Effects of cigarette smoking on lung function in four population samples in the People's Republic of China. The PRC-US Cardiovascular and Cardiopulmonary Epidemiology Research Group.

As part of an ongoing study of cardiopulmonary risk factors in the People's Republic of China, we conducted lung function tests and obtained information about smoking habits on 6,765 Chinese men and women 35 to 56 yr of age residing in or around Beijing in the north and in or around Guangzhou in the south. Within each region, separate urban and rural populations were recruited. This study examined the relationship between tobacco consumption (both manufactured cigarettes and leaf tobacco) and lung function in a subset of current smokers and never smokers who had acceptable lung function data. All methods were strictly standardized. Overall, tobacco smoking was associated with a statistically significant mean difference in FEV1 among men (-89 ml) and women (-52 ml) relative to never smokers after adjusting for age, height, and residence. Differences between smokers of cigarettes and smokers of leaf tobacco were not significant. Among the subset of smokers who smoked only cigarettes, this decrement increased with increasing duration of cigarette smoking, but it was small (-4 ml/yr of smoking for FEV1 for both men and women) in comparison with the effects of smoking reported from western countries. Although the smoking effect tended to increase with increasing dose, these differences were small and generally not statistically significant. The relatively small smoking effect in this study may result from differences between developed and developing countries in the cumulative dose of tobacco products. Alternative explanations or contributing factors such as racial differences in susceptibility and differences in the form and delivery of tobacco cannot be discounted.

[Mutagenicity of tyramine extracted from salted dried fish in high-risk area of gastric cancer in Zhuanghe county].

Content of tyramine was determined in salted and dried small fish collected from high-risk area of gastric cancer in Zhuanghe County, Liaoning Province with high pressure liquid chromatography, and its mutagenicity after nitrosification was assayed. Results showed content of tyramine in the fish correlated significantly with its mutagenicity (r = 0.993, and P < 0.01). And, tyramine extracted from the fish was verified by thin layer chromatography.

The association of polymorphisms at a VNTR locus 3' to the apolipoprotein B gene with coronary heart disease in Chinese population.

The polymorphisms of variable number of tandem repeats (VNTR) 3' to the apolipoprotein B (apo B) gene were investigated using polymerase chain reaction (PCR) in a sample of 103 patients with documented coronary heart disease (CHD) and 100 healthy individuals selected from Chinese Han nationality. Twelve segregating alleles (3' beta 29-51) were observed in the pooled total of 203 subjects. The most common allele was 3' beta 37, followed by 3' beta 39 with frequencies of 0.362 and 0.296, respectively. This model of allele distribution was coincident with the results form different ethnic groups, but the relative frequencies of alleles were different. In comparison with the allele frequencies between the patients and controls, alleles bigger than 3' beta 39 (3' VNTR-B) were significantly more common among the patients than among the controls (P < 0.001). Moreover, in the CHD group patients with plasma levels of TC > or = 3.88 mmol/L, LDL-C > or = 2.59 mmol/L and HDL-C < 1.16 mmol/L had significantly higher frequencies of 3' VNTR-B allele (P < 0.01). Therefore, it is suggested that 3' VNTR-B allele might be involved in the development of coronary atherosclerosis, presumably through their influences on lipid metabolism.

[Free radical production in the subdermal vascular network thin skin flap].

In this experiment 12 piglets were used to study the survival length and free radical production of subdermal vascular network thin skin flap, fascial flap and PSVN. The flap or graft was designed 15 cm x 3 cm in size on the flanks of the animals. Twenty-four flaps or grafts were formed in each group. The observation on 7 postoperative day indicated that the survival length of the subdermal vascular network thin skin flap was 38% over that of fascial flap, while the free radical was less in the former, and it could be one of the reasons for its better survival.

[Hemodynamic effects of inhaled nitric oxide in patients with pulmonary hypertension].

12 patients with pulmonary hypertension, primary pulmonary hypertension 3, secondary to recurrent pulmonary embolism 4, chronic obstructive pulmonary disease 2, and complicated with congenital heart disease 3, inhaled nitric oxide (NO). The NO concentration was adjusted in the range between 20-160 ppm to get a maximum hemodynamic effects. On inhaling NO, pulmonary artery pressure decreased in all patients. The maximum hemodynamic effects showed that pulmonary systolic, diastolic and mean artery pressure dropped by 22.9%, 21.0% and 20.3% (P < 0.001) respectively. Total pulmonary resistance decreased by 32.7% (P < 0.01). Cardiac index and oxygen delivery increased by 33.2% and 24.8% (P < 0.01) respectively. The ratio of pulmonary systolic artery pressure and systemic systolic artery pressure changed from the baseline 0.73 to 0.54. It is indicated that NO dilated pulmonary artery selectively in patients with hyperkinetic, obliterative and hypoxic pulmonary hypertension.

[Determination of procollagen III levels in serum and broncho-alveolar lavage fluid in patients with interstitial lung disease and its clinical significance].

Serum and broncho-alveolar lavage fluid (BALF) levels of procollagen III (PCIII) were determined in 26 patients with interstitial lung disease (ILD) and healthy subjects as controls. The results showed: (1) Levels of PCIII in serum and BALF were significantly higher in the patient group than those in the control group; (2) A highly positive correlation was found between PCIII levels and alveolar macrophage counts in BALF in the patient group. (3) PCIII level in BALF (albumin as a denominator) is higher than that in own serum both in control and patient group, the higher level of PCIII in BALF might be the result of local production. We are of the opinion that changes of PCIII level in serum and BALF might be helpful in the evaluation of the turnover of collagen in the lung.

[Diagnosis and treatment of interrupted aortic arch: report of 6 cases].

Six patients with interrupted aortic arch were operated on from Nov. 1986 to Jun. 1992. 4 patients were type A and 2 type B. The result was good except a patients of type A died early after operation. Two-stage operation was performed for 4 patients of type A, and 2 patients of type B accepted posterolateral incision. The aortic arch was constructed with artificial vessel. Cardiopulmonary bypass was set up with patients lying on back, then PDA was ligated and VSD closed. Pulmonary artery trunk reconstruction and VSD closing were performed. Three months after operation a patient died of septicemia caused by infection of incision.

[Preliminary study on multi-resistant gene location of 20 strains of Pseudomonas aeruginosa].

20 strains of Pseudomonas aeruginosa were isolated from burn patients of Xijing Hospital in Xi'an. These 20 strains which showed resistance to multiple antibiotics were selected to be studied regarding the location of multi-resistant gene. According to the sensitivity tests before and after plasmid removal, we found that in 70% of the bacteria resistance to multiple antibiotics was mediated by the plasmid. In 30% the bacteria remained resistant to antibiotics after plasmids were removed. Two explanations were postulated. (1) plasmids were not removed completely; (2) resistance gene was encoded by the chromosomes.

[Electroacupuncture promotes enlargement of adrenals and enhances level of blood corticosterone in ovariectomized rats].

The present article reports EA with special acupoints may promote the enlargement of bilateral adrenals and enhance the level of blood corticosterone in ovariectomized (OV) rats. The mean of adrenal weight (M +/- SE, mg) as following, OV + EA group 71 +/- 2.1, OV group 57.4 +/- 4.7, EA group 57.6 +/- 1.1 and control group (CT group) 54 +/- 2.8 respectively. For observation of a self control, the detection of blood corticosterone contents was performed in the same animals before and after ovariectomy. The results showed that before ovariectomy the contents of blood corticosterone in the rats were 3.27 +/- 0.7 micrograms/100ml before EA and 3.63 +/- 0.9 micrograms/100ml after EA. No significant difference of blood corticosterone level was found before and after EA. After the animals ovariectomized the level of blood corticosterone increased from 4.38 +/- 0.62 micrograms/100ml to 6.02 +/- 0.77 micrograms/100ml following EA treatment (P < 0.01). No similar effect of EA treatment with control acupoints in OV rats was seen. These results suggest that EA may regulate the abnormal function of endocrine and this effect of EA possesses relative specificity of acupoint.

[Application of the local flap pedicled with a superficial vein in the repair of limb wounds].

Fourteen cases of limb wounds were repaired with the local flap pedicled with a superficial vein. All the flaps survived with satisfactory results. The superficial vein that speeds drainage from the flap can also provide blood supply to the flap, which is beneficial to the survival of the flap. The ratio of length to width of the flap is greater than that of the traditional flap. The creation of the flap is easy and the nearby artery is intact. The technique details are described in the article.

[Experimental studies on the pharmacokinetics of cefoperazone (CPZ) injection under burn eschar].

UNLABELLED: In this study, the rabbits were categorized into three groups. The scald (III degrees TBS 10%-15%) was caused by exposing these area to hot water at 90 degrees C for 40 seconds. Every rabbit was injected CPZ (25 mg/kg) three times. The changes in the concentration of CPZ were measured by employing MBPD and HPLC methods and experimental data were analyzed by using pharmacokinetic computer program. RESULTS: 1) The T1/2e of CPZ injection in subeschar region was longer and the Cmax was higher than that of intravenous route. 2) The concentration in blood was shown two times higher than after subeschar injection. 3) The levels of concentration were higher than MIC'S in an area of 6.5 cm diameter 24-36 hours after injection. CONCLUSION: CPZ injection in subeschar region was the best route of treatment and prevention of subeschar infection.

[An analysis of ocular manifestations in cases with tumors of accessory nasal sinuses].

The ocular manifestations of 60 cases with tumors of accessory nasal sinuses were reported. They were proptosis, impairment of ocular movement, diplopia, decrease or loss of visual acuity, ptosis, etc. Of them, 42 patients came to the ophthalmology department first. All the patients were diagnosed by computerized tomography (CT scan) and pathological findings. Operative treatment was applied in 48 cases. 32 cases had improvement in ocular symptoms. Particular attention was paid to the analysis of the causes of the patients, ocular symptoms and signs. They are due to the close anatomical relationship between accessory nasal sinuses and the orbit and optic nerve canal. CT scan is helpful to the early diagnosis of the disease.

Cloning, sequencing and overexpression of the Desulfovibrio gigas ferredoxin gene in E. coli.

We have cloned the gene encoding Desulfovibrio gigas ferredoxin using a photodigoxigenin-labelled probe synthesized with the polymerase chain reaction. The DNA sequence of the gene predicts a polypeptide of 58 residues after removal of the initial formyl methionine (polypeptide M(r) = 6,276). The ferredoxin gene was expressed in aerobically grown E. coli behind the lac promoter of pUC18 resulting in a high level of ferredoxin expression which comprises about 10% of the total cell protein. EPR analysis of recombinant ferredoxin revealed the presence of a [3Fe-4S] cluster which is characteristic of native D. gigas ferredoxin II.
