Chronomics: circadian and circaseptan timing of radiotherapy, drugs, calories, perhaps nutriceuticals and beyond.

We suggest a putative benefit from timing nutriceuticals (substances that are both nutrients and pharmaceuticals) such as antioxidants for preventive or curative health care, based on the proven merits of timing nutrients, drugs, and other treatments, as documented, i.a., in India. The necessity of timing melatonin, a major antioxidant, is noted. A protocol to extend the scope of chronoradiotherapy awaits testing. Imaging in time by mapping rhythms and broader time structures, chronomes, for earliest diagnoses, for example detection of vascular disease risk, is recommended. The study of rhythms and broader chronomes leads to a dynamic functional genomics, guided by imaging in time of free radicals and antioxidants, amongst many other variables.

Side-effects of local hyperthermia: results of a prospectively randomized clinical study.

In 1986, 25 patients with stage II and III carcinoma of the cervix were treated by a combination of radiation and local hyperthermia using an endotract intravaginal applicator. Another 25 patients were treated with radiation alone. Both groups were followed up for a minimum period of 18 months. The acute and long-term toxicity of local hyperthermia was closely monitored. Our study shows that whereas local hyperthermia adds significantly to the local control achieved with radiation alone, it is not in any way associated with any significant short- or long-term toxicity, and does not enhance the radiation reactions.

Management of mediastinal compression due to bronchogenic carcinoma.

Fifty-six cases of bronchogenic carcinoma with mediastinal compression were analyzed retrospectively. Microscopic confirmation was obtained in 23 patients and small cell carcinoma was most frequent. Radiation treatment gave complete remission in about 70 per cent when the total tumour dose exceeded 35 Gy fractionated during 3 weeks. Addition of cyclophosphamide marginally increased the remission rate to about 85 per cent. Total tumour doses of less than 35 Gy gave obviously poorer results. The response rate was highest in small cell carcinoma.

Investigation of children with chronic diarrhoea.

OBJECTIVE: To study the lower G.I. endoscopic and histopathological findings in children with chronic diarrhoea. DESIGN: Prospective analysis. SETTING: Hospital based. SUBJECT: 83 children with age up to 14 years admitted in Umaid Hospital, Jodhpur including 33 subjects with chronic diarrhoea and 50 normal children in control group. METHODS: All 83 children including study (33) and control group (50) underwent lower G.I. Endoscopy using FUJINON-SIG-ET sigmoidoscope in older children and PENTAX FG-23H gastroscope in younger children after proper preparation. Rectal biopsies were taken in all the subjects except one and was sent for histopathological examination. RESULTS: Majority (90.8%) were in 0-5 year age group with median age of 3.8 years. We could finally reach to definitive diagnosis in 13% patients with chronic diarrhoea. Over all, colitis was the most common cause. CONCLUSION: Both, endoscopy and biopsy are needed to reach final diagnosis.
