[Intracamera penetration of ofloxacin in man].

The authors have studied the penetration of Ofloxacin in the aqueous humour in 30 patients (30 eyes). Six hours after a single oral dose of 200 mg, the concentration in the anterior chamber reached 0.6 microgram/ml which is 23% of the average serum level. It is over the minimal inhibitory concentration necessary for many bacterial agents found in endophthalmitis (except Streptococcus) because the MIC of Ofloxacin are the lowest among quinolones. Side effects and association with other antibiotics are precise.

[Intracameral penetration of pefloxacine in man].

Authors have studied the intraocular penetration of a new antibiotic, pefloxacin in 35 patients (35 eyes). This study emphasizes the three main qualities of this recent quinolone in ophthalmology: wide spectrum adequate for the most pathogens commonly found in endophthalmitis. Very large penetration in the aqueous humour flow with an average level of 0.95 mu/ml, which is over the minimal inhibitory concentration for many bacterial infections and is equal to 25% of average serum level.

[Congenital microcoria: a case report with histopathological study].

The authors report a case in which congenital microcoria was associated with glaucoma; a complete histopathological study was performed and the relation between microcoria and glaucoma was considered. The association with extra-ocular abnormalities, particularly renal lesions is noticed.

[In vitro semi-automated morphometry of preserved human corneal endothelium].

Cellular morphometry allows us to quantify the structural alterations of the corneal endothelium. Our study was a semi-automatized in vitro analysis of fresh and preserved corneal endothelium (MK medium cryopreservation). The equipment used was the Quantimeter 720 connected to a Digital PDP1123 computer. Nineteen corneas were studied and the following cellular parameters were established: perimeter, surface area, form factor and polymorphism coefficient. The computer program used was the SPSS/PC (Statistical Package for the Social Sciences) and the principal statistical tests were the Bartlett test and variance analysis. The statistical computerized study allowed us to compare the four cellular parameters and to establish the incidence rate of the conservation process on endothelial morphometry. The statistical and morphometry methods have proved to be reproducible and reliable and carry a supplementary quantitative element to analyse the influence of the conservation process.

[Computer management of the data of automated perimetry using Octopus 2000 R].

Current perimetry involves automated, static and punctual procedures, which permit the examiner to obtain a numerical value of the sensibility concerning the points tested. Besides its precision and its reliability, perimetry can be used in data processing management due to the characteristic of easy duplication of the examinations. We use a micro-computer which is seperate from the Octopus 2000 R (Apple IIc) and the disc (OmnisII) to process the files. The study is composed of the following areas: the storage and the data processing of patient files, the transformation of numerical data a graphics display, the study of the percentage of loss of surface according to the decibel of sensibility. The data processing of files improves storage according to the criteria defined by the examiner and helps avoid inconvienent research. The graphic transformation of numerical data into isopteres of chosen sensibility improves the definition of the zones of intermediate sensibility. The calculations of the percentage of loss of surface according to the decibel of sensibility allows the appreciation of the size of defects based on depth and quantification. It is also easy to make the comparison between the two fields.

[Statistical analysis of visual fields by the automatic perimeter Octopus 2000 R with the Delta program].

The difference tables gives, on a numeric table, the difference between first and second examination. The confidence interval is given out by a statistical test (T-test) which can be applied to the following parts of visual field: pathological area, whole field. We present a number of visual field examinations (Program 33) from glaucomatous patients, analysed through "change mode". We can make the following remarks: When we assess an improvement, it is underlined in "Delta Program", which indicates: significant improvement. When the visual field is unchanged, "Delta Program" indicates a fluctuation: aggravation or improvement (almost always insignificant). A subjective alteration is always underlined by "Delta Program". The program allows an objective interpretation and, when it is combined with the usual objective interpretation, makes the evaluation of campimetric deficiency easier. The importance of statistical analysis, besides the fact that it associates the visual field to one or several mean values, is to set it off as exactly as possible. Flammer and coll. (1985) propose a new index: "corrected loss variation".

[Automated and semiautomated perimetry. Comparative trial of 3 devices (Baylor programmer, Friedmann Mark II campimeter, Octopus 2000 R.)].

Modern perimetry involves automated or semi-automated procedures, this computer assisted perimetry (CAP) providing results independent of the examiner, and with marked discrimination of light sensibility through quantification of points tested. A wide variety of CAP devices are available, and a comparative study was conducted in 50 patients using 3 perimetric apparatuses with similar supraliminal static techniques but with variable automated potentials: The Goldman perimeter modified by "Baylor visual fields programmer": static technique for the middle area and kinetic for the peripheral area. The Friedmann Mark II Visual Fields Analyser with a multiple stimulus static technique for the middle area. The Fankhauser Octopus 2000 R perimeter with a precise static technique for middle and peripheral areas. This perimeter, highly automated, also allows statistical evaluation of defects changes, owing to storage of results on magnetic discs. Patients studied had either: glaucoma (25 cases) or neuro-ophthalmologic and retinal disorders (24 cases). Conclusions drawn from the experience acquired by the authors during more than one year of use of the 3 devices were: Rate of detection of defects is excellent with the Octopus and quite good with the Friedmann MKII, but limited in the middle area. The Baylor program does not allow quantification of defects. The image is clear with the Octopus as far as the gray scale is concerned, and more exact as far as the comparative tables are concerned; is not clear with the Friedmann MKII; is typical and clear with the Baylor. The length of examination (which varies with the programs used as far as the Octopus is concerned, and with the patient's reaction time) is usually long with the Octopus and the Friedmann, but short with the Baylor. Time and energy can be gained by the use of the Octopus, since it supplies data obtained from use of the Baylor program associated with that of the Friedmann MKII. The authors prefer to follow up campimetric evolution of glaucomatous patients with the Octopus because it offers more sensibility and precision in quantifying losses. The application of the statistical evaluation delta program on the 2000 R device, which will soon be tested in the Ophthalmological Department, could in an objective way indicate whether there is stabilization or a real progression of glaucomatous defects.

[Corneal cryopreservation in man: a proposal for an original technic].

Using the method of Capella and Kaufman, the authors propose an original cryopreservation technique of human corneas. Sixty biomicroscopically normal corneas were obtained from cadavers (50-70 years) within six hours after death. Initial protocol involves four phases: 1) cryoprotection is carried out in three baths of three minutes each with human plasma and DMSO (dimethylsulfoxide) at 2-4-7%; 2) freezing is performed with 10 ml of the final cryoprotective fluid in a controlled rate freezer, the "Minicool" (CFPO). The cornea is cooled at a rate of 2 degrees C/min from + 6 degrees C down - 40 degrees C then at 5 degrees C/min down - 140 degrees C; 3) a tank containing liquid nitrogen at - 150 degrees C is used for a long term storage; 4) thawing is carried out in a water-bath at 37 degrees C for one minute, then in serum albumin 25% for three minutes and in M-K medium for thirty minutes. Effects of three cryobiological parameters are studied: cryoprotection: DMSO is diluted with human plasma at the following increasing concentrations: 2-4-7% or 4-6-10%; freezing medium: the cornea is frozen with or without the final cryoprotective bath. Thus freezing is carried out either in dry or liquid phase; thawing: initial temperature and the osmolar decrease of the dilution are modified in order to obtain four protocols: n degrees 1, initial protocol; n degree 2, an additional step using Hanks 5% - SA 25% for three minutes (1 000 mosm); n degree 3, after initial step reheating the cornea is directly placed in M-K medium; n degree 4, temperature of 50 degrees C for ten seconds precedes the initial protocol. For each protocol change, the endothelial viability is controlled by histological and/or SEM studies results: For each protocol change, the endothelial viability is controlled by histological and/or SEM studies results: cellular osmotic lesions are greater for DMSO concentrations of 10%; dry phase freezing induces more intense and more diffuse nuclear damage than in liquid phase; thawing under protocols 4 and 2 cause a high degree of nuclear damage associated with diffuse cytoplasmic vacuole formation. Histological and SEM studies clearly show endothelial cell lesions due to the cryoconservation. The human cornea endothelium is affected by any modification of the cryobiological parameters.(ABSTRACT TRUNCATED AT 400 WORDS)
