[Giant leiomyoma of the esophagus detected by echocardiography and corroborated by x-ray computed tomography and surgery].

The authors report the case of a 44 year old man with a giant leiomyoma of the lower third of the esophagus. The patient presented with chest pain and the tumour was detected by echocardiography. The diagnosis was confirmed by computerised tomography and histological examination of the surgical specimen weighing 501.5 g. The surgeon performed a large esophago-gastric resection and reestablished the continuity of the digestive track by interposing a section of colon. A good result was obtained with a follow-up of 4 years. The authors underline the potential value of a simple barium swallow during cardiological assessment.

[Infectious complications after surgery of esophageal pathology. Apropos of 60 cases].

Infections after oesophageal surgery are studied on sixty patients who received perioperatively two antibiotics (cefoxitin and amikacin). Postoperative infection rate is 62% (pneumopathies: 27%, leakage of cervical anastomoses: 17%). Different parameters which can induce postoperative infection are analyzed. The only significative data are the duration of total parenteral nutrition and of intensive care stay. The commonest isolated organisms are gram negative bacilli (61%) and streptococci (30%). Yeasts infections are frequent, and significantly correlated with antibiotic treatment duration.

[Double cystic duct. Apropos of a case].

In relation to a case of duplicated cystic duct encountered in surgical practice and based on a review of the literature, the authors stress the rarity of this condition. After reviewing the classification of the accessory hepatic ducts, they emphasise the surgical consequences of failure to recognise this type of anomaly during cholecystectomy.

[Cystic dilatation of the common bile duct initially treated by cystojejunostomy and reoperation 25 years later because of infectious and lithiasic complications].

A case of cystic dilatation of the common bile duct is reported. Initially treated by Roux-Y cystojejunostomy, twenty five years after, the patient presented a cholecystitis. This patient was treated by cholecystectomy and a few months after by excision of the cyst with Roux-Y hepato-jejunostomy. This attitude is proposed to avoid the complications of the cystic dilatation of the common bile duct: secondary biliary cirrhosis, angiocholitis, and malignant degeneration.

Ultrasonographic anatomy of the liver.

Two characteristics of the ultrasonic image, that it is rebuilt and non figurative in its details, led us to establish specific relationships between anatomic structures and their ultrasonic images. The anatomic study was made on fresh, injected, frozen, livers, sectioned along the classical echographic planes. The echographic study was conducted in healthy patients, without any hepatic lesions. The results fall under three heads: analysis of the basic structures (portal vein, hepatic vein, and bile ducts) enables to establish perfect relationship between the anatomic and echographic features; the echo-anatomic study of the echographic planes pertaining to percutaneous examination of the liver provides a satisfactory correlation between the anatomy and the ultrasonogram; analysis of the hepatic vascular arrangement enables to define a sectorial systematization of liver structure. Segmental systematization of the right side of the liver is more difficult to define because of the virtual limits of the upper and lower segments of the right paramedian and lateral sectors. This study leads to better echographic understanding of the liver and to a tentative systematization essential to the localization of focal hepatic lesions.

[Endometriosis localized in the sigmoid. Apropos of a case].

Colic endometriosis is a classical but rare localization. The authors present one case treated by partial colectomy associated with a total hysterectomy and preservation of ovarian parenchyma. The hysterectomy was necessary because of adenomyosis and inflammatory adhesions undissociable from the sigmoid.

[Carbohydrate antigen CA 19-9: value in pancreatic pathology].

In order to determine the diagnostic usefulness of the dosage of carbohydrate antigen (CA 19-9) serum levels in pancreatic diseases, 38 patients with pancreatic lesions proved at laparotomy were studied. Pancreatic lesions were classified into two groups: a) group I: pancreatic adenocarcinoma (17 cases) b) group II: benign pancreatic diseases (21 cases). CA 19-9 serum levels were statistically higher in patients of the first group (p less than 0.001). All patients except one with CA 19-9 higher than 60 U/ml had pancreatic carcinoma. All patients except two with CA 19-9 less than 60 U/ml had a benign lesion. The sensitivity and specificity of CA 19-9 were 88 and 95 p. 100 respectively. These results show that CA 19-9 dosage is useful in distinguishing between benign and malign lesions of the pancreas.

The muscles of the abdominal wall: a new functional approach with anatomoclinical deductions.

The basic functional anatomy of muscle can be investigated by histochemical analysis of the muscle fibers and kinesiologic electromyography. The results of these techniques lead to anatomoclinical deductions based on the structure of muscle. Histochemical analysis of the striated fibers of a given muscle shows the existence of three functionally different populations of fibers. The relative abundance of each category of muscle fibers (expressed as a percentage) demonstrates the basic function of the muscle. The investigation of muscle function by kinesiologic electromyography shows that there are three categories of motor activity in the course of movement. These different types of motor activity are dependent upon the muscle fibers which display different physiological features, especially with respect to their contraction time. These two investigative techniques, the overall results of which are concordant, were applied to the study of the function of the muscles of the abdominal wall and led to the following anatomo-clinical deduction: tonico-postural motor activity is predominant in the rectus abdominis and flat abdominal muscles. Kinesiologic electromyography also allowed to demonstrate the functional development of these muscles according to the age, sex, corpulence and physical activity of the subjects studied.

[Peritonitis following cesarean section. Report of 2 cases. The value of laparostomy].

The authors describe two cases of severe septic complications after caesarean section. They compare primary closure of the abdomen with the method of leaving it open. This should only be used very rarely but may help to shorten the stay in hospital and lessen the severity of the complications of severe peritonitis, which still today in these rare cases of surgery carry very heavy mortality.

[Distant metastases of cervical cancer. Apropos of an unusual case. Review of the literature].

The authors present a single but unusual case of metastases from a squamous cell cancer of the cervix which had been treated radically 35 months previously; the metastases occurring in the digestive tract as well as the peritoneum. They try to explain the physiopathological mechanisms, which are probably not unequivocal, as to how theses metastases could have occurred. They point out that further studies should be carried out with a view to seeing whether prophylactic chemotherapy should be given even in very early cancers.

[Rupture of the gallbladder in closed injuries of the abdomen].

A case of rupture of the gallbladder after blunt abdominal trauma is reported. The literature shows that this lesion is rare, and that it occurs mainly in the young. Diagnosis is difficult and tardy, especially when the lesion occurs as an isolated injury, and normally indicates choleperitoneum. Peritoneal lavage provides an early diagnostic aid, and CT scanning subsequently visualizes intraperitoneal fluid. The best procedure is cholecystectomy, and the prognosis is excellent in the case of isolated ruptures.
