Dysgerminoma. A report of five cases.

We analyzed five cases of pure dysgerminoma following the same schedule for the treatment, which were studied during the period of 1971 to 1983. The age of patients during the period of histological examination ranged from 15 years and 6 months to 28 years. All five patients had already had their "first menstruation". The above cases examined according to the clinical findings, the symptoms, the stage of the disease; the sort of treatment the place, the period and kind of therapy in case of reappearance of the disease and final survival.

Management of uterine adhesions with 'Massouras Duck's Foot' and 'Butterfly' IUDs.

The Massouras Duck's Foot (MDF) IUD and the Butterfly IUD have been clinically evaluated. The MDF, which is Y-shaped, has 2 or 3 horizontal arms on a vertical stem and 2 triangular wings, in between the 'V'. When inserted after curettage, it prevents intrauterine or intracervical adhesions. The fan-shaped Butterfly IUD has 2 overlapping wings, thereby facilitating adjustment of the IUD to variable uterine cavities. Its wing span occupies the entire uterine cavity when inserted according to the instructions. Both IUDs are made of polyethylene plastic with barium sulfate. Several substitutions for barium sulfate are suggested, e.g., copper, silver, hormones, biodegradable polymers. The high antifertility action of these IUDs is due primarily to their occupancy of the entire uterine cavity.

Treatment of primary dysmenorrhea with mefenamic acid.

Thirty-five patients (16--23 years old) who had severe primary dysmenorrhea were each treated with 500 mg of mefenamic acid every eight hours for a maximum of three days during menstruation for three consecutive cycles. A total of 194 treated cycles could be evaluated, 110 cycles with mefenamic acid and 84 with placebo. Mefenamic acid produced complete relief of all the symptoms of dysmenorrhea in 31 (88.6%) patients in all 98 treated cycles and, in another two patients, moderate relief in five of the six cycles. While on placebo, only five patients (13%) experienced moderate to slight relief in 11 of the 15 cycles. It is concluded that the mefenamic acid is safe and effective in most patients for the relief of primary dysmenorrhea and represents a rational short-term therapy for this syndrome.

Prolactin levels during labor.

To test the hypothesis that prolactin (PRL) plays a role in the hormonal events of labor, serum PRL levels in 15 normal secundigravidas were measured on 2 occasions 10-15 days before delivery, at the onset of labor, at cervical dilatation of 5 and 10 cm, at the time of delivery, and on the first, second, and fifth days postpartum. The mean level of PRL was 163 ng/ml +/- 26 ng/ml at the onset of cervical dilatation; it typically decreased with the progress of labor, reaching a value of 140 ng/ml +/- 21 ng/ml at the time of delivery. The differences during the various stages of labor, however, were not found to be statistically significant. Postpartum values were significantly lower (P less than 0.01) on the fifth day after parturition. It is therefore unlikely that PRL is involved in or influenced by the hormonal interplay that occurs during labor.

[Endometrial chondroma].

A case of ectopic cartilage formation is reported in the endometrium of woman suffering from tuberculosis of the internal genitalia. The possibilities as far as the theories relating to the explanation of cartilage formation are analysed.

Post-hysterectomy morbidity after previous punch cervical biopsy plus diagnostic curettage.

The post-operative morbidity of 70 patients who underwent total abdominal hysterectomy five to thirteen days after punch-biopsy of the cervix uteri and diagnostic curettage was compared of that of a control group of patients, with hysterectomy alone. It was observed that the sequence punch-biopsy hysterectomy presented a significantly higher post-operative morbidity. This increase was mainly due to higher parametritis, excessive vaginal discharge and wound infections rates. The interval between the two operations was also found to influence the post-hysterectomy morbidity, which seems to decrease with longer intervals.
