[Use of low-frequency currents in the treatment of severe intestinal paralysis].

Low-frequency impulse currents (Bernar current) are suggested for treatment of paralytic intestinal obstruction. The method was used in 108 patients, in whom the cause of paresis was as follows: acute purulent peritonitis, pancreatitis, operations on the abdominal aorta and its branches. In 102 patients the stimulation proved to be effective, in 6 cases of progressive purulent peritonitis no effect was gained. In a number of patients the procedure had to be repeated during 2--3 days. Generally, diadynamic currents were employed in a complex with other measures: syphon enema, paranephral blockade, cholinergics, etc. A grave cardiac pathology and oncological diseases are considered as contraindications.

Electric Stimulation↗

[10-year experience with heart stimulation in atrioventricular heart block].

Data covering 177 cases of the atrioventricular block where cardiostimulation was effected by using Soviet-made batch-produced electrocardiostimulators are analyzed. The largest group included patients 50 to 70 years old (71 per cent). In 47 per cent of the cases the atrioventricular block stemmed from ischemic heart disease. As indications for cardiostimulation served the Morgagni-Adams-Stocks syndrome and a well-marked circulatory insufficiency. The hospital lethality comprised 15.8 per cent. The death was caused by acute cardio-pulmonary incompetence, thromboembolism of the pulmonary artery and myocardial infarction. Among complications secondary to cardiostimulation figure suppuration of the operating wound, disturbed integrity of the myocardial electrodes and a premature exhaustion of the cardiostimulators reserves. From among 130 patients kept under observation the duration of cardiostimulation comprised: up to 1-year--in 20.7, from 1 to 3 years--in 26.1, from 3 to 5 years--in 20.7, from 5 to 10 years--in 1m.7 and over 10 years--in 14.8 per cent.

Adams-Stokes Syndrome↗

[Evaluation of the prognostic value of endocrine stress disturbances in patients with disseminated peritonitis].

The following endocrine disturbances observed in the early postoperative period are prognostically unfavourable: persisted high blood hydrocortisone level, decreased thyroid hormone, especially T-3, values, damaged negative feedback between T-3 and TTH, decreased leucine-enkephalin blood immunoreactivity. The loss of correlation between hydrocortisone and ACTH, T-3 and TTH, insulin and glucose blood levels, as well as close correlation between hydrocortisone and insulin, hydrocortisone and glucose levels and an abrupt increase in hydrocortisone/insulin, hydrocortisone/STH, hydrocortisone/T-3 and hydrocortisone/T-4 ratios have the same predictive value.
