Laparoscopic resection of an adenoma of the urachus in combination with a laparoscopic cholecystectomy.

In a 40-year-old male patient with symptomatic cholecystolithiasis, ultrasound examination disclosed a large cystic mass on the dome of the bladder. Laparoscopic resection of this mass was carried out in combination with a laparoscopic cholecystectomy. Histology disclosed an urachal adenoma. Postoperative recovery was uneventful. We conclude that urachal adenomas can be managed safely by laparoscopic means.

A phase II trial of dose-escalated doxorubicin and ifosfamide/mesna in patients with malignant mesothelioma.

BACKGROUND: This study investigated the feasibility and efficacy of doxorubicin dose-escalated chemotherapy in combination with ifosfamide in patients with malignant mesothelioma. PATIENTS AND METHODS: In this single institution phase II study, 24 chemotherapy-naive, eligible patients were entered. The chemotherapy regimen consisted of doxorubicin 75 mg/m2 in combination with ifosfamide 5 gr/m2 given as a continuous 24 hour infusion, every 21 days with either rhG-CSF (5 micrograms/kg) or rhGM-CSF (250 micrograms/m2) as haematopoietic support from d3 to d14. Cycles were repeated every 3 weeks. RESULTS: We treated 24 patients, of whom 22 are evaluable for tumour response. One of the two inevaluable patients died from a cerebral haemorrhage during a period of grade III thrombocytopenia after the second course. In 7 patients a partial response was observed, resulting in a response rate of 32% (95% confidence interval 13%-51%). Median response duration was 6 months (range 1-13) and median survival was 7 months (range 1-18). CONCLUSIONS: The high-dose regimen with growth factor support is feasible in this group of patients and leads to an interesting response rate. The limiting toxicity for further dose increments and more courses of treatment, was cumulative thrombocytopenia. There seems to be a subgroup of patients with malignant mesothelioma which is less susceptible to develop thrombocytopenia. However, the overall toxicity and the poor response duration limit the use of this schedule in the treatment of malignant mesothelioma.

Cytomegalovirus colitis in a patient with AIDS: CT findings.

This case report describes an AIDS patient with a cytomegalovirus (CMV) colitis. A CT scan of the abdomen showed a thick colonic wall, producing a target sign. CT scanning of the abdomen may be useful in patients suspected of CMV rectocolitis in order to decide whether a rectosigmoidoscopy or a complete colonoscopy should be performed. A definitive diagnosis of CMV rectocolitis can only be made by intestinal biopsy.

Hyperfunctioning unilateral adrenal macronodule in three patients with Cushing's disease: hormonal and imaging characterization.

We aimed to investigate the dynamics of adrenocorticotropin (ACTH) and cortisol secretion in pituitary-dependent Cushing's syndrome with bilateral macronodular adrenal hyperplasia presenting as a single adrenal macronodule, and to determine the imaging characteristics of this syndrome. Three female patients were studied. Plasma ACTH and serum cortisol secretion were studied by determining their rhythmicity and pulsatility and their responses to the administration of ovine corticotropin-releasing factor, thyrotropin-releasing hormone, metyrapone, tetracosactrin, insulin and dexamethasone. Techniques used to localize the anatomical lesion were bilateral simultaneous inferior petrosal sinus sampling, magnetic resonance examination of the pituitary, computed tomography (CT) scanning and [75Se]cholesterol scintigraphy of the adrenal glands. Plasma ACTH and serum cortisol levels were measured using a commercial radioimmunoassay and an immunoradiometric assay. The ACTH and cortisol pulse number and amplitude were calculated using established computer software. In all three patients ACTH and cortisol secretory dynamics fulfilled the requirements for diagnosis of pituitary-dependent Cushing's syndrome. A close relationship between ACTH and cortisol pulses also favored a pituitary dependency. Study of the amplitude of cortisol pulses classified two patients in the group of hypopulsatile Cushing's disease. Adrenal CT scanning demonstrated the presence of a large single nodule. [75Se]Cholesterol scintigraphy showed bilateral radionuclide uptake, although mostly localized over the adrenal nodule. All patients underwent successful trans-sphenoidal hypophysectomy. Over a period of 1 year, a slow shrinkage of the adrenal nodule was observed in two patients, while no change in volume was observed in one patient.(ABSTRACT TRUNCATED AT 250 WORDS)

Computed tomography of major salivary gland tumors. A retrospective study of 31 cases.

To assess the value of computed tomography (CT) in grading (benign versus malignant) and characterization of salivary gland tumors we retrospectively analysed the CT examinations of 31 histologically proven cases (15 pleomorphic adenomas, 6 cystadenolymphomas, 1 lipoma, 4 malignant tumors, 4 cysts, and 1 abscess). Subsequently we studied the localization, size, shape, margins, intratumoral calcification, central lucency, local invasion, homogeneity, density, degree and pattern of enhancement after intravenous contrast injection, and presence of enlarged locoregional lymph nodes. A lesion with ill-defined margins, local invasion, diffuse spread throughout the gland and enlarged locoregional lymph nodes was considered malignant. All malignant lesions were correctly diagnosed (no false negative) but 3 benign lesions had malignant features on CT (3 false positives). Further characterization between pleomorphic adenomas and cystadenolymphomas is often impossible. A cystic lesion with papillary contrast-enhancing projections was found in 2 out of 6 cystadenolymphomas and not in other lesions.

Ultrasonography of the salivary glands in the evaluation of Sjögren's syndrome. Comparison with sialography.

Ultrasonography (US) was evaluated as noninvasive method to determine salivary gland involvement and compared with sialography in patients with Sjögren's syndrome (SS) and patients with only objective xerostomia and/or xerophthalmia. Patients with complaints of dry eyes and dry mouth but with no objective abnormalities served as control group. Ultrasound revealed a decreased parotid gland reflectivity in most of the patients with SS, compared with only 2 patients in the other group (p = 0.002). Our study suggests that US can be a useful, noninvasive method in the diagnostic work-up of patients with sicca complaints.

Cervical tuberculosis. Differential diagnosis and CT imaging.

This paper shows some typical CT-scan patterns of tuberculous cervical adenitis. Two cases are presented which suggested to be a cyst of the first and second branchial cleft according to their appearance and location on CT-scan, but which turned out to be cervical tuberculous adenitis after further investigation. It is meant to stress the importance of tuberculosis in the differential diagnosis of cystic lesions of the neck on CT imaging.

Intrapulmonary lymph nodes in the differential diagnosis of solitary pulmonary nodules: case report and review of the literature.

A 49-year-old man was admitted for further investigation of a coin lesion with a diameter of 1 cm. He was a heavy smoker with no professional exposure. Since the percutaneous needle biopsy did not yield a definitive diagnosis, a thoracoscopy was performed. The solitary pulmonary nodule was found to be an intrapulmonary lymph node with anthracosilicotic pigment and a thoracoscopic wedge resection was performed. A review of the case reports in the period 1961-1993 shows that intrapulmonary lymph nodes could be more frequent than originally thought. All patients were smokers but professional exposure was not a constant finding. The differential diagnosis and management of the indeterminate solitary pulmonary nodule are discussed.
