Reconstruction of the horizontal palpebral aperture after failed ectropion surgery with temporal migration of punctum.

Involutional ectropion of the lower lid is the result of progressive stretching and elongation of the lid margin and medial and lateral canthal tendons. The relative laxities of the components of the lower lid-canthal tendon complex will determine the location and extent of the ectropion. Whereas inadequate canthal tightening or horizontal lid shortening will result in recurrent ectropion, overzealous lid shortening without tendon plication will result in a noticeably narrower horizontal palpebral fissure, a persistent ectropion, or temporal migration of the punctum. We used lateral cantholysis, medial canthal tendon plication, and punctal rotation to return the punctum to its normal position and temporalis muscle to support the lower lid in eight cases in which previous procedures had failed to correct the lid malposition. In all eight cases there was improvement of the lid position. In one case of severe medial ectropion, the punctal eversion was not completely corrected. This technique is not recommended as an initial procedure for ectropion repair. It is only used to manage previous surgical failures. It is a new application and combination of well-accepted techniques.

Sexual counselling of diabetic impotence.

A study of 54 diabetic men was conducted to determine the incidence of impotence, the nature of the sexual counselling they received, and their referral routes. Health providers associated with diabetes care also were interviewed to determine how they perceived professional responsibility for counselling about sexual dysfunctions in their clients. In the diabetic men studied, 48.1% had experienced primary impotence and 16.7% secondary impotence. Only 11.5% of the diabetic men received any sexual counselling, which each had initiated because of their personal concerns. Generally neither information about sexual dysfunctions nor even anticipatory warnings were given by members of the care-giving team. A discussion of impotence including etiology for the diabetic patient and sexual therapy as it relates to patient counselling is explored. Inclusion of this type of counselling for the male diabetic can contribute to continuity of care.

Primary culture of supporting cells from bovine testes.

Supporting cells from 6- to 8-wk-old bovine calf testes were maintained in cell culture and were characterized morphologically and biochemically. Light and electron microscopy showed that the cultures consisted principally of supporting cells that were identified as epithelial cells with nucleolar vesicles. The bovine supporting cells in culture responded to follicle stimulating hormone with increased cAMP synthesis and enhanced incorporation of [3H]leucine into secreted proteins. No androgen binding protein could be detected in the medium.

Effectiveness of an outpatient urine screening program.

We evaluated the effectiveness of a routine outpatient urinalysis screening program on a sample population of 2600 patients. The 189 abnormal urine results found in 182 patients were followed up by study of any new clinical and laboratory investigations or therapeutic modifications initiated on the basis of any abnormal test result. The urinalysis screening program appeared to have significant bearing on diagnosis or treatment in only 13 patients. Abnormalities found in 150 of the 182 patients were either not noted or no further positive action was taken. Thus we concluded that under the conditions of this study the urine screening program added to hospital costs without significant benefit to the patient.

Urinalysis by use of multi-test reagent strips: two dipsticks compared.

We compared Ames' "N-Multistix" with Boehringer's "Combur-8" ("Chemstrip-8") multi-test urine reagent strips by analysis of contrived urine specimens, testing accuracy, precision, specificity, and limits of detection of both products. Relative cost and ease of use were also examined. Each brand of urinary dipstick had specific advantages but it is unlikely that patient care would be adversely affected by preferential use of either product.

Computer printing and filing of microbiology reports. 1. Description of the system.

From March 1974 all reports from this microbiology department have been computer printed and filed. The system was designed to include every medically important microorganism and test. Technicians at the laboratory bench made their results computer-readable using Port-a-punch cards, and specimen details were recorded on paper-tape, allowing the full description of each specimen to appear on the report. A summary form of each microbiology phrase enabled copies of reports to be printed on wide paper with 12 to 18 reports per sheet; such copies, in alphabetical order for one day, and cumulatively for one week were used by staff answering enquiries to the office. This format could also be used for printing allthe reports for one patient. Retrieval of results from the files was easily performed and was useful to medical and laboratory staff and for control-of-infection purposes. The system was written in COBOL and was designed to be as cost-effective as possible without sacrificing accuracy; the cost of a report and its filing was 17-97 pence.
