Metastatic renal cell carcinoma presenting as shoulder arthritis.

Three patients with previously undiagnosed renal cell carcinoma presented with complaints of shoulder pain secondary to tumor metastases involving the clavicle or upper humerus. In two patients, bone scintigraphy and/or gallium scintigraphy demonstrated the primary renal lesion. One patient had been treated with anti-inflammatory drugs and intra-articular steroid injections for presumed arthritis before the cause of the symptoms was recognized. Review of the literature reveals that renal cell carcinoma frequently metastasizes to bones of the shoulder girdle, and that presenting complaints may relate to osseous metastases. Careful evaluation should be carried out before patients with shoulder pain are treated for presumed arthritis.

Internal mammary lymphoscintigraphy in patients with breast cancer. Correlation with computed tomography and impact on radiation therapy planning.

Twenty patients with Stage I or II breast cancer, all of whom had undergone radiation therapy planning, were examined with internal mammary lymphoscintigraphy (IMLS) and computed tomography (CT). Based on the results of IMLS, radiation fields were revised in 12 cases (60%). The mean number of nodes identified by IMLS in each patient was 7.8, which is in agreement with previously published autopsy and scintigraphic data. CT identified 243 possible nodes of normal size, but only 49 of them were within 10 mm of regions shown to be positive on the scintigram. The authors conclude that IMLS is the method of choice for defining parasternal lymphatic drainage and identifying those internal mammary nodes that are normal in both size and function.

Internal derangement of the temporomandibular joint: detection by single-photon emission computed tomography. Work in progress.

The diagnostic accuracy of both planar and single-photon emission computed tomography (SPECT) bone scintigraphy, radionuclide angiography, arthrography, and conventional radiography was evaluated in 36 patients with temporomandibular joint (TMJ) dysfunction undergoing preoperative testing. The sensitivity of SPECT bone scintigraphy (0.94) was comparable with arthrography (0.96) and significantly better than planar bone scintigraphy (0.76), radionuclide angiography (0.35), and transcranial lateral radiographs (0.04). While data for a larger asymptomatic control population are needed, preliminary results give SPECT a diagnostic specificity of 0.70 for internal derangements of the TMJ requiring surgical correction. It is concluded that SPECT bone scintigraphy is a useful noninvasive imaging test to screen for internal derangement of the TMJ.

Erythrocyte - endothelial cell adherence in sickle cell disease, diabetes mellitus, and falciparum malaria: adverse effects reversed with piracetam.

The erythrocytes of patients with sickle hemoglobin, diabetes, and Falciparum malaria adhere disproportionately to endothelial cells. Such pathophysiological activity compromises the microcirculation and results in clinical disease. Since Piracetam (2-oxo-1 pyrrolidine acetamide) has been shown to have a number of clinically beneficial actions on the formed elements of the blood including disengagement of adherent diabetic and sickle erythrocytes there is a rational basis for the trial of Piracetam as an adjuvant drug in SS disease and in diabetes mellitus to improve function of the microcirculation. For similar but somewhat more complex reasons Piracetam may potentiate the efficacy of anti-malarial drugs at any given dosage. Piracetam, a drug known to be safe in a decade of clinical usage, merits serious study in the 3 cited diseases.

Ultrasound directed placement of needles into brains of rhesus monkeys.

Gray scale ultrasound identified the lateral and third ventricles, basal ganglia, columns of the fornix, and the suprasellar optic system of monkeys with craniotomy defects. Needles were placed in these structures under ultrasound guidance. Radiographs and formalin fixed brain sections confirmed the expected position of the needle tracts.

Simultaneous 99mTc-P-butyl-IDA and 131I-rose bengal scintigraphy in neonatal jaundice.

Eight neonates with jaundice were studied simultaneously with 99mTc-p-butyl-IDA and 131I-rose bengal. Due to physical decay, 99mTc-p-butyl-IDA failed to demonstrate delayed excretion through the patent extrahepatic biliary tract in 3 of 5 patients with concomitant hepatitis; 131I-rose bengal showed small-bowel activity in all 5. Neither agent demonstrated small-bowel activity in 3 neonates with extrahepatic biliary atresia. Based on this clinical trial, 131I-rose bengal remains the radiopharmaceutical of choice for distinguishing between hepatitis and biliary atresia in these patients.

Arthrosonography: gray-scale ultrasound evaluation of the shoulder.

Gray-scale ultrasound images of the shoulders of 6 rhesus monkeys were obtained before and after fluid instillation to assess the ability of ultrasound to diagnose joint disorders. The normal bony landmarks and muscular structures were shown. After the fluid was introduced, fluid collections were identified in the axillary pouch and subscapular bursa. Ultrasound is a promising new method of detecting intra-articular effusions of the shoulder.
