [HCO3-] of 8 to 9 mEq/L. In response to HCO3- infusions, large increases in excretion were not seen until perfusate values were over 24 to 26 mEq/L. HCO3- Tmax was 2.94 +/- 0.07 mEq/dl of glomerular filtrate. The isolated kidney failed to raise U-B PCO2 with HCO3- infusion secondary to low urine [HCO3-] and [Pi]. During perfusion in another group of kidneys from dogs fed NH4Cl and given DOC, perfusate pH and [HCO3-] were similar to those in the first group. Urine pH was also inappropriately high, 7.12 +/- 0.09, and there was no UNet H+V. In response to Na2SO4 infusion, urinary pH fell to 5.00 +/- 0.27. Log10UUAV was correlated to urine pH during the control perfusions in both groups and after Na2SO4 in the NH4Cl + DOC group. Thus production of a low urine pH in the isolated kidney may be mediated by changes in transtubular potential difference resulting from increased distal nephron delivery of Na+ and nonabsorbable anion. The defect in acidification is similar to that observed in incomplete forms of clinical type 1 (distal) renal tubular acidosis.

The all-RN staff: why not?

The emphasis on cost containment and consumer satisfaction has spurred a new upsurge in interest in all-RN nursing. Data are mounting that show that an all-RN staff doesn't necessarily cost more and that its use markedly improves the quality of patient care. Although obstacles to implementation exist, they are not insurmountable, and nurse leaders are asking, "Why not?"

New sharing arrangements meet hospitals' technical support needs.

Just within thid decade, a variety of multihospital arrangements have been developed that make clinical engineering services available to hospitals on a shared basis. This article summarizes available data to describe the diverse approaches and organizational structures into which these programs are evolving.

Cost containment in laundry and linen service.

One major problem looms among all others in the area of laundry and linen service, whether a hospital has an in-house operation, is part of a shared or central laundry, or uses a commercial service. That is the costly problem of controling linen consumption and replacement. A recent seminar offers some insight into reasons for the problem and some possible solutions.

A direct enzyme-linked immunosorbent assay (ELISA) for detection of antibodies for rubella virus in human sera.

A direct enzyme-linked immunoassay (ELISA), based on the "sandwich" principle on an antigen-coated plastic disc, was used for the rapid detection of rubella antibody. Results were obtained the same day, and the prior adsorption of sera to remove non-specific inhibitors was not necessary. The ELISA was compared to the hemagglutination-inhibition (HAI) test on 500 serum samples. There was general agreement between the two methods; most discrepancies occurred with low-titered HAI positive sera. There was excellent correlation between the tests with serum samples negative for rubella antibodies and those samples with HAI titers greater than or equal to 1:40.
