[Revision suture in covered goniotrepanation].

Adhesion of the scleral flap is one of the postoperative complications in glaucoma surgery, especially in cases with trabeculectomy. From July 1989 to February 1990 a trabeculectomy was done in 102 patients with chronic open-angle glaucoma. To prevent adhesion of the scleral flap in the early postoperative period, a 9-0 nylon suture loop was fixed at the scleral flap and led through the conjunctiva. If in the early postoperative phase the intraocular pressure rose due to adhesion of the scleral flap, the flap was opened by pulling the suture loop and thereby releasing the scleral flap. The success of this method is indicated by the drop in intraocular pressure and the filling of the filtering bleb. In 31 of 102 patients an adhesion was observed. Twenty-four of these 31 patients recompensated after one or more pulls at the suture loop, causing the intraocular pressure to remain at a normal level.

[Melanocytoma--a case report].

The present paper reports on a case of melanocytoma with extensive visual field defects in a left eye. Fluorescein angiography, sonography, and computerized tomography showed the tumor to be located in the prelaminar part of the optic disk. There was no sign of infiltrative growth. There has been no change in the findings in the 9 months since the diagnosis was established.

[Classification and importance of cystoid macular edema in pseudophakia].

In a prospective study the authors attempted to establish the incidence of cystoid macular edema (CME) in uncomplicated intracapsular cataract extraction with implantation of Choyce Mark IX lenses. Fluorescein angiography was performed after eight days, six weeks, six months, and at least one year post-operatively. Any edema found was classified as Grade I to III. Grade III CME was demonstrated in 4% of the cases studied after six weeks, though not after six months or one year, respectively. After one year, Grade II was found in 2% and Grade I in 15%, respectively. However, visual function was not impaired. At no time was there any correlation between the severity of the macular edema and visual acuity: all combinations are possible. Grade III CME evidently only impairs visual function if it persists for a prolonged period of time. Hyperfluorescence in the late phase of the angiogram permits the breakdown of the blood-aqueous barrier to be estimated semi-quantitatively.

[Incidence of cystoid macular edema following cataract extraction].

In a prospective study we research the incidence of cystoid macular edema following intracapsular cataract extraction with the Choyce-Mark-IX implantation method. The only eyes included in the study were those for which the operation had been uncomplicated. Fluorescence angiography was performed after 8 days, 6 weeks, 6 months and 1 year. The edema was graded accordingly: grades I-III. Cystoid macular edema of grade III was demonstrated in 4% after 6 weeks but not after 6 months and 1 year. After 1 year, grade II occurred in 2% and grade I in 15%, respectively. Visual acuity, however, was not impaired.

[Direct electrostimulation in the treatment plan of pseudarthroses].

Direct electrostimulation with bipolar rectangular pulse currents was used as a complementary measure on 13 patients with pseudoarthrosis and delayed healing of fractures. In ten cases now completed, evidence was produced to accelerated ossification and transformation of autologous spongiosa, which cut consolidation periods by several months. Implantable mini-stimulators have yielded good clinical results.

[Experiences with Lezius nailing. Report on 1,090 pertrochanteric femoral fractures].

Between 1968 and 1983, 1090 patients suffering from pertrochanteric fractures of the femur were admitted to our clinic. Internal fixation by means of the Lezius-nail was performed in 644 cases. This method has some striking advantages: load stability immediately after operation, hardly measurable loss of blood, short duration of the operative procedure and a low rate of complications (6.5%). The mortality rate amounted to 17.5%, caused by the high frequency of multimorbidity within the patient's group (average age: 80.5 years).

A social and psychiatric survey of unemployment among young people.

Unemployment raised the odds of having a psychiatric disorder by a factor of six in this sample of 401 young single people. The effect of unemployment was borne by men and women equally, since the difference between the sexes was preserved in the proportion who had a psychiatric disorder. Those particularly at risk for psychiatric disorder were men who were dismissed, who could not borrow money readily, and who had large debts. Women at high risk were those who resigned, and who could not borrow money readily.
