Inhibition of protein synthesis by spectinomycin.

Spectinomycin selectively inhibits protein synthesis in cells and in extracts of Escherichia coli. Mutations to high-level resistance to this antibiotic map close to the streptomycin locus, and the site of action of spectinomycin, like that of streptomycin, is the 30S ribosomal subunit, as shown by experiments with reconstituted 70S ribosomes containing subunits from sensitive and from resistant ribosomes. In contrast to streptomycin, however, spectinomycin is not bactericidal and causes no detectable misreading of polyribonucleotides.

Bactericidal synergism between beta-lactams and aminoglycosides: mechanism and possible therapeutic implications.

This work, presented as the introductory lecture of the symposium, reviews the mechanisms of antibiotic action of beta-lactams and of aminoglycosides from a historical perspective and gives emphasis to the features relevant to their synergistic interaction. These features include the limiting effect of penetration of aminoglycosides on the rate of killing; the ability of beta-lactams to increase that penetration, even under conditions where they cannot cause a lethal damage to the cell envelope; and the ability of aminoglycosides to be lethal with a smaller amount of cell growth than is required for killing by beta-lactams. The paper also briefly discusses theoretical arguments favoring a broader use of bactericidal synergism in clinical practice.

The socioeconomic impact of chronic pain.

A concern with the socioeconomic effects of chronic nonmalignant pain, as well as the human aspects, inspired a search of the literature for evidence in this area. The review has identified three main areas of interest, socioeconomic statistics; pain prevalence in the community, and the quality of life issues. A selection of the literature is reviewed here under these headings, and the conclusion indicates possible areas for further work.
