Clostridium difficile and cytotoxin in routine faecal specimens.

Over a five-month period 1239 unselected, routine faecal specimens from 856 patients were examined for Clostridium difficile. One hundred specimens representing 69 patients were culture-positive. Toxin was detected in the stool of ten. During the study period, there were 41 Salmonella, 12 Campylobacter and 9 Shigella infections. C difficile was isolated together with Salmonella from 12 patients. No patient required specific treatment for C difficile infection. The significance of these findings is discussed.

Candida tropicalis meningitis: a case report.

A 54-year-old man presented with candida tropicalis meningitis after exploration of the mastoid. He responded well to the combined intravenous administration of amphotericin B and 5 fluorocytosine. But 5 fluorocytosine had to be withdrawn when the candida proved to be resistant to this antifungal agent. However, the patient made an uneventful recovery. This seems to be the first report of a candida tropicalis meningitis in an otherwise healthy adult patient in this country.

Permanent perceptive deafness due to Streptococcus suis type II infection.

We report a case of meningitis and perceptive deafness caused by Streptococcus suis type II (Group R beta-haemolytic streptococcus). The illness is known to be an occupational hazard of pigmeat handlers and our patient worked in a pet food factory processing pig meat. His hearing loss was very severe and showed no improvement despite adequate antibiotic treatment. The organism that was isolated is the first to show penicillin resistance. The implications of this for antibiotic treatment are discussed.

In vitro combination of mecillinam with cephradine or amoxycillin for organisms resistant to single agents.

Of 36 multi-resistant Gram-negative bacilli isolated from different cases of urinary tract infection, 18 strains of Escherichia coli, klebsiella, enterobacter, proteus, providencia and serratia with the exception of Acinetobacter species showed synergy when mecillinam was combined with cephradine or amoxycillin in the ratio of 1:1. Combination with cephradine was synergistic on sixteen occasions and amoxycillin on six. Synergy, which might have clinical relevance, was demonstrated only when the test organism was either fully or moderately sensitive to one of the two antibiotics combined but not when resistant to both.
