The prevalence of hepatitis B in Sweden; a statistical serosurvey of 3381 Swedish inhabitants.

The prevalence of hepatitis B virus markers in the adult Swedish population was investigated according to age, sex, origin and demographic stratum. Sera were collected from 3382 persons in 1990-1. The sera were selected on a statistical basis considered to be representative of the Swedish population from adults aged > or = 18 years. Two of the sera (0.06%) were found to be hepatitis B surface antigen positive. The two hepatitis B carriers were of non-Scandinavian origin as were (8.9%) of those tested. A total of 90 persons had a marker of previous, hepatitis B virus infection, i.e. antibodies against hepatitis B core antigen. Of these, 66 (2.0%) were of Scandinavian origin and 24 (18.1%) from highly endemic areas. The overall hepatitis B virus marker prevalence was 2.7%. The highest age-specific prevalence of hepatitis B markers in those of Scandinavian origin was in those born in 1939 and earlier. In this age-group, women had a significantly higher prevalence (3.6%) than males (1.9%). The lowest prevalence was found in those born in 1970 and later. No significant, age-related differences between younger or older persons, or between men and women, could be found in persons of non-Scandinavian origin. The results showed significant differences in exposure to hepatitis B virus among the indigenous population, compared with those of non-Scandinavian extraction. The results do not support the proposal to include hepatitis B vaccination in the Swedish immunization schedule.

Hepatitis A immunity in the Swedish population. A study of the prevalence of markers in the Swedish population.

After a 20-year interval, the prevalence of seroimmunity to Hepatitis A (HA) was again investigated in a statistical sample of the adult Swedish population. Sera from 3382 of the 4800 originally selected persons were tested. The prevalence of antibodies to HA had not changed since the 1960s when only the Scandinavian population was considered. In the oldest population born at the beginning of this century, the presence of antibodies amounted to 69%. It gradually declined to 6% in those born in the 1940s. In the population born after 1950, the percentage of seropositive individuals was only 2%. A slightly higher prevalence was seen in the big cities, compared with the rural areas (13% vs 9%). Persons of non-Scandinavian origin showed a different pattern. Those from other European countries showed a prevalence of about 70% in all the age-groups investigated. Among the young adults of Arabic or Asiatic origin, the figure was > 90%. The conclusion is that the native Swedish population has a low natural exposure to HA, which has not changed during the last 20 years. Prophylaxis before going to countries where the disease is endemic is strongly recommended.

Carriage of multiresistant Streptococcus pneumoniae among children attending day-care centres in the Stockholm area.

To determine the prevalence of the asymptomatic carriage of drug-resistant Streptococcus pneumoniae (DRSP) by children attending day-care centres in the Stockholm area, nasopharyngeal swabs were cultured from 1129 children and 308 day-care personnel in 36 day-care centres during a 3-week period, from March to April 1995. Approximately 36% of the children were asymptomatic carriers of S. pneumoniae sensitive to penicillin and other antibiotics. The highest prevalence of nasopharyngeal carriage was found in the 2-year-old group (50%), whereas among the 4-year-old children colonization was observed in 42%, and among the 7-year-old children 21% were asymptomatic carriers of penicillin-sensitive S. pneumoniae. In 2 day-care centres, 4 and 5 children, respectively, were found to have DRSP strains in the nasopharynx. The same serotype of DRSP strain was found among the children attending the same day-care centre. During the same period, none of the staff were found to harbour DRSP in the nasopharynx, but 3% were asymptomatic carriers of penicillin-sensitive S. pneumoniae. The patterns of nasopharyngeal colonization by Haemophilus influenzae, Moraxella catarrhalis and Group A streptococci were also studied in 635 children during the same period. 42% of the nasal cultures yielded Moraxella, 32% H. influenzae and 2% Streptococcus pyogenes.

Early results of laparoscopic surgery for colorectal cancer. Retrospective analysis of 372 patients treated by Clinical Outcomes of Surgical Therapy (COST) Study Group.

UNLABELLED: This study was undertaken to determine the early experience of the embers of the COST Study Group with colorectal cancer treated by laparoscopic approaches. METHOD: A retrospective review was performed of all patients with colorectal cancer treated with laparoscopy by the COST Study Group before August 1994. Tumor site, stage, differentiation, procedure completion, presence of recurrence (local, distant, trocar site), and cause of death were analyzed. RESULTS: A total of 372 patients with adenocarcinoma of the colon and rectum were treated by laparoscopic approach between October 1991 and August 1994 (170 men and 192 women): right colectomy, 170; sigmoid colectomy, 55; low anterior resection, 56; abdominoperineal resection, 44; left colectomy, 22; colostomy, 8; total colectomy, 6; transverse colectomy, 7; exploration, 2. Conversion to an open procedure was required in 15.6 percent of cases. Operative mortality was 2 percent. Tumor characteristics were as follows: TNM state: I, 40 percent; II, 25 percent; III, 18 percent; IV, 17 percent; Differentiation: well-moderate, 88 percent; poor, 12 percent; carcinomatosis, 5 percent. Local (3.6 percent) and distant implantation occurred in four patients (1.1 percent). Only one of these patients died a cancer-related death (Stage III at 36 months). Cancer-related death rates increased with increasing stage of tumor: I, -4 percent; II, 17 percent; III, 31 percent; IV, 70 percent. CONCLUSION: A laparoscopic approach to colorectal cancer results in early outcome after treatment that is comparable with conventional therapy for colorectal cancer. A randomized trial is needed to compare long-term outcomes of open and laparoscopic approaches with colorectal cancer.

[Panorama of infections among refugees--the risk of epidemics from the East].

The article consists in an account of the numbers of reported cases of infectious diseases among approximately 10,000 refugees who came to the Stockholm area, primarily from the former Yugoslavia, Africa, Asia and Irak during the period 1992-1993. High prevalences were noted of hepatitis B and tuberculosis, and in certain groups high prevalences of HIV. Hitherto there has been no increase in the spread of infections to the general population. In the event of a wave of refugees from Russia and the Baltic countries, it is primarily diphtheria and tuberculosis that will entail vigilance.

Long-term follow-up study of rubella antibodies in naturally immune and vaccinated young adults.

Selective rubella vaccination of 12-year-old schoolgirls was introduced in Sweden in 1973 and at the same time a long-term follow-up cohort study was initiated. In 1982, a two-dose programme with a combined vaccine against measles, mumps and rubella (MMR) was introduced and vaccinations were given at the ages of 18 months and 12 years to both boys and girls. The cohort initially comprised 486 girls. It was followed for between 8 and 16 years. All the girls enrolled were seronegative before vaccination and had seroconverted to a haemagglutination-inhibition (HAI) titre of at least 1:16. On the last test occasion 16 years later, 22% had titre values below 1:16, and 6% lacked detectable antibodies against rubella (< 1:8). A fourfold or greater rise in titre was seen in 36% of the girls during the first 8 years of observation, whereas during the following 8 years only 1% showed a significant increase of titre values. The geometric mean titre declined from 1:110 to 1:34 during the first 8 years and further to 1:18 during the following 8 years. From 1982 to 1990, the seroimmunity to rubella of 18-year-old girls and boys was studied yearly. The number studied was 3308 18-year-old schoolgirls and 6347 18-year-old recruits born between 1964 and 1972. The recruits were divided into two groups, 4610 unvaccinated and born in 1964-1969 and 1737 vaccinated and born in 1970-1972. Seropositive recruits in the first group were thus naturally immune only, while the second group had a mixture of natural and vaccine-induced immunity.(ABSTRACT TRUNCATED AT 250 WORDS)

Measles antibody: comparison of long-term vaccination titres, early vaccination titres and naturally acquired immunity to and booster effects on the measles virus.

A two-dose vaccination programme, using a combined measles, mumps and rubella vaccine (MMR) and administration at the ages of 18 months and 12 years, was introduced into Sweden in 1982. Since the combined MMR vaccine was introduced, a yearly evaluation of the immunity patterns and seroconversion rates in 12-year-old children has been carried out. This study includes three study groups. All groups consisted of pre- and postvaccination samples from 12-year-old children taken in connection with the MMR immunization. There were 332 paired samples from children who 8-10 years previously had received live measles vaccine. Prevaccination sera from these children represent late postvaccination titres. The postvaccination sera of 49 children seronegative to measles before vaccination, obtained 2 months after vaccination, represent early postvaccination immunity. Ninety-five children who had not been vaccinated earlier and who were seropositive to measles represent naturally acquired immunity. All samples were studied by an enzyme-linked immunosorbent assay (ELISA). Sera from children with late postvaccination immunity had significantly lower titres than children in group 2, who represented early postvaccination sera (p < 0.001). Children with naturally acquired immunity had significantly higher titres than children with vaccine-induced immunity titres (p < 0.001). After revaccination of the previously vaccinated children, a significant booster rise was seen (p < 0.001). After revaccination of children with naturally acquired immunity, no significant booster effect was observed. Sixty-five children of the 332 (20%) previously vaccinated children had no or borderline measles titres prior to the booster. The study suggests that the vaccine-induced measles antibodies decline with time and may fall under the protective level.(ABSTRACT TRUNCATED AT 250 WORDS)

Salmonellal splenic abscess in the antibiotic era: a Latin American perspective.

Ten cases of salmonellal splenic abscesses recently documented in various Latin American countries are discussed. All patients were adults; the mean age was 32.6 years, and there was a predominance of males (seven). Predisposing conditions were identified in four cases. All 10 cases were documented by diagnostic imaging techniques; in one case, exploratory diagnostic laparotomy was also performed. Splenectomy was performed on eight patients, while two other patients responded to long courses of intravenous antimicrobial therapy alone. One patient died as the result of perioperative splenic rupture, and two patients underwent second laparotomies because of left subphrenic abscesses. Except for one human immunodeficiency virus-infected individual, all patients were immunocompetent and had large solitary lesions. Salmonella typhi was the predominant organism isolated and was recovered in six of the 10 cases.
